    Present empirical and culture guided antibiotic therapy of severe infection, particularly sepsis, in neonates and children, treated in the multidisciplinary paediatric intensive care unit, are ... described. Empirical therapy is based on many years of meticulous follow-up of causative organisms,both on admission and after 48 hours (hospital infection) and their sensitivity to antibiotics. The basis of effective culture guided antibiotic therapy is the use of correct, internationally accepted system of determination of sensitivity to antibiotics. Novelties in the antibiotic treatment of severe Gram positive, Gram negative and fungal infections are presented. Consultation with experienced paediatric infectious disease specialist is very important in the choice of most appropriate antibiotic in the critically ill child. The recently introduced limitation of special antibiotics in the University Medical Centre also contributes to the more rational antibiotic therapy.
