  • Kronična venska insuficienca in arterio-venske komunikacije = Arterio-venous communications associated with chronic venous insufficiency

    Although the presence of arteriovenous communications in patients with chronicvenous insufficiency has been confirmed by several studies in the past,their role in the pathogenesis of this condition ... is still unclear. On onehand, it is possible that they play a key role in its etiology, but there is also sufficient evidence that their development is secondary to chronic venous insufficiency. The aims of this study were: to prove the existence of arteriovenous communications in 34 patients with chronic venous insufficiency and to establish whether the microembolisation procedure in the muscular branches of the crural arteries can improve the healing of lower venous ulcersin the leg which were resistant to conservative and surgical treatment in 16 cases. This study has provided evidence that arteriovenous shunting is usual in patients with varicose veins and ulcers, and its existence can be demonstrated by digital substraction angiography (31/34 = 91%). Embolisation of the muscular arterial branches which feed the arteriovenous shunts with theuse of micro-spirals and microparticles seems to be an effective method forthe treatment of nonhealing venous ulcers in lower leg sections (13/16 = 81%).
