    The most common indications for anticoagulation therapy which are specificallyimportant in family medicine are atrial fibrillation, the presenceof a mechanical heart valve and venous thromboembolism. ... Treatment withwarfarin presents a problem if patients with these indications need surgery, because the interruption of anticoagulation increases the risk of thromboembolism. The paper presents the thrombosis risk of stopping anticoagulation in specific settings, bleeding risks of certain procedures andwarfarin and heparin pharmacokinetics. Recommendations are discussed regarding management of patients on anticoagtzlants who undergo invasive procedures.
