    Background: Premolar roots are characterized by a longitudinal groove of variable depth, which can complicate endodontic or periodontal treatment. The paper deals with variations in the shape of the ... root and root canal, dependingon the depth of the longitudinal groove and number of roots. Materialand methods: Twenty-one upper and twenty lower extracted premolars were x-rayed, horizontally dissected and examined in a stereomicroscope. Results: The roots of single-rooted premolars had a deep, bucco-distally oriented groove, which was wide in upper and narrow in lower teeth. On x-ray, the middle and apical root region was not clearly outlined. Double-rooted premolars had a deep groove on the buccal root, while threerooted premolars showed no grooves. Roots displaying a deep groove had a C-shaped root canal, occasionally separated by secondary dentin in two or three canals. Conclusion:A deep groove may be present on the roots of single- and double-rooted premolars, which is not accessible by periodontal instruments. Secondary dentin may cause additional variations in the shape and number of root canals, complicating endodontic treatment. An unclear x-ray image of the middle and apical root region alerts the dentist to the possible existence of a deep groove and/or presence of two root canals.
