    The article presents infrapopliteal percutaneous transluminal angioplasty. Technical advances have improved the initial results, mainly by decreasing therate of complications of these interventions. ... In most institutions, infrapopliteal PTA is a procedure that is still reserved for critical limb ischemia. Since patients with an occlusive disease of small vessels are at a high risk of restenosis, it is accepted that angioplasty of small vessels is not worthwhile in the case of claudication. Short-term patency suffices for the disappearance of pain at rest and/or ulcer healing. Subsequently, clinicalsuccess is maintained for longer than angiographic patency. Infrapopliteal PTA is a safe procedure when performed by a skilled interventional radiologist. It can be performed without any particular contraindications in patients with coronary disease, nephropathy or high surgical risk.
