  • Zapleti akutnih in kroničnih vnetij v področju ušes, obnosnih votlin, ustne votline in žrela = Complications of acute and chronic inflammations in the ENT region
    Podboj, Jernej
Introduction: Acute and chronic inflammations of the ear, paranasal sinuses and throat are the most common diseases in all age groups. Even though the efficacy of antibiotic treatment has improved, ... infection can spread and resultin serious local, regional and intracranial complications. Physicians must be aware of the risk of such complications, recognize them in time and urgently start appropriate diagnostic procedures and therapy. Complications ofacute and chronic inflammation of the ear can be divided into extracranial (acute and masked mastoiditis, petrositis, paresis or paralysis of the facial nerve, labyrinthitis) and endocranial (meningitis, extradural, subdural and brain abscess, thrombophlebitis and thrombosis of the sigmoid sinus) complications. CT and MRI are the most important diagnostic tools, while antibiotic therapy and surgery are the main treatment options. Complications of sinusitis can be classified as local (abscess and osteomyelitis of the maxilla and frontal bone), orbital (periorbital edema, orbital cellulitis, subperiostal and orbital abscess, thrombosis of the cavernous sinus) and endocranial (meningitis, epidural, subdural and brain abscess). Diagnosis is made by endoscopy, CT and MRI. Surgery supported with antibiotics is the therapy of choice. Complications of infections of the mouth and throat: Peritonsillar abscess and acute cervical lymphadenitis are the most common local complications. Less common ones include Ludwig's angina and para- and retropharyngeal abscess. Ultrasound of the neck is important for diagnosis. Antibiotic treatment is combined with incision and drainage. Conclusions: Primary care physicians and ENT surgeons must always be aware of the risk of complications during infections in the ENT region. Early evaluation and management of these patients is paramount for successful treatment and prognosis.
