    Atrial fibrillation is the most common sustained cardiac arrhythmia. Atrial flutter is less common, less stable and unpredictable. The mechanism of both arrhythmias is via re-entry. In fibrillation, ... there are several re-entry circuits in the atria; flutter, however, is based on a single re-entry circuit. Paroxysmal atrial fibrillation is triggered by premature atrial beats, most often from the orifices of the pulmonary veins. Increased atrial size, inflammation, and toxic and hormonal influences all promote atrial arrhythmias. Atrial flutter can be successfully treated by radiofrequency ablation. Experimentally, the same is true of paroxysmal atrial fibrillation, but the success rate is much lower. Therefore, antiarrhythmic drugs are still the treatment of choice. Permanent atrial fibrillation is best managed by regulating ventricular response and anticoagulation.
