    Arterial hypertension is the primary manageable risk factor for cerebrovascular disorders, such as hemorrhagic and ischemic stroke. Regardlessof age, the presence of diastolic hypertension greatly ... increases thelikelihood of stroke. Among older patients, isolated systolic hypertension also contributes to stroke. The successful management of arterial hypertensionis one of the leading causes for the decline in strokes in the USAand western European countries. However, this is not the case in eastern European countries, where arterial hypertension has not been treated effectively, as was shown in the Monica Trial. Management of arterial hypertension is important for primary and secondary prevention as well. Recentevidence suggests that newer ACE inhibitors may have additional benefitsfor stroke prevention, probably due to the stabilizing of vascular endothelium, which in turn might lower the risk of thromboembolism. Therefore,arterial hypertension should be managed by all means available. However, in the early phase of ischemic stroke, blood pressure should be kept high enough to secure effective cerebral blood flow. In other words, the earlyphase of stroke is the only period during which arterial hypertension should not be treated.
