Almost a year after the declaration of the pandemic due to the SARS-CoV-2 virus which causes the COVID-19 disease and the need to contain the progression and treatment, the promising option was ...designing an effective and safe vaccine to reach a state of massive immunity. The first vaccine approved was the one produced by Pfizer–BioNTech, and its application started in December 2020. Within days of the first applications, 0.2% of adverse events were reported. Herein, a series of 26 cases with the manifestation of adverse events related to the application of the first dose of the BNT162b2 vaccine from Pfizer–BioNTech in healthcare workers from Mexico. Of these cases, only five patients were classified with a certainty of anaphylaxis; two of them presented seizures, and their management is described individually. After the examination of all the cases, the symptoms were resolved. In Mexico and around the globe, the vaccination process continues, and the report of possible AEFIs is still needed to contribute to the pharmacovigilance of this new vaccine and improve its safety profile.
Hyper-IgE syndrome (HIES) is a rare primary immunodeficiency characterized by elevated levels of immunoglobulin E (IgE), eczematous dermatitis, cold abscesses, and recurrent infections of the lung ...and skin caused by Staphylococcus aureus. The dominant form is characterized by nonimmunologic features including skeletal, connective tissue, and pulmonary abnormalities in addition to recurrent infections and eczema. Omalizumab is a humanized recombinant monoclonal antibody against IgE. Several studies reported clinical improvement with omalizumab in patients with severe atopic eczema with high serum IgE level. We present the case of a 37-year-old male with HIES and cutaneous manifestations, treated with humanized recombinant monoclonal antibodies efalizumab and omalizumab. After therapy for 4 years, we observed diminished eczema and serum IgE levels.

Resumen: Introducción: la inmunodeficiencia común variable se clasifica en el grupo III de IX, donde se incluyen las inmunodeficiencias con predominio de deficiencia de anticuerpos. La mutación en el ...gen TNFSF13B es responsable de codificar la proteína TACI y provoca una expresión clínica variable que acompaña a la enfermedad. Reporte de caso: paciente femenino de 25 años de edad que inicia padecimiento desde el nacimiento, estudiada por diarrea crónica sin etiología definida; al agudizarse la sintomatología presenta evacuaciones disminuidas en consistencia y rectorragia. Se realiza el diagnóstico de inmunodeficiencia común variable y se inicia el tratamiento de sustitución con inmunoglobulina intravenosa a dosis de sustitución cada cuatro semanas con mejoría clínica. En la secuenciación de ADN de siguiente generación (NGS) de exoma completo se identifica una variante en TNFRSF13B en el exón 3 c.310T>C; p.cys104Arg. Conclusiones: la presentación clínica de la inmunodeficiencia común variable es diversa. Dentro del estudio clínico de un paciente con la sospecha de esta enfermedad es primordial tomar en cuenta la presencia y evolución de los síntomas gastrointestinales. El diagnóstico molecular de los errores innatos de la inmunidad contribuye a esclarecer la fisiopatología de este grupo de enfermedades.
Vaccination against SARS-CoV-2 began in December 2020 and with this strategy, the course of the pandemic would be modified. BNT162b2 (Pfizer–BioNTech), mRNA-1273 (Moderna), and ChAdOx1 nCoV-19 ...(Oxford–AstraZeneca) were the main vaccines that demonstrated a reduction in hospitalization and death from SARS-CoV-2 in clinical trials. Post-vaccination infection by COVID-19 is called breakthrough infection. Breakthrough infection is more frequent in health workers, and smokers, and critical illness is more frequent in patients with advanced age, and cardiovascular disease. Following vaccination efforts around the world, the incidence of severe COVID-19 infection has decreased, declaring the end of the pandemic by the World Health Organization. However, efforts to keep vaccination active, especially in at-risk populations, must continue.
