As a non-destructive evaluation method for friction stir welded joints, this research aims to develop and corroborate a method for material flow analysis and defect detection based on X-ray computed ...tomography (X-ray CT). Using a cylindrical FSW tool with a broad shoulder, joints of dissimilar materials AA6061-T6/AZ31b are friction welded employing tool rotary speed ranging from 1000 to 1500 RPM and tool feed from 125 to 400 mm/min. The welded joints are scanned via X-Ray CT with an image bit depth of 16-bit then segmented based on the Hounsfield Units scale (HU) and the global Otsu thresholding method. This segmentation divides the DICOM images into masks for each different material, from which 3D renderings are generated to record volumetric data. For analyzing elemental mixing, measurements of material penetration and transfer are carried out. Corroborating these results was accomplished using destructive cross-sectioning and Energy Dispersive X-ray Spectroscopy (EDX). The results show that this method can detect internal defects and characterize the material mixing with results comparable to that of destructive EDX analysis. The effect of improving scan resolution on reconstructed images was shown to slightly improve the accuracy of the thresholding method while reducing the standard deviation of segmented material ranges.

Some mathematical and algorithmic basics were contributed for narrative description of information and communications technologies (ICT) architectural deployment could be used for outer regulation of audience response character by Social Media Marketing (SMM) principles. That could be performed by controlled distribution of specified digital content that contains respective key phrases, for example social advertisements and analyzing respective feed-backs. Some results of the empiric study of live audience response dependence on controlled impacts are discussed. Election processes data and recent media recordings for preliminary proof of the contributed concept feasibility have been analyzed. There were shown using gathered empiric data sets, that the extent of impacts to targeted audience response intensity could be the subject of outer regulation. The index has been contributed for assessment the efficiency of the impact’s propagation inside the audience by calculation of row correlation of keyword occurrence and audience response intensity. The approaches suggested in the article can be useful both for building effective interactive systems of state-society interaction and for detecting manipulative traits when influencing a specific audience
Solitary plasmacytoma is a rare neoplasm of monoclonal plasma cells that falls along the spectrum of plasma cells dyscrasias. Unlike bone plasmacytomas, extramedullary plasmacytomas occur outside the ...bone marrow in any soft tissue. The aerodigestive tract is the most common site, but any organ, including the parotid gland, may be rarely involved. Extramedullary plasmacytoma lacks the defining features of multiple myeloma or medullary plasmacytoma. Here, we report a case of a 60-year-old man who presented with a mass in the retro-auricular area of 2 months’ duration. On physical examination, a 2-cm mass was palpated in the right parotid area with no palpable cervical lymph nodes. Fine-needle aspiration cytology showed atypical cells. MRI of the head and neck showed a 2x2-cm mass in the parotid tail with features suggestive of muco-epidermoid carcinoma and absence of lymphadenopathies. Total parotidectomy was performed. Histopathological evaluation revealed monoclonal plasma cells expressing kappa immunoglobulin light chains. To complete the workup, serum protein electrophoresis, immunofixation, quantitation of immunoglobulins and urine protein electrophoresis, and Bence-Jones were normal. A whole-body 18F-FDG PET/CT scan did not reveal any other lesions. Bone marrow aspirate and biopsy demonstrated less than 3% clonal plasma cells. A complete blood count showed a normal hemoglobin level. Chemistry screen revealed normal calcium and creatinine levels. A diagnosis of solitary plasmacytoma of the parotid gland was made, and the patient received post-operation radiation therapy to the right parotid area. Because less than 7% of patients with solitary medullary plasmacytoma will develop local recurrence and approximately 10–15% of patients will ultimately develop multiple myeloma, the patient was followed up by laboratory tests and imaging at periodic intervals; he is still in complete remission. Although patients with parotid masses are commonly observed in our practice, parotid plasmacytoma is rare. Despite its rarity, it should be considered in the differential diagnosis of parotid gland tumors. Biopsy, blood tests, and imaging are important in the accurate diagnosis and management of this neoplasm.
Myxoid liposarcoma (MLPS) is the second most common type of LPS after the well differentiated LPS. MLPS is primarily localized to the extremities. The incidence of LPS is ~2 per million worldwide. ...MLPS accounts for ~30% of all LPS cases. MLPS is usually encountered in adults, but can also occur in younger individuals more than other types of LPS. MLPS can be divided into low- and high-grade subtypes, which present with differences in patient prognosis and outcome. Methods of tumor management include surgery, radiotherapy and chemotherapy; however, there is no unified treatment based on tumor characteristics alone. The present manuscript reviews the surgical management, radiotherapeutic and chemotherapeutic approaches reported in the literature for different types of MLPS in the extremities, as well as the post-treatment outcomes. In addition, the present review provides an evidence-based management plan for MLPS in the form of an organogram based on specific tumor and patient parameters. Thyroid storm is a life-threatening condition characterized by a high level of circulating thyroid hormones and harbors high mortality and morbidity, even if diagnosed and treated early. The ...condition is frequently overlooked and under-recognized in emergency departments owing to its rarity. Here, we present a case of a 24-year-old male patient, previously healthy, who presented with cardiac arrest and was found to have heart failure and high thyroid hormone levels after investigations. Consequently, the presentation was attributed to thyroid storm. His clinical status and cardiac function improved after treatment of the hyperthyroidism.
BACKGROUND Arthrogryposis multiplex congenita is a multifactorial syndromic or non-syndromic group of conditions consisting of multiple congenital contractures of the body, of unknown etiology. It is ...associated with a heterogenous group of disorders that include but are not limited to processes such as myopathic and neuropathic. Neural tube defect is a neuropathic disorder that incorporates myelomeningocele that might be either isolated or within a spectrum of multiple diseases. CASE REPORT This is a case report of a 28-day-old male born with lower limb arthrogryposis with myelomeningocele and Chiari II malformation in a Mediterranean population. CONCLUSIONS Lower extremity arthrogryposis with myelomeningocele and Chiari II malformation is a prenatal diagnosis that requires high clinical suspicion, early multidisciplinary intervention, and genetic counselling. As long as new approaches are being explored in the management of such cases, babies born now with neural tube defects can expect better quality of life.
Abstract Twenty-five consecutive patients who underwent the Wada test using propofol as anesthetic were compared with 15 randomly selected patients who were tested using amobarbital. Time to verbal ...and nonverbal responses and time to motor power 3/5 did not differ between the two groups ( P > 0.05). The number of doses received by each patient and the percentage of patients needing more than one dose were significantly greater in the propofol group ( P < 0.005). Only one patient developed confusion, combativeness, and agitation. Despite the need for multiple doses, our patients had no residual drowsiness within 10 to 15 minutes of the propofol injection. This allowed us to perform the test on both sides on the same day separated only by 45 minutes. Propofol is an effective alternative to amobarbital in the Wada test, and may be used successfully in multiple repeated injections within the same test without significant residual sedation or significant adverse effects.

It was briefly shown a change of cloud infrastructure to set up Data Embassies as the main part of e-Gov distributed service delivery platform. There were analyzed the main features of digital continuity support aspect. Thus, there the substantiation of an optimal topological solution is done for data embassies cloud info-communication system. It was shown that to choose the most appropriate telecommunication system network design on the IaaS and PaaS planes it is useful to adopt concrete topological design realization of distributed cloud in accordance with solution of presented equations, using initial topological network parameters. These approaches are good for purposes of finding the ways to organize effectively the system of distributed Data Centers plurality, and respective their inner SaaS or/and PaaS realizations, when optimization task object will consist of the structured set of handlers or virtual machines, respectively.
