P. aeruginosa
bloodstream infections (BSI). Their impact on outcome and antibiotic susceptibility was analyzed. BSI which consecutively occurred at our institution during a 70-month period were evaluated and correlated with type of pathogen, status of underlying disease, neutropenia, previous antibiotic therapy, resistance to antibiotics, and outcome. During the observation period, 441 BSI were recorded. Frequency of Gram-negative BSI was higher than that of other pathogens (57.3%). Overall, 66

BSI were recorded; 22 out of 66 were multiresistant (MR
P. aeruginosa
). Thirty-day mortality for all BSI was 11.3%; it was 27.3% for
P. aeruginosa
BSI and 36.4% for MR

. At multivariate analysis, only active hematological disease and
P. aeruginosa
BSI were associated to an increased risk of death. For MR
P. aeruginosa
, BSI mortality was 83.3% vs. 18.8% when empiric therapy included or not an antibiotic with in vitro activity against
P. aeruginosa
(
p
= 0.011). Together with active disease, the emergence of
P. aeruginosa
BSI, particularly if multiresistant, was responsible for an increased risk of death among hematological patients at our institution. In this scenario, reconsidering the type of combination antibiotic therapy to be used as empiric treatment of neutropenic fever was worthwhile.
Héritant de plusieurs générations d’érudits et d’archéologues modernes et contemporains, la recherche sur les aqueducs romains de Lyon a connu, après la thèse de Camille Germain de Montauzan (1908), ...deux nouvelles phases de progression lors du dernier tiers du xxe s. Louis Jeancolas et Jean Burdy, respectivement ingénieur des Ponts et Chaussées et professeur agrégé en Sciences physiques puis docteur ès lettres avec les aqueducs de Lyon pour sujet, sillonnent la campagne : ils cartographient des centaines de points de découvertes et de destructions catastrophiques tout au long de quatre longs tracés représentant plus de 200 kilomètres linéaires de canaux. Les décennies 1980 et 1990 voient l’émergence de l’archéologie préventive et la publication des monographies de J. Burdy dédiées aux quatre grands aqueducs lyonnais (Mont d’Or, Yzeron, Brévenne et Gier). S’ensuivent plusieurs décennies de recherche et une accumulation de données nouvelles avant que ne se mette en place un cadre dédié aux échanges. À partir de 2015, une dizaine de journées d’étude organisées sous l’égide de l’UMR 5138 ArAr rassemble ainsi tous les chercheurs de l’archéologie lyonnaise ou régionale, professionnelle et associative, partageant données et expériences. L’organisation d’un colloque international en octobre 2020 n’est pas seulement l’aboutissement de ces travaux. L’objectif est également de réunir à Lyon, plus de 40 ans après les Journées d’étude sur les aqueducs romains de 1977, des spécialistes français et étrangers à même d’échanger et d’éclairer le cas lyonnais. Ce dossier Gallia en accueille les résultats. Plus de trente contributions sont rédigées par quarante-trois auteurs et rassemblées en quatre thèmes relatifs à la protection des tracés, à la construction, à la datation et à la desserte urbaine. Le panorama demeure incomplet et d’autres thématiques restent à ouvrir. Pour autant, ces nouveaux repères acquis collectivement figurent une avancée notable sur le sujet de l’hydraulique romaine que des approches encore inédites viendront plus tard enrichir.
The prognostic significance of CD20 expression in acute lymphoblastic leukemia has been investigated in children and adults but is still a subject of debate. The aim of our study was to correlate ...CD20 expression with clinical-biological characteristics and outcome in 172 Philadelphia chromosome negative patients prospectively treated in a multicenter trial introducing the molecular evaluation of minimal residual disease for therapeutic purposes. We considered 20% as the threshold for CD20 positivity. Complete remission rate, minimal residual disease negativity rate at weeks 10, 16 and 22, and disease-free and overall survival were similar among CD20-positive and -negative patients, even considering minimal residual disease results and related therapeutic choices. Our study failed to demonstrate any prognostic significance for CD20 expression in Philadelphia chromosome negative acute lymphoblastic leukemia. This conclusion is supported for the first time by a comparable minimal residual disease response rate among CD20-positive and -negative and positive patients.
We present a case of a patient with a three-month history of peripheral blood cytopenia without a confirmed diagnosis of myelodysplastic syndrome, who developed a favourable-risk acute myeloid ...leukemia (AML), according to the European Leukemia Net (ELN) criteria. The patient achieved a complete remission with incomplete platelet recovery (CRi) after induction. The patient achieved the morphological CR after the first consolidation and completed the first-line treatment with a syngeneic stem cell transplantation (SCT). A disease relapse occurred after one year of CR (blast cell count in the bone marrow 15%), and the patient was offered a haplo-SCT, which he refused due to personal reasons. In this paper, we discuss the interplay between clinical and biological risk factors in non-high-risk AML patients and speculate that some old clinical risk factors (e.g., age of the patient, achievement of CR after induction, and previous history of myelodysplastic syndrome) may still impact on the treatment decision algorithm of some of these patients.
Background
Invasive fungal infections (IFIs) are considered a major problem among patients undergoing acute leukaemia (AL) induction treatment. PROphylaxis of Fungal invasive Infections in ...Leukaemia-Caspofungin (PROFIL-C) is a multicentre study aiming to assess the comparative yield of using caspofungin versus standard policy (SP) regimens and the overall impact of IFI in routine clinical care conditions.
Methods
All AL patients receiving IFI prophylaxis according to local SP were prospectively included in the study by Northern Italy Leukaemia Group (NILG) centres. To allow the comparison of caspofungin versus SP regimens as prophylaxis strategies, caspofungin treatment was assigned via a centralized randomized procedure. The study was registered at http://www.clinicaltrial.gov (NCT00501098).
Results
Over a 2 year period, 175 patients were included. The overall incidence of IFI was 32/175 (18.3%) 10/175 (5.7%) probable/proven and 22/175 (12.6%) possible, with no statistically significant differences between caspofungin-based versus SP-based regimens overall: 15/93 (16.1%) versus 17/82 (20.7%), relative risk (RR) 0.78, 95% confidence interval (CI) 0.42-1.46; probable/proven: 7/93 (7.5%) versus 3/82 (3.7%), RR 2.06, 95% CI 0.55-7.7; possible: 8/93 (8.6%) versus 14/82 (17.1%), RR 0.5, 95% CI 0.22-1.14. Only one IFI-related death was recorded (10%).
Conclusions
The incidence and mortality of IFI were lower than expected in this strictly sequential cohort representative of the routine care in the NILG network. The efficacy and safety of caspofungin were similar to other prophylactic regimens.
Dans ses aspects aussi bien théoriques que matériels, le système du vote dans les mondes grec et romain a depuis longtemps été exploré au sein d’études plus générales sur les institutions ou les ...différents types de régimes politiques. Il n’a cependant jamais fait l’objet de publications réunissant à la fois les témoignages textuels et les résultats des fouilles archéologiques, dans l’optique d’une compréhension globale de cette pratique. De ce constat est né le projet d’une synthèse portant sur les modalités, les lieux et les finalités du vote en Grèce, à Rome et en Gaule, dans une perspective comparatiste. Menée dans le cadre d’un programme de recherche interdisciplinaire soutenu par l’université Lumière Lyon 2 et la Maison de l’Orient et de la Méditerranée, cette recherche a suscité, selon les régions et les périodes concernées, des questionnements spécifiques mais elle a aussi fait émerger des points de convergence. La collaboration de chercheurs issus de plusieurs disciplines – l’histoire, la philologie et l’archéologie – a permis de cerner la pratique du vote à travers ses implications politiques, ses modalités procédurales et la place qui lui a été réservée dans l’espace civique par les différentes sociétés antiques qui l’ont mise en œuvre. Le présent ouvrage, qui présente une synthèse sur chacune des aires géographiques étudiées et rassemble vingt et une contributions issues de séminaires ou de journées d’études qui se sont tenus à Lyon, à la Maison de l’Orient et de la Méditerranée, de la fin de l’année 2012 au printemps 2014, propose une approche inédite de l’acte de vote dans l’Antiquité.
