A Doença Falciforme (SS) é considerada de maior risco para vasculopatias crônicas em SNC culminando, geralmente, com eventos de acidente vascular cerebral isquêmico (AVCI). Ocorrem lesões das grandes ...artérias do círculo de Willis, por hiperplasia da camada íntima e proliferação muscular e de fibroblastos, culminando na formação de estreitamentos segmentares na luz de grandes e médias artérias. Aos 20 anos de idade, 11% e, aos 45 anos, 24% dos pacientes já tiveram AVCI. A maior incidência ocorre nas crianças entre 2 a 9 anos, voltando a aumentar após os 20 anos, sendo a média das idades em 13,8 anos. O risco de isquemia cerebral (sintomática ou silenciosa) no decorrer da vida do paciente é de 30%. Pacientes com dupla heterozigose das hemoglobinas S e C – DF(SC) seguem curso clínico mais brando, sendo muito raro eventos macrovasculares.
Sexo masculino, 18 anos, com DF(SC), com várias internações prévias, cerca de 3-5/ano dos 6 aos 10 anos de idade, posteriormente com uma internação ao ano, por crises dolorosas. Aos 8 anos, submetido à colecistectomia. Aos 13 anos, relatou a presença de diminuição discreta da movimentação do membro superior esquerdo (MSE) observado há 3 meses, com piora progressiva da força muscular, relatando dificuldade para pegar objetos, tremores na mão esquerda ao levantar peso, dificuldade para abrir e fechar a mão e estender o membro. Ao exame neurológico: hemiparesia à esquerda desproporcionada, força grau IV em todos os membros, porém diminuída em região distal de MSE (grau III). Rigidez articular distal, rotação incompleta do ombro esquerdo, tônus adequado, discreta hipotrofia de musculatura interfalangeana à esquerda . Marcha atípica com hiperreflexia em MSE e hipertonia em hemicorpo esquerdo. AngioRM crânio: Alteração de sinal no centro semioval direito associada à discreta atrofia que deve corresponder à área sequelar por evento isquêmico, com comprometimento focal das fibras do trato corticospinal. Introduzido hidroxiuréia, sem aumento de HbF e elevação dos valores de Hb, sendo suspenso. Iniciado transfusões de troca mensais, porém não foi possível atingir o alvo de HbS < 30%. Como apresenta hemoglobina basal de 13–14g/dL, optado por manter apenas esquema de sangrias mensais com o objetivo de reduzir viscosidade sanguínea.
A dupla heterozigose com HbS e HbC – DF(SC) determina uma doença mais branda: quadro hemolítico menos intenso, eventos agudos microvasculares menos intensos e macrovasculares (principalmente em SNC), bastante raros. Alguns estudos relataram que velocidades de fluxo sanguíneo ao doppler TC > 128 cm/s são anormalmente elevadas para pacientes SC e devem ser monitorados. A terapia com hidroxiuréia mostrou-se benéfica em alguns casos, diminuindo a taxa de eventos vaso-oclusivos. O aumento de hemoglobina pode levar a síndrome de hiperviscosidade, sendo indicada neste caso a flebotomia terapêutica.
Pacientes com Doença Falciforme (SC) podem apresentar as mesmas complicações que as formas mais severas, mas com frequência reduzida e, um pequeno subconjunto de indivíduos com DF(SC) tem fenótipo clínico semelhante ao SS. Futuros estudos são necessários para avaliar risco de AVCI em formas mais brandas da doença.
O metotrexato (MTX) é um antimetabólito que inibe a síntese, reparo e replicação celular do DNA, inibindo a diidrofolato redutase. Indicado para o tratamento de diversas neoplasias, pode ser ...administrado oral, endovenoso, intramuscular, subcutâneo ou intratecal (IT). Quando administrado IT, pode ocasionar neurotoxicidade, classificada em aguda, subaguda e de longo prazo, se manifestam como meningite asséptica, mielopatia transversa, encefalopatia aguda/subaguda e leucoencefalopatia.
Paciente sexo feminino, 3 anos. Iniciou seguimento em nosso serviço, com diagnostico de leucemia linfoblástica aguda, tratada com protocolo BFM-2009. Quatro dias após receber primeiro MTX IT, evoluiu com cefaleia intensa recorrente, melhora temporária ao uso de analgésicos, acompanhada de fotofobia, com duração de 2 dias. Solicitado TC crânio, fundoscopia e líquor: sem alterações. Após segundo MTX IT, apresentou dois episódios convulsivos, com intervalo de uma hora, caracterizados por movimento mastigatório com eversão ocular e liberação esfincteriana, administrado diazepam endovenoso. Evoluiu com sonolência, hiporeatividade, choro incoercível, paresia de MSD e MMII, deficit na fala e amaurose interrogada. TC crânio: Extensa alteração bilateral dos coeficientes de atenuação do córtex e substância branca subcortical dos lobos parietais e occipitais. As características de imagem, sugerem síndrome de encefalopatia posterior reversível relacionado ao quimioterápico intratecal. RM crânio: mantido alterações anteriormente descritas, sugerindo síndrome de vasoconstrição associado ao uso de quimioterápico intratecal. Fundoscopia: papiledema bilateral +/4+. Líquor: sem alteração. Administrado Aminofilina 2,5 mg/kg de 6/6 horas endovenoso, durante o quadro agudo e associado as quimioterapias intratecais seguintes. Atualmentena fase de manutenção, evoluindo com melhora lenta e progressiva, senta sem apoio, engatinha, fala e visão preservadas, porém com dificuldade de deambular.
MTX intratecal é comumente associado à meningite asséptica. Raramente associada à síndrome de leucoencefalopatia reversível posterior, convulsões, déficits neurológicos focais subagudos, polirradiculopatia lombossacral, edema pulmonar não cardiogênico, pneumonite e morte súbita. Não existe um tratamento padrão para qualquer uma dessas neurotoxicidades, que estão relacionadas ao acúmulo de adenosina e homocisteína com efeito excitatório no receptor N-metil-D-aspartato, do qual a aminofilina é antagonista, e portanto utilizada para prevenção. A descontinuidade do MTX deve ser discutida e individualizada.
Em estudo, a encefalopatia subaguda se desenvolve em 3,8% das crianças tratadas com MTX. Portanto, diagnóstico e tratamento precoce são fundamentais.
Os infartos cerebrais em indivíduos com doença falciforme (DF), em geral diferem da etiologia da população geral, pois devem-se a hemodinâmica cerebral alterada em função da hemólise crônica, baixas ...concentrações de hemoglobina (Hb), desrregulação vasomotora e estado pró-inflamatório. Acidente vascular cerebral (AVC) isquêmico prevalece em relação ao hemorrágico e os locais frequentemente acometidos incluem grandes territórios de vasos, como artérias cerebrais anterior, média e posterior.
Sexo masculino, em seguimento com doença falciforme (SS), diagnosticado em triagem neonatal. Aos 9 meses, internado por quadro de febre diária não aferida, coriza e tosse produtiva há 1 semana, acompanhado de desconforto respiratório, inapetência e vômito pós tosse. Colhido sp e urina para culturas e iniciado antibioticoterapia empírica. Durante internação, evoluiu com palidez cutânea (Hb 3,5 apresentou queda > 2g/dL e reticulocitose 12%), esplenomegalia (3 cm RCE) e instabilidade hemodinâmica, instalando quadro de sequestro esplênico agudo. Recebeu hidratação e concentrado de hemácias. Na evolução, apresentou um único episódio de crise convulsiva focal disperceptiva com generalização. Ao exame neurológico apresentava hipertonia e clonias em mmss, com desvio do polo cefálico para esquerda e sialorreia, com duração de 3 minutos, interrompida com diazepam. TC de crânio: área hipodensa em hemisfério cerebelar esquerdo, associado a discreto efeito expansivo regional, decorrente de evento vascular isquêmico recente no território da artéria cerebelar superior. EEG: área irritativa focal em hemisfério cerebral esquerdo. Não foi possível coletar líquor, ajustado ceftriaxone para cobertura de infecção em SNC. Paciente evolui sem novas crises convulsivas, discreto comprometimento motor transitório em MSD, sem outros sinais aparentes de liberação piramidal. Atualmente está em uso de hidroxiuréia, esquema de transfusão de troca contínua (para manter Hb > 9.0 e HbS < 30%), sem sequelas motoras. EFHb: A1 65,5% A2 2,5% F 8,5% S 23,5%. AngioRM: tênues focos de hipersinal no hemisfério cerebelar esquerda, podendo representar áreas de gliose sequelares. Irregularidade dos contornos da artéria vertebral direita e junto à emergência da artéria cerebelar póstero-inferior deste lado, porém mantendo calibres e sinal de fluxo normais.
A sintomatologia de AVCI em lactentes se apresentam geralmente com fraqueza focal, convulsões e alteração do estado mental. Na vigência de febre e achados neurológicos agudos, deve tratar presumivelmente para infecção bacteriana até que esta possibilidade seja descartada. Na suspeita clínica de um AVC agudo, antes mesmo da realização de exames por imagem, transfusão simples de CH deve ser administrada de imediato.
AVCI é complicação incomum em crianças durante o primeiro ano de vida. É relatado que decréscimos agudos em valores sanguíneos de hemoglobina é fator de risco para AVCI em pacientes com doença falciforme SS. O quadro de sequestro esplênico agudo que precedeu o quadro em SNC pode ter favorecido o evento.

The discovery of the first transiting hot Jupiters (HJs), giant planets on orbital periods shorter than P ~ 10 days, was announced more than 20 years ago. As both ground- and space-based follow-up ...observations are piling up, we are approaching the temporal baseline required to detect secular variations in their orbital parameters. In particular, several recent studies have focused on constraining the efficiency of the tidal decay mechanism to better understand the evolutionary timescales of HJ migration and engulfment. This can be achieved by measuring a monotonic decrease in orbital period dP/dt < 0 due to mechanical energy being dissipated by tidal friction. WASP-12b was the first HJ for which a tidal decay scenario appeared convincing, even though alternative explanations have been hypothesized. Here we present a new analysis based on 28 unpublished high-precision transit light curves gathered over a 12-yr baseline and combined with all the available archival data, and an updated set of stellar parameters from HARPS-N high-resolution spectra, which are consistent with a main-sequence scenario, close to the hydrogen exhaustion in the core. Our values of dP/dt = −30.72 ± 2.67 and Q′* = (2.13 ± 0.18) × 105 are statistically consistent with previous studies, and indicate that WASP-12 is undergoing fast tidal dissipation. We additionally report the presence of excess scatter in the timing data and discuss its possible origin.

Context.
TOI-2076 is a transiting three-planet system of sub-Neptunes orbiting a bright (
G
= 8.9 mag), young (340 ± 80 Myr) K-type star. Although a validated planetary system, the orbits of the two ...outer planets were unconstrained as only two non-consecutive transits were seen in TESS photometry. This left 11 and 7 possible period aliases for each.
Aims.
To reveal the true orbits of these two long-period planets, precise photometry targeted on the highest-probability period aliases is required. Long-term monitoring of transits in multi-planet systems can also help constrain planetary masses through TTV measurements.
Methods.
We used the MonoTools package to determine which aliases to follow, and then performed space-based and ground-based photometric follow-up of TOI-2076 c and d with CHEOPS, SAINT-EX, and LCO telescopes.
Results.
CHEOPS observations revealed a clear detection for TOI-2076 c at $P = 21.02538_{ - 0.00074}^{ + 0.00084}$ d, and allowed us to rule out three of the most likely period aliases for TOI-2076 d. Ground-based photometry further enabled us to rule out remaining aliases and confirm the
P
= 35.12537 ± 0.00067 d alias. These observations also improved the radius precision of all three sub-Neptunes to 2.518 ± 0.036, 3.497 ± 0.043, and 3.232 ± 0.063

⊕
. Our observations also revealed a clear anti-correlated TTV signal between planets b and c likely caused by their proximity to the 2:1 resonance, while planets c and d appear close to a 5:3 period commensurability, although model degeneracy meant we were unable to retrieve robust TTV masses. Their inflated radii, likely due to extended H-He atmospheres, combined with low insolation makes all three planets excellent candidates for future comparative transmission spectroscopy with JWST.

The CHaracterising ExOPlanet Satellite (CHEOPS) was selected on October 19, 2012, as the first small mission (S-mission) in the ESA Science Programme and successfully launched on December 18, 2019, ...as a secondary passenger on a Soyuz-Fregat rocket from Kourou, French Guiana. CHEOPS is a partnership between ESA and Switzerland with important contributions by ten additional ESA Member States. CHEOPS is the first mission dedicated to search for transits of exoplanets using ultrahigh precision photometry on bright stars already known to host planets. As a follow-up mission, CHEOPS is mainly dedicated to improving, whenever possible, existing radii measurements or provide first accurate measurements for a subset of those planets for which the mass has already been estimated from ground-based spectroscopic surveys. The expected photometric precision will also allow CHEOPS to go beyond measuring only transits and to follow phase curves or to search for exo-moons, for example. Finally, by unveiling transiting exoplanets with high potential for in-depth characterisation, CHEOPS will also provide prime targets for future instruments suited to the spectroscopic characterisation of exoplanetary atmospheres. To reach its science objectives, requirements on the photometric precision and stability have been derived for stars with magnitudes ranging from 6 to 12 in the V band. In particular, CHEOPS shall be able to detect Earth-size planets transiting G5 dwarf stars (stellar radius of 0.9

) in the magnitude range 6 ≤
V
≤ 9 by achieving a photometric precision of 20 ppm in 6 hours of integration time. In the case of K-type stars (stellar radius of 0.7
R
⊙
) of magnitude in the range 9 ≤
V
≤ 12, CHEOPS shall be able to detect transiting Neptune-size planets achieving a photometric precision of 85 ppm in 3 hours of integration time. This precision has to be maintained over continuous periods of observation for up to 48 hours. This precision and stability will be achieved by using a single, frame-transfer, back-illuminated CCD detector at the focal plane assembly of a 33.5 cm diameter, on-axis Ritchey-Chrétien telescope. The nearly 275 kg spacecraft is nadir-locked, with a pointing accuracy of about 1 arcsec rms, and will allow for at least 1 Gbit/day downlink. The sun-synchronous dusk-dawn orbit at 700 km altitude enables having the Sun permanently on the backside of the spacecraft thus minimising Earth stray light. A mission duration of 3.5 years in orbit is foreseen to enable the execution of the science programme. During this period, 20% of the observing time is available to the wider community through yearly ESA call for proposals, as well as through discretionary time approved by ESA’s Director of Science. At the time of this writing, CHEOPS commissioning has been completed and CHEOPS has been shown to fulfill all its requirements. The mission has now started the execution of its science programme.
Context
. The lack of sub-Jovian planets on orbits of
P
orb
< 3 days is a puzzling aspect of galaxy formation with regard to the distribu-tion of exoplanets whose origins are currently unresolved.
...Aims
. The possible explanations behind the formation of the sub-Jovian or Neptunian desert include several scenarios that can lead to different shapes for the boundary, predicting various dependencies between the position of the boundary and the stellar parameters.
Methods
. We explored the exoplanet distribution in various 2D and 3D projections, revealing the stellar-dependent substructures in the
P
orb
–
M
P
and the
P
orb
–
R
P
parameter plane.
Results
. We demonstrate that the upper boundary includes a range of planets, namely, inflated hot Jupiters and normal hot Jupiters, in the two parameter planes, respectively. We confirm the dependence of the boundary on several stellar parameters and, based on a fuzzy clustering analysis, we provide quantitative formulae for the dependencies in groups of smaller and larger planets. The overall period-radius distribution shows chemical substructures as well, with the boundary being dependent on volatiles and alpha-elements, alongside marginal (to none) dependence found for refractory elements.
Conclusions
. These findings confirm multiple plausible causes for the formation of the desert, particularly preferring those scenarios related to the irradiation-driven loss of the atmospheres of moderately massive planets as the predominant process in shaping planetary distributions.
Context.
The detection of a super-Earth and three mini-Neptunes transiting the bright (
V
= 9.2 mag) star HD 108236 (also known as TOI-1233) was recently reported on the basis of TESS and ...ground-based light curves.
Aims.
We perform a first characterisation of the HD 108236 planetary system through high-precision CHEOPS photometry and improve the transit ephemerides and system parameters.
Methods.
We characterise the host star through spectroscopic analysis and derive the radius with the infrared flux method. We constrain the stellar mass and age by combining the results obtained from two sets of stellar evolutionary tracks. We analyse the available TESS light curves and one CHEOPS transit light curve for each known planet in the system.
Results.
We find that HD 108236 is a Sun-like star with

Gyr. We report the serendipitous detection of an additional planet, HD 108236 f, in one of the CHEOPS light curves. for planets HD 108236 b to HD 108236 f, respectively. These values are in agreement with previous TESS-based estimates, but with an improved precision of about a factor of two. We perform a stability analysis of the system, concluding that the planetary orbits most likely have eccentricities smaller than 0.1. We also employ a planetary atmospheric evolution framework to constrain the masses of the five planets, concluding that HD 108236 b and HD 108236 c should have an Earth-like density, while the outer planets should host a low mean molecular weight envelope.
Conclusions.
The detection of the fifth planet makes HD 108236 the third system brighter than
V
= 10 mag to host more than four transiting planets. The longer time span enables us to significantly improve the orbital ephemerides such that the uncertainty on the transit times will be of the order of minutes for the years to come. A comparison of the results obtained from the TESS and CHEOPS light curves indicates that for a
V
~ 9 mag solar-like star and a transit signal of ~500 ppm, one CHEOPS transit light curve ensures the same level of photometric precision as eight TESS transits combined, although this conclusion depends on the length and position of the gaps in the light curve.
