Abstract Background Recent data about hepatitis A virus (HAV) seroprevalence in industrialized countries and the impact of travels to endemic areas are sparse or absent, particularly for children. ...Objective To determine the impact of travel to endemic areas on HAV seroprevalence and estimate the overall HAV seroprevalence in children in France. To identify risk factors for positive HAV serologic results. Study design This prospective multicentre cross-sectional seroprevalence study took place in eight paediatric emergency units throughout France. Children 1–16 years of age following all inclusion and exclusion criteria were included. Demographic, socioeconomic, and travel data were prospectively collected with a standardized questionnaire before measurement of specific HAV antibodies. HAV seroprevalence was determined and its association with diverse variables assessed by univariate and multivariate analyses. Results 430 children were included, of whom 116 had travelled to endemic areas. The HAV seroprevalence in the overall population was 5% (95%CI, 3–7) and was higher among the travellers (12% 95%CI, 6–18) than among the others (2% 95%CI, 0–3), OR = 7.0 95%CI, 2.6–18.8. Risk factors identified for positive serologic results for HAV were travel to an endemic area >7 days (adjusted OR aOR = 4.3 95%CI, 1.5–12), age of 14–16 years (aOR = 7.7 95%CI, 1.6–38.3) and mother's birth in an endemic area (aOR = 5.2 95%CI, 1.8–14.8). Conclusion Statistical evidence showed that travel to endemic areas and parents’ place of birth both play a role in HAV serologic results in children with a significant difference of HAV seroprevalence between traveller and non-traveller children in France.

This article aims to give an update of catch-up vaccination of internationally adopted or refugee or migrant children in France. The vaccination status of a child who recently arrived in France is complex and has to be adapted to his country of origin. Some of them were never vaccinated whereas the vaccine status of others is uncertain or unknown. Three parameters have to be considered: the age of the child, the country of origin, and sometimes serology in the case of doubts of his vaccine status. Catch-up vaccination of foreign children has to be adapted to French vaccine recommendations, as a reference, and to vaccines already administered to the child.
En France, l’adoption internationale comprend environ 90000 enfants depuis 1980 et celle des enfants immigrés en 2008 représente plus de 300000 personnes. Il s’agit d’une population très hétérogène ...(âge, origine géographique) et non négligeable. Il n’est donc pas toujours simple de faire un bilan vaccinal fiable et complet. La grande variété des situations individuelles rend difficile une décision et des modalités du rattrapage vaccinal homogènes. Cet article propose une mise au point pour la mise à jour des vaccinations de l’enfant arrivant de l’étranger (adopté, réfugié, ou migrant). Le bilan de vaccination d’un enfant nouvellement arrivé dans notre pays est complexe et doit être adapté au pays d’origine. Certains enfants n’ont jamais été vaccinés et d’autres ont un statut vaccinal incertain ou inconnu. L’âge de l’enfant, son pays d’origine et quelques sérologies vaccinales doivent guider la décision vaccinale initiale. La mise à jour vaccinale d’un enfant arrivant de l’étranger doit tenir compte des vaccinations supposées déjà reçues et des recommandations vaccinales françaises, qui vont servir de référence.

Chaque année, la National Foundation for Infectious Diseases (NFID - Fondation américaine pour les maladies infectieuses) rassemble plus de 300 personnes pour faire le point des dernières avancées ...scientifiques dans le domaine des vaccins et identifier les nouvelles voies de recherche les plus prometteuses. Ces conférences figurent parmi les plus importantes réunions scientifiques entièrement dédiées à la recherche sur les vaccins, que ce soit dans le secteur de la santé humaine ou dans le secteur de la santé animale. Lors de la 15e conférence, qui s’est tenue à Baltimore en mai 2012, se sont succédé conférences plénières par des experts invités de renommée internationale, sessions parallèles, présentations de posters et rencontres entre experts et jeunes chercheurs. Différents thèmes ont été abordés, allant des fondements de la vaccination, de l’exploration de la réponse immunitaire et de la conception de nouveaux vaccins et adjuvants jusqu’à l’évaluation de l’impact sur le terrain des vaccins nouvellement introduits (rotavirus, HPV) et de la sécurité des vaccins, en passant par les stratégies de vaccination, notamment dans les pays en développement. Les nouvelles techniques de la vaccinologie systémique permettant l’étude de la réponse immunitaire dans son ensemble ouvrent de nouvelles pistes pour établir des corrélats de protection, et concevoir de nouveaux vaccins, notamment contre les maladies chroniques, comme le sida ou le paludisme, pour lesquelles l’immunité naturelle est imparfaite.

These conferences are among the most important scientific meetings entirely dedicated to vaccine research for both humans and animals, and provide a mix of plenary sessions with invited presentations by acknowledged international experts, parallel sessions, poster sessions, and informal exchanges between experts and young researchers. During the Fifteenth Conference that took place in Baltimore in May 2012, various topics were addressed, including the scientific basis for vaccinology; exploration of the immune response; novel vaccine design; new adjuvants; evaluation of the impact of newly introduced vaccines (such as rotavirus, HPV vaccines); vaccine safety; and immunization strategies. The new techniques of systems biology allow for a more comprehensive approach to the study of immune responses in order to identify correlates of protection and to design novel vaccines against chronic diseases such as AIDS or malaria, against which natural immunity is incomplete.

The annual meeting of the Infectious Disease Society of America (IDSA) ; which brought together nearly 5000 participants from over 80 countries in Vancouver, Canada, October 21 to 24, 2010 ; provided ...a review of the influenza (H1N1) 2009 pandemic, evaluated vaccination programmes and presented new vaccines under development. With 12,500 deaths in the United States in 2009-2010, the influenza (H1N1) 2009 pandemic was actually less deadly than the seasonal flu. But it essentially hit the young, and the toll calculated in years of life lost is high. The monovalent vaccines, whether live attenuated or inactivated with or without adjuvants, were well tolerated in toddlers, children, adults and pregnant women. In order to protect infants against pertussis, family members are urged to get their booster shots. The introduction of the 13-valent Pneumococcal conjugated vaccine in the beginning of 2010 may solve - but for how long ? - the problem of serotype replacement, responsible for the re-increasing incidence of invasive Pneumococcal infections observed in countries that had introduced the 7-valent vaccine. The efficacy of a rotavirus vaccine has been confirmed, with a reduction in hospitalization in the United States and a reduction in gastroenteritis-related deaths in Mexico. In the United States, vaccination of pre-adolescents against human papillomavirus (HPV) has not resulted in any specific undesirable effects. Routine vaccination against chicken pox, recommended since 1995, has not had an impact on the evolution of the incidence of shingles. Vaccination against shingles, recommended in the United States for subjects 60 years and over, shows an effectiveness of 55 %, according to a cohort study (Kaiser Permanente, Southern California). Although some propose the development of personalized vaccines according to individual genetic characteristics, the priority remains with increasing vaccine coverage, not only in infants but also in adults and the elderly. Vaccine calendars that cover a whole lifetime should be promoted, since the vaccination of adults and seniors is a determining factor of good health at all ages.

Évaluer l’impact des voyages en pays d’endémie d’hépatite A (HA) sur la séroprévalence (SP) de l’HA dans la population pédiatrique française, estimer la SP actuelle de l’HA chez l’enfant en France et ...identifier les facteurs associés à une sérologie positive.
Étude de SP dans 8 hôpitaux français. Les IgG anti-HA ont été dosés dans 4 classes d’âge. Des données démographiques, socioéconomiques et les voyages étaient recueillis. La SP de l’HA et les facteurs associés à une sérologie HA positive, dont le rôle d’au moins 1 séjour > 7j était déterminés.
430 patients ont été analysés. Un impact des voyages en pays d’endémie sur la SP de l’HA était observé. Il y avait une différence significative entre les enfants sédentaires (S
P = 2 %; IC95 %=0-3) et les enfants voyageurs (S
P = 12 %; IC95 %=6-18). 20 patients avaient une sérologie HA positive (S
P = 5 %; IC95 %=3-7). Après ajustement, les facteurs liés à une sérologie HA positive étaient l’âge (OR=7,7; IC95 %=1,6-38), l’origine des parents hors de métropole (OR=5,2; IC95 %=1,8-15) et ffl1 séjour en pays d’endémie (OR=4,3; IC95 %=1,5-12).
Les voyages en zone d’endémie d’HA ont un impact sur la SP de l’HA dans une population pédiatrique française. La SP de l’HA en France a diminué à 5 % pour les moins de 17 ans.

La réunion annuelle de la Société américaine d’infectiologie (Infectious Disease Society of America IDSA), qui a réuni près de 5000 congressistes de plus de 80 pays à Vancouver, Canada du 21 au ...24 octobre 2010, a permis de faire le bilan de la pandémie de grippe A(H1N1) 2009 en Amérique du Nord, d’évaluer les programmes de vaccination, et de présenter les nouveaux vaccins en cours de développement. Avec 12 500 décès aux États-Unis en 2009–2010, la grippe pandémique a été moins meurtrière que la grippe saisonnière. Mais elle a frappé essentiellement des jeunes et le bilan calculé en nombre d’années de vie perdues est élevé. Les différents vaccins monovalents (vivants atténués, inactivés avec ou sans adjuvants) ont été bien tolérés, chez les enfants comme chez les adultes et les femmes enceintes. La protection des très jeunes nourrissons contre la coqueluche passe par le renforcement des rappels vaccinaux de son entourage familial. L’introduction du vaccin pneumococcique conjugué 13-valent au début de l’année 2010 pourrait résoudre – mais pour combien de temps ? – le problème de remplacement des sérotypes, responsable d’une recrudescence des infections invasives à pneumocoques observée dans les pays qui avaient introduit le vaccin conjugué 7-valent. L’efficacité du vaccin rotavirus se confirme, avec une diminution des hospitalisations aux États-Unis et une diminution des décès dus aux gastroentérites au Mexique. Aux États-Unis, la vaccination des préadolescentes contre le papillomavirus humain (HPV) n’a pas entraîné d’effet indésirable particulier. La vaccination de routine contre la varicelle, recommandée depuis 1995, n’a eu aucun impact sur l’évolution de l’incidence du zona. Quant au vaccin contre le zona, recommandé pour les sujets de 60
ans et plus aux États-Unis, son efficacité sur le terrain serait de 55 %, d’après une étude de cohorte (Kaiser Permanente de Californie du Sud). Si certains proposent l’élaboration de vaccins à la carte, en fonction des caractéristiques génétiques individuelles, la priorité reste l’augmentation de la couverture vaccinale, non seulement chez les enfants, mais aussi chez les adultes et les personnes âgées. Il convient de proposer un calendrier vaccinal étalé sur toute la vie, la vaccination à l’âge adulte et chez les seniors étant déterminante pour une bonne santé à tous les âges de la vie.
