Pregnancy and postpartum are vital times of greater vulnerability to suffer a decompensation of bipolar disorder (BD). Methods: A systematic literature search was performed on public electronic ...medical databases, following PRISMA guidelines. Studies were included if they reported postpartum relapse in patients diagnosed with BD according to Diagnosis Statistical Manual (DSM) or International Classification Disease (ICD) criteria. Results: Sixteen articles describing 6064 deliveries of 3977 women were included in the quantitative analyses. The overall risk of postpartum relapse was 36.77%. The methodology of the studies, the diagnostic criteria, the discrimination between BD type I and II, and the origin of the sample were very heterogeneous. Conclusions: the rate of postpartum bipolar relapse is very high, as it is considered to be a critical period. It is especially important to detect decompensation in this period and to evaluate mood-stabilizing treatment, given the high risk of relapse concentrated in a short period.

A random sample of 56 individuals injured in the attacks was interviewed in person at one, six, and twelve months after the attacks. Current DSM-IV mental disorders: depressive disorders and anxiety disorders (PTSD, generalised anxiety, agoraphobia, social phobia, and panic disorder) were assessed with the Spanish version of the MINI (Mini International Neuropsychiatric Interview), a structured, lay-administered psychiatric interview. PTSD was the most prevalent psychiatric disorder (35.7% at month 1, 34.1% at month 6, and 28.6% at month 12), followed by major depression (28.6%, 22.7%, and 28.6%, respectively). Others relevant conditions were suicide risk, generalised anxiety disorder (GAD), agoraphobia, and panic disorder. No significant differences in the prevalence of the disorders were found between the different assessment times. Patients with a psychiatric history prior to 11 March had a higher prevalence of PTSD, major depression, GAD, and panic disorder at month 1. Females had higher prevalence of PTSD, agoraphobia, and panic disorder at month 1. The only predictive factor for PTSD at month 12 was PTSD at month 6 (OR = 14.007). The only predictive factor for major depression at month 12 was major depression at month 6 (OR = 15.847). The prevalence of PTSD and major depression was high and remained stable between month 1, month 6, and month 12. The only predictive factor for PTSD at month 12 was PTSD at month 6. To determine the change in prevalence of posttraumatic stress disorder (PTSD) symptoms in victims of the March 11 attacks and their relatives, 1 and 6 months after the attacks.
Evaluation of PTSD ...symptoms using the Davidson Trauma Scale (DTS) and General Health Questionnaire (GHQ) in a sample of 56 patients admitted to an emergency room of a general hospital, and assessment of PTSD symptoms in relatives of the patients.
At Month 1, 41.1% of patients (31.3% of males and 54.2% of females) presented with PTSD. At Month 6, this figure was 40.9% (30.4% of males and 52.4% of females). There was a significant improvement in perception of health among females between Month 1 and Month 6. Relatives presented similar DTS scores at baseline and at 6 months.
We verified that rates of PTSD did not vary substantively between the two evaluations. PTSD symptoms positively correlated with psychological health involvement. This correlation points out that both PTSD symptoms and subjective general health involvement are part of the psychological response to trauma.
The prevalence of PTSD symptoms was high and remained stable between Month 1 and Month 6, while subjective perception of health improved significantly.
Introducción: Las alteraciones del sueño son apreciadas con frecuencia en la mayoría de los pacientes con trastornos mentales y, a su vez, los trastornos psiquiátricos tienden a ser comunes en ...pacientes con alteraciones del sueño. Objetivo: Aportar al médico psiquiatra o de atención primaria una visión amplia sobre la relación entre las alteraciones del sueño y la valoración de los pacientes con trastornos mentales. Método: revisión de la literatura. Desarrollo y resultados: En enfermedades como la depresión, la ansiedad y la esquizofrenia, se pueden apreciar alteraciones características de los patrones del sueño que se correlacionan con las manifestaciones clínicas y que en algunos casos pueden predecir una recaída o la aparición de una nueva crisis, incluso antes de la presencia de la sintomatología propia de la enfermedad. De igual manera, conocer los ciclos normales del sueño y cuál es su infl uencia en las enfermedades mentales puede convertirse en una alternativa al momento de utilizar tratamientos como la privación de sueño o la exposición a la luz. Conclusión: El sueño se ve afectado en los pacientes con trastornos psiquiátricos, e incluso puede en algunos casos predecir una exacerbación de la enfermedad o el inicio de una nueva crisis. Estar al tanto de esto otorga al médico una perspectiva que va más allá de la determinación de los síntomas y signos clásicos de las enfermedades mentales.
