BACKGROUND This study is the first study to evaluate the nonadherence rate and reasons of same patient with inflammatory bowel disease (IBD) in Iran. METHODS During 9 months, 500 patients with IBD ...were enrolled in the study. Patients were interviewed about their nonadherence behaviors. Factor analysis was used to analyze the collected answers. RESULTS The overall rate of nonadherence was 33.3% (27.6% intentional nonadherence and 5.7% unintentional nonadherence). 33.6% of the patients had at least one relapse after discontinuing treatment. The most frequent reason for intentional nonadherence was discontinuing the treatment after recovering from symptoms (42.7%). The most frequent reason for unintentional nonadherence was forgetfulness (5.2%). 19.8% of the patients did not visit their gastroenterologist on time and they purchased drugs from the drugstore. These patients reported that their clinics were too far and difficult to access. There was no significant relationship between nonadherence and demographic variables. CONCLUSION Multiple reasons are suggested as factors of medication nonadherence and they seem to be different among different populations. Determining these possible reasons, could lead to finding suitable strategies to overcome or reduce them.
Organizational theory, as a field of study, offers to those interested some effective ways of thinking. This knowledge helps managers make organizational behavior more efficient through analyzing ...complex situations and developing effective tools to resolve them. In other words, it opens human mind to different aspects of life inside and outside the organization. Therefore, the value of organizational theory is in changing managers' thinking ways, thought patterns and performance. But a fact should also be considered, that organizational theories, affected by the passage of time and existing changes, have transformed and become variant, thus have not been always neatly compatible with one another. To the extent that, some scientists have considered this situation as a shaky foundation for a scientific discipline. In the same vein, the present paper investigates systemic thinking as a new knowledge of organizational theory. The literature indicates that knowing this thinking is essential and it triggers change in people’s thought and attitudes, as such, organizations' management, i.e., systems' management becomes more effective. But through more reflecting, it can be apprehended that this knowledge is accompanied by some contradictions and complexities that harm its coherence and scientific message. This situation requires wisdom, reinvestigation and reform.
Background: Quality of life (QOL) is becoming an important indicator of treatment efficacy in patients with breast cancer. Most previous studies have compared patients' QOL following breast ...conserving surgery (BCS) and mastectomy with or without reconstruction. Our aim was to assess the impact of BCS versus oncoplastic breast surgery (OBS). Methods: Selection of patients for BCS or OBS was performed according to standard criteria e.g the breast and tumor size. The QOL was assessed by employing EORTC QLQ-C3 and QLQ-BR23 questionnaires concurrently and one year after the surgery. The QOL scores one year after the surgery were compared between two groups using analysis of covariance, after adjusting for the baseline values. Results: A total of 120 patients with a mean age of 46.16±1.4 years were enrolled in the study. BCS and OBS were the main surgical treatment techniques in 57(47.5%) and 63(52.5%) patients, respectively. At the time of the last follow-up visit, there were no differences between the two groups regarding functional scales such as physical (P = 0.761), role (P = 0.356), emotional (P = 0.107), cognitive (P = 0.051), and social functioning (P = 0.659). No differences were observed between the two groups regarding nine symptom scales. Based on the results of breast cancer specific module of the questionnaire, no differences were observed in functional scales and symptoms with the exception of arm symptoms which were less common in OBS group (P = 0.023) Conclusions: Based on the results of the current study, it could be suggested that there are no significant differences in the in scores of QOL components between patients who received BCS or OBS.
Driving safety can be achieved by better understanding critical situations which may require the knowledge of interaction between vehicle tyres and the road surfaces. It is thus essential to have a ...good estimation of the tyre/road friction parameters in real-time. The paper deals with the trust-region based method for on-line estimation of tyre/road friction parameters. This method provides an appropriate modeling (of a vehicle and the tyre/road contact) to observe the tyre/road friction coefficients directly using measurable signals in real-time. In this work, we present a new LuGre model-based nonlinear least squares (NLLS) parameter estimation algorithm using vehicle dynamic to obtain the parameters of LuGre model based on recursive nonlinear optimization of the curve fitting errors. The proposed estimation method can also be utilized in large-scale problems with similar conditions. Very promising results have been obtained in real-time simulations for most of the driving and road situations.

This study introduces a new technique for generating synthetic faces using eyes-to-face inpainting methods. The proposed method can synthesize a face image using a combination of the eyes of two ...different individuals and use it as an input for inpainting, demonstrating its vast potential for various applications in biometrics. Despite minor biases in age and gender, our method proved effective in training reliable age- and gender-detection models using the generated datasets. We also addressed the challenge of training face recognition models using synthetic datasets, and the results demonstrated satisfactory accuracy across four benchmark face recognition datasets. This method could be particularly beneficial for underrepresented groups, for whom there is a scarcity of face samples in biometric datasets.
Purpose - This study seeks to build a conceptual model of agricultural extension intrapreneurship that discusses the concept and phenomenon of intrapreneurship as well as its prerequisites and ...outcomes. The proposed model is intended to depict the main factors that affect the phenomena of intrapreneurship within the agricultural extension organizations and the impact of intrapreneurship on agricultural extension organizational outcomes, as well as factors influencing its continuous outcomes.Design methodology approach - The paper identifies and describes the existence and past literature on prerequisites, phenomena and outcomes of intrapreneurship in organizations as a basis for analysis and developing an appropriate model of agricultural extension intrapreneurship. This is a conceptual paper selecting targeted scholarly works that provide support for the proposed model.Findings - The paper presents a model of Iranian agricultural extension in which the organizational, behavioral and environmental factors influence intrapreneurship and the construct of intrapreneurship influences organizational outcomes. The paper provides a comprehensive analysis that develops an existing model for a systematic approach to the agricultural extension intrapreneurial process. The proposed model of agricultural extension intrapreneurship joins a growing number of works that explore how intrapreneurship contributes to organizational outcomes in an Iranian agricultural extension organization. It is suggested that the framework may help scholars identify potential strategies of intrapreneurial activity that could help extension organization position intrapreneurship as a vehicle for improving organizational outcomes.Originality value - Based on this exploration, new insights about agricultural extension intrapreneurship are developed, practical implications for agricultural extension intrapreneurs on how to approach agricultural extension entrepreneurship more systematically and effectively are presented and opportunities for further research are identified.
In the current competitive world, the agency has a chance to sustain which could be adapted itself to the environmental changes continually. Therefore, one of the ways for acquiring sustainable competitive advantage, is focusing on persistent learning of employees for approach to the organizational targets with highest influence. The aim of the present research is the review the organizational learning level by using Marquardt Model in the Noor municipality. In order to assay hypothesizes, the single - sampled has been used following assignment of their normality through Kolmogorov-Smirnov test. Finally, the component of learning organization has been categorized by using of bar bias diagram. The findings show that, the learning dynamic component, organizational evolution, enrichment of individuals and knowledge management are include in municipality of Noor city but component of technology implementation has been realized. With regard to these results, could be said that, the municipality of Noor city was a Learning organization and, indeed must work in the field of development and implementation of technology.
Cholestasis is associated with an accumulation of endogenous opioids in plasma which are known to reduce erythrocyte deformability in‐vitro. We compared chlorpromazine‐induced erythrocyte shape ...change in bile duct ligated (cholestatic), sham‐operated and control rats to evaluate the effect of cholestasis on erythrocyte deformability.
Cell shape change was measured using a polarized light scattering method. Cholestasis reduced erythrocyte shape change induced by chlorpromazine and the decrease in erythrocyte deformability correlated with the duration of cholestasis. Doses of chlorpromazine which produced 50% effect on erythrocyte shape change in cholestatic groups were significantly (P < 0.001) higher than controls. Erythrocyte osmotic fragility was also measured. Cholestasis produced increased resistance to osmotic shock in erythrocytes.
Erythrocyte shape change is reduced in cholestasis probably due to the accumulation of endogenous opioids in plasma.
