V članku je opisana zgodovina klasifikacije vmesne skupine ščitničnih rakov, ki so uvrščeni med zrele oblike ščitničnega raka (WDTC) in nezrele oblike (ATC). Ta vmesna skupina se imenuje slabo ...diferencirani rak ščitnice (PDTC). Tumorji iz te skupine povzročajo slab potek bolezni z lokalno ponovitvijo bolezni ali oddaljenimi zasevki ali z obojim, medtem ko je pri največji skupini dobro diferenciranih rakov ščitnice, ki predstavljajo veliko večino rakov ščitnice, napoved poteka bolezni večinoma veliko ugodnejša. Po poteku so slabo diferencirani raki ščitnice podobni nezreli obliki ščitničnega raka, vendar pa je ob izbranem zdravljenju pričakovati znatno boljše preživetje kot pri anaplastičnem raku. Opisane so nekatere patološke značilnosti slabo diferenciranih rakov in osnovna klinična slika. Prikazani so tudi 3 primeri bolnikov, od tega 2 bolnic z izjemnim potekom in 1 bolnika s pričakovanim potekom bolezni. Nakazane so tudi nove možnosti sistemskega zdravljenja raka ščitnice.

of a 34-year-old male patient, who presented with the rapidly growing mass in ...the neck, extending to the anterior and middle mediastinum, tightly closing the upper thoracic apperture and causing tracheal and oesophageal deviation, with minimal compression and stenosis. A diagnosis of columnar cell carcinoma, arising from the ectopic thyroid tissue just adjacent to the left thyroid lobe was based on histological and intraoperative findings. Near total thyroidectomy and lymph-node dissection were followed by external beam radiotherapy of the neck and mediastinum, chemotherapy and radioiodine ablation of the remaining functional thyroid tissue. Pre-and post-operative radionuclide imaging (99mTc(V)-DMSA, 99mTc-MIBI, 123-I-mIBG and Octreoscan findings are discussed, with a special emphasis given to the dilemmas in histological characterisation of the tumor, the problems in therapeutic approach and the dilemmas and pitfalls in the interpretation of radionuclide findings in this patient, especially the ones performed post radiotherapy of the neck and mediastinum.
La maladie de Pompe est une glycogénose musculaire rare de transmission autosomique récessive due au déficit en alpha-glucosidase acide. Seule une dizaine de cas adultes rapportés s’associent à une ...macroglossie.
Nous présentons 5 cas issus du Registre français des patients atteints de la maladie de Pompe, 3 femmes et 2 hommes, actuellement âgés de 71 à 88 ans. Ils sont tous atteints d’une forme tardive de maladie de Pompe associée à une macroglossie.
Trois patients marchent avec canne ou déambulateur, deux sont en fauteuil roulant. Tous ont une atteinte diaphragmatique et un syndrome d’apnée du sommeil (SAOS) appareillé ; un patient présente une persistance des obstructions malgré la ventilation nocturne. Un seul patient a présenté la macroglossie avant le diagnostic, les 4 autres l’ont développé pendant le suivi. Deux patients ont une dysphagie sévère entraînant un amaigrissement, l’un d’entre eux a été gastrostomisé pour bénéficier d’un support nutritionnel entéral. Tous ont une dysarthrie, deux ont un suivi orthophonique régulier. Tous ont des difficultés de protraction de la langue. Un patient a présenté une hypertrophie localisée de la langue importante ayant motivé une glossectomie 10 ans avant le diagnostic. Pour les 3 patients pour lesquels nous disposons de l’IRM musculaire de la face, celle-ci montre une importante infiltration graisseuse de la langue.
Liée à l’involution graisseuse de la langue, la macroglossie peut orienter le diagnostic de maladie de Pompe chez un adulte atteint de myopathie. L’hypoventilation nocturne liée à l’atteinte respiratoire diaphragmatique peut être aggravée par l’obstruction liée à la macroglossie. Lorsqu’elle entraîne une dysphagie ou dysarthrie, une prise en charge nutritionnelle et rééducative se révèle indispensable.
La macroglossie est un signe clinique important à rechercher, elle peut orienter le diagnostic devant une faiblesse musculaire chez l’adulte, et entraîner des complications nécessitant des prises en charge spécifiques.
La maladie de Pompe est une glycogénose musculaire rare, de transmission autosomique récessive, liée à un déficit de l’activité lysosomale de l’alpha-glucosidase.
L’objectif de cette présentation est ...de présenter le Registre français des maladies de Pompe.
Avec les filières neuromusculaire (FILNEMUS) et métabolique (G2M), il a été colligé depuis 2004 les données fonctionnelles, cliniques et biologiques de tous les patients atteints de maladie de Pompe suivis dans les centres de référence français.
Parmi les 210 patients, 186 sont traités. Les premiers symptômes musculaires sont une faiblesse proximale des membres inférieurs à 38 ans en moyenne, associée à des symptômes respiratoires (17 %). À l’inclusion, 109 patients marchent sans aide technique et 29 ont recours au fauteuil roulant. Une assistance respiratoire est nécessaire pour 77 patients. Le dosage de l’activité enzymatique est réalisé chez 151 patients et la mutation « commune »c.-32-13T>G est identifiée pour 87 % des patients.
Cette large cohorte reflète l’état de patients atteints de maladie de Pompe. Ce registre a déjà permis de préciser les caractéristiques moléculo-épidémiologique de cette maladie, et évaluer le bénéfice de l’enzymothérapie substitutive par Myozyme®. L’arrivée prochaine de nouvelles thérapies (enzymothérapie, thérapie génique) renforce l’importance de poursuivre la collecte de données afin d’évaluer sur des bases académiques les effets des traitements.
Ce registre permet de suivre l’état de la population de patients français atteints de la maladie de Pompe et de les accompagner à l’ère de l’extension des options thérapeutiques.
Abstract Aim The aim of this study is to evaluate the effectiveness and safety of continuous subcutaneous insulin infusion (CSII) under real-life conditions among all patients treated with CSII in ...the south of Paris. Methods The 42 diabetologists practising in the region enrolled all patients treated with CSII or admitted for CSII initiation. During the study visit, the data for pump use and clinical results were recorded. Results Data were obtained for 424 patients, mean age 44.2 ± 15.6 years, disease duration 18.7 ± 10.6 years, including 339 treated with CSII for longer than three months (mean duration: 3.5 ± 3.5 years; range: 3–258 months). Most of the patients ( N = 285, 84.8%) had type 1 diabetes; 44 (13.1%) had type 2 diabetes. In patients treated for more than three months, HbA1c decreased significantly between CSII initiation (9.1 ± 1.9%) and the study visit (7.8 ± 1.4%; P < 0.0001). Patients with HbA1c > 9%, using the pump, experienced a significant 0.9% improvement in their HbA1c levels with CSII versus multiple daily injections ( P = 0.001). The number of episodes of moderate hypoglycaemia was 2.7 ± 2.5 per patient per week; of severe hypoglycaemia, 0.34 per patient per year and of ketoacidosis, 0.11 per patient per year. Factors significantly associated with HbA1c levels included amount of physical activity, pregnancy, HbA1c at CSII initiation and number of glucose self-determinations. Those associated with the number of moderate hypoglycaemia episodes were basal rate number, female gender and HbA1c level. HbA1c was negatively correlated with moderate hypoglycaemia ( P < 0.001), but not with severe hypoglycaemia. Conclusion This ‘pump’ registry establishes the effectiveness of CSII in everyday practice, yet underscores the risks of severe hypoglycaemia and ketosis episodes. It could help diabetologists to improve patient training programmes and follow-up.
Abstract Background and aims As concerns over interference with sexual activity may be an obstacle to initiating pump therapy in diabetic patients, the aim of the study was to assess the impact of ...continuous subcutaneous insulin infusion (CSII) therapy on sexual activity. Patients and methods Patients filled out a questionnaire on their demographic data, diabetes history, pump-treatment history, metabolic control, inconvenience/convenience of the pump and catheter, and information on sexual activity. Results A total of 271 diabetic patients (aged 44 ± 17 years, 51% women, 22% single), treated with CSII for 4.2 ± 5.9 years and with a diabetes duration of 19 ± 11 years, filled out the questionnaire. Their HbA1c was 7.7 ± 1.1%, with 2.4 ± 2.1 mild hypoglycaemic episodes over the past week, and their frequency of sexual activity was: never 29.9%; < 1/month 12.3%; > 1/month and < 1/week 18.2%; and > 1/week 39.6%. Age and cohabitation were independently correlated with frequency of sexual activity ( P < 0.0001 and P < 0.0003, respectively), but not diabetes duration or complications. To the question “Does the pump have an influence on your sexual activity?”, The answer was “no” in 90% and “yes” in 10%. However, intercourse frequency was significantly decreased in the latter ( P = 0.04). On multivariate analyses, this negative influence of CSII was correlated with HbA1c ( P < 0.05), discomfort with the pump ( P < 0.05) and the number of mild hypoglycaemic episodes ( P < 0.01). Conclusion Frequency of sexual activity appears to be unaffected by pump therapy or diabetes, but is decreased by the expected characteristics–namely, age and being single. Also, only 10% of patients believe that CSII is an obstacle during sexual activity and, in particular, because of the catheter.
