The purpose of this study was to analyze the feasibility and utility of 3D imaging to help lead positioning during a deep brain stimulation (DBS) procedure. A bilateral subthalamic DBS procedure was ...conducted in 2 patients for idiopathic Parkinson's disease. Subthalamic nucleus targeting was based on preoperative stereotactic MRI. We used the Medtronic O-arm to perform 2D-imaging control (frontal and lateral) as well as quick (<30 s) 3D acquisition. This allowed us to check the positioning of micro-macro electrodes and definite electrodes. 3D images were fused with postoperative CT to assess their accuracy, and with preoperative MRI to visualize the anatomical location of the electrodes. 3D imaging is a quick and safe method to ensure perioperative control of lead placement during DBS procedures.
L’embolisation pré-opératoire des méningiomes intra-crâniens permet de réduire les pertes de sang et d’améliorer la résécabilité de la tumeur lors de la chirurgie. Peu d’études ont évalué les risques ...et les bénéfices liés à l’utilisation de l’Onyx dans cette indication. Le but de cette étude était d’étudier la sûreté et l’efficacité de l’embolisation à l’Onyx dans une série de volumineux méningiomes intra-crâniens en comparaison d’une série appariée n’ayant pas bénéficié de l’embolisation.
Nous avons procédé à une étude rétrospective intéressant des patients consécutifs porteurs de méningiomes prouvés histologiquement et embolisés à l’Onyx entre 2010 et 2018. Les complications liées à l’embolisation ont été rapportées. Deux échantillons appariés composés de méningiomes embolisés à l’Onyx et de méningiomes non embolisés ont été constitués afin de déterminer les bénéfices de la procédure.
Résultats 44 méningiomes embolisés à l’Onyx chez 44 patients ont été inclus dans cette étude. 44 méningiomes non embolisés leur ont été comparés. Les deux groupes étaient semblables en termes d’âge, de sexe, de taille tumorale, d’importance de l’œdème péri-lésionnel et de grade OMS. Les patients du groupe embolisation ont eu un meilleur devenir avec un taux moindre de transfert en soins de suite (50,0 % vs 79,5 % ; p<0,01), de récidive (15,4 % vs 35,9 % ; p<0,04), de résection incomplète et de complications chirurgicales. Seulement trois complications majeures ont été rapportées consistant en des pertes de fonction visuelle.
Cette étude est la première étude comparative à montrer les bénéfices de l’embolisation pré-chirurgicale à l’Onyx dans le cas de volumineux méningiomes intra-crâniens. Les patients embolisés présentaient notamment des taux moindres de complications chirurgicales, de résection incomplète ainsi que de récidive, avec un faible risque procédural.

Objective The aim of this study was to evaluate the prevalence of venous thromboembolism (VTE) in stage IV melanoma and determine risk factors, outcomes associated with the development of VTE and the number of haemorrhagic complications in patients under anti‐coagulant treatment.
Patients and Methods In this retrospective study, we included all consecutive patients with stage IV melanoma among 290 patients followed‐up in the department of Dermatology each year between January 2005 and 31 December 2007. The diagnosis of VTE was confirmed by venous ultrasound, pulmonary perfusion‐ventilation technetium scan and angiography. The primary outcome was to evaluate the number of TE diagnosed in stage IV melanoma patients. The secondary outcomes were to study the influence of TE on survival, its prevalence according to metastatic sites and to evaluate the number of haemorrhagic complications.
Results Twenty‐four VTE events were found 25.2% (CI: 16.5–34). Eighteen VTE were deep venous thrombosis in lower limbs associated with pulmonary embolism (PE) in 50% of cases. Twenty‐five percent were asymptomatic and were revealed in the pulmonary scan performed for follow‐up. Eight percent of VTE events revealed stage IV melanoma. Seventeen patients developed thrombosis at home after stopping heparin prophylaxis. Seven thrombotic events occurred during oral anti‐coagulant therapy.
Conclusion We found as high a prevalence of VTE in stage IV melanoma as in lung and gastrointestinal cancers. All patients suffered thrombotic events when they were treated with chemotherapy and at home when they stopped heparin prophylaxis. Therefore, heparin prophylaxis should be maintained at home.
Antacid (AA) in a very low dose (88 mmol/day) was compared to the standard 800-mg dose of cimetidine in healing duodenal ulcers. The influence of sex, age, symptom duration at entry, night pain, ...smoking, coffee consumption, and alcohol on ulcer healing was studied. The antacid was given in two different schedules: group I--20 ml 1 hr after breakfast and at bedtime; group II--10 ml 1 hr after breakfast and lunch and 20 ml at bedtime. Cimetidine (group III) was given in two divided doses: 400 mg 1 hr after breakfast and 400 mg at bedtime. Endoscopic control was performed after four weeks and, if necessary, after eight weeks of treatment. The healing rate after four weeks of treatment was, respectively, for groups I, II, and III, 45.5%, 55.8%, and 69.4% (group I = group II, and group III different from groups I and II). After eight weeks of treatment the healing rate was 61.5%, 80.8%, and 88.0% for groups I, II, and III, respectively (group II = group III, and group I different from groups II and III). Except for group I, smoking did not influence healing rate. Age, sex, symptoms at entry, night pain, and coffee consumption did not influence the treatment results. The authors concluded that the very low dose of magaldrate (88 mmol/day), when administered in three divided doses (10 ml after breakfast and lunch and 20 ml at bedtime) for eight weeks was as effective as 800 mg of cimetidine (400 mg twice a day) in healing duodenal ulcer.
Invasion of intestinal epithelial cells by Salmonella enterica is decreased after exposure to butyric acid. To understand the molecular mechanisms of this phenomenon, a comparative transcriptomic ...analysis of Salmonella enterica serovar Enteritidis and Salmonella enterica serovar Typhimurium grown in medium supplemented with butyrate was performed. We found that butyrate down-regulated the expression of 19 genes common to both serovars by a factor of twofold or more, and 17 of these genes localized to the Salmonella pathogenicity island 1 (SPI1). These included the SPI1 regulatory genes hilD and invF. Of the remaining two genes, ampH has 91% homology to an Escherichia coli penicillin-binding protein and sopE2 encodes a type III-secreted effector protein associated with invasion but located at a separate site on the chromosome from SPI1.
We studied the effects of repeated inducements of mental fatigue (MF) from using social media on smartphones immediately before training sessions by young male volleyball athletes, focusing ...specifically on their decision-making, endurance, and countermovement jump performance (CMJ). We pair-matched 24 participants according to their decision-making abilities and then, randomly assigned them to one of two 4-week block training groups: control (CON) and smartphone use (SMA). For a 30-minute period before each training session, the CON group watched TV and the SMA group used social media apps on smartphones. We found a significant group x time interaction effect for decision making (attack, p = 0.03; passing, p = 0.02) during training blocks. More specifically, only the CON group improved their decision making (attack, p = 0.03; passing, p = 0.02). Both groups significantly improved their CMJ performance (p = .01), with no significant group x time interaction effect for CMJ (p = 0.91). Neither group significantly improved their endurance (p = 0.56). We concluded that 30-minutes of repetitive social media app use on a smartphone immediately before 4-weeks of volleyball training sessions negatively affected decision-making in young male volleyball athletes.
