Hand, foot and mouth disease (HFMD) and herpangina (HA) are frequently caused by several distinct serotypes belonging to the human enterovirus A species (HEVA). Enterovirus 71 is considered as a ...significant public health threat because of rare but fatal neurological complications. A sentinel surveillance system involving paediatricians from Clermont-Ferrand (France) was set up to determine the clinical and epidemiological characteristics of HFMD/HA associated with enterovirus infections. A standardized report form was used to collect demographic and clinical data. Throat or buccal specimens were obtained prospectively and tested for the presence of enteroviruses. The frequency of HEVA serotypes was determined by genotyping. Phylogenetic relationships were analysed to identify potential new virus variants. From 1 April to 31 December 2010, a total of 222 children were enrolled. The predominant clinical presentation was HA (63.8%) and this was frequently associated with clinical signs of HFMD (48%). An enterovirus infection was diagnosed in 143 (64.4%) patients and serotype identification was achieved in 141/143 (98.6%). The predominant serotypes were coxsackievirus A10 (39.9%) and A6 (28%), followed by coxsackievirus A16 (17.5%) and enterovirus 71 (6.3%). Fever was observed in 115 (80.4%) children. No patient had neurological complications. Coxsackievirus A10 and A6 strains involved in the outbreak were consistently genetically related with those detected earlier in Finland and constituted distinct European lineages. Although several enterovirus serotypes have been involved in HFMD/HA cases, the outbreak described in this population survey was caused by coxsackievirus A6 and coxsackievirus A10, the third dual outbreak in Europe in the last 3 years.

Qu’elles soient symptomatiques ou non, les infections néonatales à entérovirus (EV) peuvent être à l’origine d’infections nosocomiales dans les services de néonatologie et de réanimation pédiatrique. ...Nous décrivons 2 observations cliniques d’infections néonatales neuro-méningées à echovirus 11 et apportons la preuve moléculaire de la transmission nosocomiale de la souche dans un service de néonatalogie, grâce au génotypage et à l’analyse phylogénétique. Ces observations illustrent l’importance d’inclure les recherches moléculaires prospectives de ces infections dans tout bilan infectieux pour ne pas les sous-estimer chez les nouveau-nés, l’intérêt d’identifier les EV au-delà du genre et l’aide à la preuve de la transmission nosocomiale par l’analyse phylogénétique.
Although enteroviruses generally cause asymptomatic or mild disease, neonates are at higher risk for severe illnesses, among which systemic disease characterized by multiorgan involvement is a potentially fatal condition. Enterovirus neonatal infections may be the source of nosocomial infections in neonatology or in pediatric intensive care units. We report central nervous system infections due to Echovirus 11 in two neonates and the molecular evidence of nosocomial transmission of this strain in a neonatal unit by enterovirus genotyping and phylogenetic analysis. This report illustrates the importance of including enterovirus genome detection in the sepsis screening concomitantly with bacteriological investigations performed at admission of a neonate. Rapid diagnosis and subsequent genotyping could have a beneficial impact on clinical practices at the individual level (reducing the length of antibiotic therapy) and public health policy at the collective level by reinforcing hygiene measures to prevent nosocomial infections, with nurseries and neonatal units being at greater risks.
We report 59 cases of severe paediatric conditions linked with enterovirus (EV)-A71 and EV-D68 in France between May and October 2016. Fifty-two children had severe neurological symptoms. EV ...sequence-based typing for 42 cases revealed EV-A71 in 21 (18 subgenotype C1, detected for the first time in France) and EV-D68 in eight. Clinicians should be encouraged to obtain stool and respiratory specimens from patients presenting with severe neurological disorders for EV detection and characterisation.
L’infection à parvovirus B19 chez l’adulte est associée à l’érythroblastopénie en cas d’immunodépression ou d’hémolyse chronique 1. Des manifestations variées ont également été rapportées comme liées ...au parvovirus B19 (polyarthrites, vascularites, myocardite, néphrite, purpura thrombopénique immunologique) mais leur association est plus incertaine 2. Le diagnostic d’infection à parvovirus B19 repose sur l’association de manifestations évocatrices et d’une séroconversion ou d’une réplication virale. La prescription de PCR parvovirus B19 médullaire est fréquente en cas de cytopénies inexpliquées. Pourtant, l’ADN viral peut persister plusieurs mois après l’infection, en particulier au niveau médullaire. Nous avons étudié la pertinence clinique de la prescription de PCR parvovirus B19 médullaire en analysant rétrospectivement le contexte clinicobiologique ayant mené à cet examen dans les services les plus souvent prescripteurs.
Nous avons inclus toutes les demandes de PCR parvovirus B19 médullaire des 5 dernières années pour des patients adultes pris en charge en médecine interne et hématologie.
Les données étaient collectées rétrospectivement à partir du dossier médical informatisé:
– données démographiques: âge, sexe;
– comorbidités: immunodépression innées/acquise, hémolyse chronique, hémopathie en cours de traitement, utilisation d’immunosuppresseurs;
– tableau clinique ayant mené à la prescription: fièvre, arthralgies, rash, vascularite cutanée, myopéricardite, hépatite;
– données biologiques: hémogramme, stigmates d’hémolyse, PCR sanguine et sérologie parvovirus B19, analyse du myélogramme;
– le traitement éventuellement entrepris.
Le critère de jugement principal était la corrélation entre la positivité de la PCR médullaire parvovirus B19 et le diagnostic finalement retenu en fonction du contexte de prescription. L’extraction des données était réalisée par deux investigateurs indépendants.
Deux cent trois PCR parvovirus B19 médullaire ont été inclues, soit 164 patients (69 femmes, 95 hommes, âge médian 66 ans 54–71). Cent cinq patients étaient pris en charge en hématologie et 59 en médecine interne.
Antécédents: aucun patient n’était porteur d’une pathologie hémolytique héréditaire; 6 patients étaient transplantés d’organe solide. Les hémopathies les plus fréquentes étaient: leucémies aiguës myéloïdes n=38, myélodysplasies n=19, LLC n=14. Cinquante-six patients avaient eu une allogreffe et 12 une autogreffe. Trente-huit patients étaient porteurs de maladies auto-immunes: PTI (n=7), anémie hémolytique auto-immunes (n=6), sarcoïdose (n=3), lupus (n=2).
PCR médullaire: 31 PCR (24 patients) étaient fortement positives (>500 copies/mL); un seul de ces patients avait une réplication virale sanguine. Vingt-deux PCR (17 patients) étaient faiblement positives; aucun de ces patients n’avait de réplication sanguine.
Le diagnostic d’infection à parvovirus B19 était retenu pour 8/41 patients (6 hommes, 2 femmes), dont 6 avaient une PCR>500 copies/mL. L’infection à parvovirus B19 se manifestait par une érythroblastopénie (n=2), une pancytopénie avec aspect d’aplasie médullaire idiopathique (n=1) ou une agranulocytose (n=4), une thrombopénie avec polyadénopathies (n=1).
Un diagnostic alternatif était retenu pour les autres patients avec PCR positive, les plus fréquents étant: toxicité post-allogreffe (n=7), agranulocytose médicamenteuse (n=5), syndrome myélodyplasique (n=5), sepsis sévère (n=4), hépatite E aiguë (n=2).
La charge virale médullaire était significativement plus élevée dans le groupe où le diagnostic d’infection parvovirus B19 était retenue (2022 680; 9485 vs 315 142; 977, p=0,04). Il n’y avait pas de différence statistiquement significative quant au taux d’hémoglobine ou de réticulocytes.
La PCR parvovirus B19 médullaire est fréquemment positive chez les patients fortement immunodéprimés. La charge virale médullaire élevée pourrait indiquer une infection active à parvovirus B19 mais l’analyse du contexte clinicobiologique est indispensable pour retenir le diagnostic et ne pas retenir à tort une indication de traitement par immunoglobulines 3.

Major 5′terminally deleted (5′TD) group-B enterovirus (EV-B) populations were identified in heart biopsies of patients with fulminant myocarditis or dilated cardiomyopathy suggesting that ...these 5′TD forms are key drivers of host-cell interaction in EV cardiac infections. To date, early emergence of EV-B 5′TD forms and its impact on type 1 IFN response during acute myocarditis remains unknown. Using quantitative RACE-PCR assay, we identified major EV-B 5′TD RNA populations in plasma or heart samples of acute myocarditis cases. Deletions identified within the 5′ non-coding region of EV-B populations only affected secondary-structural elements of genomic RNA domain I and were distinguished in two major groups based on the extent of RNA structural deletions. Proportions of these two respective EV-B 5′TD population groups were positively or negatively correlated with IFN-β levels in plasma samples of myocarditis patients. Transfection of synthetic CVB3/28 RNAs harboring various 5′terminal full-length or deleted sequences into human cultured cardiomyocytes demonstrated that viral genomic RNA domain I possessed essential immunomodulatory secondary-structural elements responsible for IFN-β pathway induction. Overall, our results highlight the early emergence of major EVB-TD populations which deletions affecting secondary–structures of RNA domain I can modulate innate immune sensing mechanisms in cardiomyocytes of patients with acute myocarditis.
