(I) We studied the status of chronological blood glucose control among 118 diabetes patients who had attended our hospital for 20 years. We studied cognitive disorders using the MMSE or Mini-Cog as ...conventional methods and the DASC-8 as a new method. The analysis of the relationship between cognitive disorders and blood glucose control revealed that the cognitive function tended to decrease in the poorly controlled group. (II) The analysis of the association between the presence of retinopathy and cognitive function among 206 diabetic outpatients with 20-year history of diabetes who had attended our hospital for more than 10 years revealed that the cognitive function in patients with retinopathy tended to decrease in comparison to patients without retinopathy. (III) The analysis of the entire medication history of all 206 patients revealed that 43 patients with a low cognitive function (according to DASC-8) used medications suspected of causing hypoglycemia. The DASC-8 can be conducted over a shorter period of time in comparison to the MMSE, which makes it effective for classifying the categories of glycemic control targets among elderly patients with diabetes.
A 71-year-old woman was admitted for the treatment of diffuse large B-cell lymphoma of the ileum. She had been taking lansoprazole but was switched to vonoprazan due to epigastric discomfort. Three ...weeks after starting vonoprazan intake, she had a convulsive seizure, and a blood test showed hypomagnesiemia. The cause of hypomagnesemia was considered to be malabsorption of magnesium from the intestinal tract associated with vonoprazan. After discontinuation of vonoprazan, the magnesium level quickly recovered, and the seizures did not relapse. It is important to consider the risk of hypomagnesemia in patients taking vonoprazan, even for a short period of time.
The hepatitis C virus (HCV) is potentially associated with liver cancer, and advances in various drugs have led to progress in the treatment of hepatitis C and attempts to prevent its transition to ...liver cancer. Furthermore, reactivation of HCV has been observed in the treatment of lymphoma, during which the immortalization and proliferation of lymphocytes occur, which leads to the possibility of further stimulating cytokines and the like and possibly to the development of lymphoid malignancy. There are also cases in which the disappearance of lymphoid malignancy has been observed by treating HCV and suppressing HCV-Ribonucleic acid (RNA), as well as cases of recurrence with an increase in HCV-RNA. While HCV-associated lymphoma has a poor prognosis, improving the prognosis with Direct Acting Antivirals (DAA) has recently been reported. The reduction and eradication of HCV-RNA by means of DAA is thus important for the treatment of lymphoid malignancy associated with HCV infection, and HCV-RNA can presumably play a role as a biomarker. This review provides an overview of what is currently known about HCV-associated lymphoma, its epidemiology, the mechanisms underlying the progression to lymphoma, its treatment, the potential and limits of HCV-RNA as a therapeutic biomarker, and biomarkers that are expected now that DAA therapy has been developed.
Electrostatic atomized water particles (EAWPs) treatment was applied to investigate its effect on chlorophyll (Chl) degradation and ripening delay in ‘Namwa’ bananas. Banana fruits were pretreated ...with EAWPs generated from a device (Panasonic F-GMK01) for 0 (control), 0.5, 1.0, 1.5, and 3.0 h in a closed 50 L container, and then kept in perforated polypropylene plastic bags and stored at ambient temperature (25 ± 2°C) under dark conditions. The results showed that 1.0 h-EAWPs treatment best retained peel greenness with a significantly higher hue angle and lower L* value than other treatments on day 6. Also, the 1.0 h-EAWPs treatment maintained the total Chl content, firmness, total soluble solids (TSS), and delayed the ripening index (RI) of fruit accompanied by a delayed climacteric rise in ethylene and respiration rate compared to the control. It was found that the 1.0 h-EAWP treatment induced accumulations of nitric oxide (NO) in peel tissues and suppressed the activities of Chl-degrading enzymes (chlorophyllase, Mg-dechelatase, Chl-degrading peroxidase, and pheophytinase) in the peel. Furthermore, Chl derivatives levels (chlorophyllide a, pheophobide a, 132-hydroxychlorophyll a, and pheophytin a) were higher in fruits treated with EAWPs than the control fruits. The results suggest that EAWPs technology could be an alternative approach to delay Chl degradation and ripening in ‘Namwa’ bananas.
Mast cells (MCs) are key effector cells in allergic reactions. However, the inhibitory mechanism that prevents excessive activation of MCs remains elusive. Here we show that leukocyte ...mono-immunoglobulin-like receptor 3 (LMIR3; also called CD300f) is a negative regulator of MC activation in vivo. LMIR3 deficiency exacerbated MC-dependent allergic responses in mice, including anaphylaxis, airway inflammation, and dermatitis. Both physical binding and functional reporter assays via an extracellular domain of LMIR3 showed that several extracellular lipids (including ceramide) and lipoproteins were possible ligands for LMIR3. Importantly, MCs were frequently surrounded by extracellular ceramide in vivo. Upon engagement of high-affinity immunoglobulin E receptor, extracellular ceramide-LMIR3 binding inhibited MC activation via immunoreceptor tyrosine-based inhibitory and switch motifs of LMIR3. Moreover, pretreatment with LMIR3-Fc fusion protein or antibody against either ceramide or LMIR3 interfered with this binding in vivo, thereby exacerbating passive cutaneous anaphylaxis. Thus, the interaction between extracellular ceramide and LMIR3 suppressed MC-dependent allergic responses.
► LMIR3 deficiency exacerbates mast cell-dependent allergic responses in mice ► Ceramide is identified as a possible ligand for LMIR3 by binding and reporter assays ► Ceramide-LMIR3 binding inhibits FceRI-mediated activation of BMMCs ► The disruption of ceramide-LMIR3 binding in vivo exacerbates PCA reactions in mice
Rituximab is currently used not only in the treatment of B-cell lymphoma but also for various other diseases, including autoimmune diseases, post-transplant graft vs host disease, and rejection ...following kidney transplants. Due to rituximab’s widespread use, great progress has been made regarding research into complications that arise from its use, one of the most serious being the reactivation of hepatitis B virus(HBV), and efforts continue to establish guidelines for preventive treatment against this occurrence. This report discusses preventive measures against rituximab-induced HBV reactivation and future objectives.
This study investigated a new dismantling system for concrete structures using a steam pressure cracking agent. We improved the mechanical systems such that it can drill through reinforcing steel ...bars. Therefore, the control method of the system and shape of the drill tip were improved. When the drill tip is stuck with chips and stopped, it is automatically pulled out and reinserted to recover the rotation. By changing the tip angle of the drill bit from 75° to 90°, it became possible to cut reinforcing bars, which were difficult to cut previously. In addition, we designed a crawler-type mechanical system and improved it such that it can be moved to the appropriate position and operated at any angle. This study revealed that the energy required for the drilling process accounts for more than 90% of the total dismantling energy. Through experiments using an impact hammer drill and observations of fracture surfaces using a three-dimensional scanner, we analysed the characteristics of reinforced concrete. In addition, the feasibility of the design for dismantling reinforced concrete was confirmed based on the determined energy associated with crack propagation.
This study demonstrates that a single-crystal diamond substrate can be cut along designed lines using the diamond-saw-wire cutting method. We developed an original saw-wire fixed diamond-grain using ...a bronze solder with a high melting temperature. We created a unique product machine system with a high vacuum furnace and a bronze solder that contains a metallic compound. The diamond cutting mechanism employed in this study is based on the mild wear phenomenon, owing to the friction between the diamond surfaces. A linear relationship between the cutting length and wire feed distance was observed. The relationship can be approximated as y = 0.3622x, where y (μm) is the cutting depth and x (km) is the wire feed distance. The life of the saw-wire was longer than that of the 6000 km wire feed distance and was tested by reciprocating an 8-m short wire at a speed, tension, and cutting force of 150 m/min, 1 N, and 0.2 N, respectively. A single crystal diamond substrate could be cut along the designed line, which was more than 2 mm long. The cutting speed was maintained constant at 0.36 μm/km.
