Dans les Hauts de la Réunion, l’élevage bovin constitue la principale activité agricole. L’élevage naisseur « professionnel » représente 8828 vaches mères pour 497 exploitations. Ce sont des ...exploitations de taille modeste (18 vaches mères en moyenne) dont les performances animales sont en-deçà de leurs homologues métropolitaines. L’objectif de l’étude est d’analyser les pratiques des éleveurs naisseurs et d’en faire le lien avec les performances obtenues afin d’identifier des voies d’amélioration potentielles. Pour cela, 10 éleveurs ont fait l’objet d’un suivi de 1999 à 2002, qui était fondé sur l’évaluation des performances des animaux et sur l’enregistrement des pratiques des éleveurs en particularisant l’allotement, la gestion du pâturage et la complémentation alimentaire. Cet article montre que les pratiques sont variées et conditionnées par des objectifs de production différents : stratégies de maximisation des performances ou du chargement, simplification de la conduite.
The purpose of this study was to describe the fracture incidence and bone mineral density (BMD) evolution in a large cohort of post-menopausal women with breast cancer after 3 years of aromatase ...inhibitor (AI) therapy.
A prospective, longitudinal study in real-life setting. Each woman had an extensive medical assessment, a biological evaluation, a BMD measurement, and systematic spinal X-rays at baseline and after 3 years of AI therapy. Women with osteoporosis at baseline (T-score < -2.5 and/or non-traumatic fracture history) were treated by oral weekly bisphosphonates.
Among 497 women (mean age 63.8±9.6 years) included in this study, 389 had a bone evaluation both at baseline and after 3 years of AI therapy: 267 women (mean age 61.2 ± 8.6) with no osteoporosis at baseline and 122 women (mean age 67.2 ± 9.1) with osteoporosis at baseline justifying a weekly oral bisphosphonate treatment. Women without bisphosphonates had a significant decrease in spine BMD (-3.5%, P < 0.01), neck BMD (-2.0%, P < 0.01), and total hip BMD (-2.1%, P < 0.01) over the 3 years but only 15 of them (5.6%) presented an incident vertebral or non-vertebral fracture. In osteoporotic women treated with bisphosphonates, spine and hip BMD were maintained at 3 years but 12 of them (9.8%) had an incident fracture. These fractured women were significantly older (74.1±9.8 versus 66.5 ± 8.8) but also presented BMD loss during treatment suggesting poor adherence to bisphosphonate treatment.
This real-life study confirmed that AIs induced moderate bone loss and low fracture incidence in post-menopausal women without initial osteoporosis. In women with baseline osteoporosis and AI therapy, oral bisphosphonates maintain BMD but were associated with a persistent fracture risk, particularly in older women.
Climate change is expected to drive species ranges towards the poles and to have a strong influence on species distributions. In this study, we focused on diadromous species that are of economical ...and ecological importance in the whole of Europe. We investigated the potential distribution of all diadromous fish regularly encountered in Europe, North Africa and the Middle East (28 species) under conditions predicted for twenty-first century climate change. To do so, we investigated the 1900 distribution of each species in 196 basins spread across all of Europe, North Africa and the Middle East. Four levels were used to semiquantitatively describe the abundance of species, that is missing, rare, common and abundant. We then selected five variables describing the prevailing climate in the basins, the physical nature of the basins and reflecting historical events known to have affected freshwater fish distribution. Logistic regressions with a four-level ordinal response variable were used to develop species-specific models. These predictive models related the observed distribution of these species in 1900 to the most explanatory combination of variables. Finally, we selected the A2 SRES scenario and the HadCM3 (Hadley Centre Coupled Model version 3) global climate model (GCM) to obtain climate variables (temperature and precipitation) at the end of this century. We used these 2100 variables in our models and obtained maps of climatically suitable and unsuitable basins, percentages of contraction or expansion for each species. Twenty-two models were successfully built, that is there were five species for which no model could be established because their distribution range was too narrow and the Acipenser sturio model failed during calibration. All the models selected temperature or/and precipitation as explanatory variables. Responses to climate change were species-specific but could be classified into three categories: little or no change in the distribution (five species), expansion of the distribution range (three species gaining suitable basins mainly northward) and contraction of the distribution (14 species losing suitable basins). Shifting ranges were in accordance with those found in other studies and underlined the high sensitivity of diadromous fish to modifications in their environment.
In January 2005, an epidemic of chikungunya fever broke out in the Comoro Islands and lasted until May 2005. In April, cases were also reported in Mayotte and Mauritius. On Réunion Island, the first ...cases were reported at the end of April. Surveillance of this epidemic required an adaptive system, which at first was based on active and retrospective case detection around the cases reported, then relied on a sentinel network when the incidence increased. Emerging and severe forms of infection were investigated. Death certificates were monitored. By April 2006, the surveillance estimate was 244,000 cases of chikungunya virus infection, including 123 severe cases and 41 of maternoneonatal transmission, with an overall attack rate of 35%. Chikungunya infection was mentioned on 203 death certificates and significant mortality was observed. This epidemic highlighted the need for a mutual strategy of providing information on arboviral diseases and their prevention and control between countries in the southwestern Indian Ocean.
Trihexyphenidyl (THP) is an anticholinergic drug misused to procure hallucination, sedation, and anxiolysis. The aim of this cohort was to show and describe, within a public health risk management ...policy, the risks of a long-standing but relatively unknown addiction: THP addiction.
On Réunion island, a cohort with systematic data collection has been set up by addictologists working in the Centres for Addiction Prevention and Treatment, in the university hospital, and in general practices who have active lists of patients misusing THP. Data collection included socioeconomic data and clinical data concerning addiction.
This cohort included 69 patients during November 2016. The average age of the patients was 36 years; 97% were men; 93% had living accommodation but only 32 % were employed. In this cohort drug administration was exclusively oral. The most common reasons for use were anxiolytic (46%), stimulation (26%), and sedation (10%), the main effects described were dyskinesia and behavioral disorders. Over half (61%) of the patients reported a coaddiction, mainly to benzodiazepines, cannabis, tobacco, alcohol, and buprenorphine.
This cohort describing the clinical characteristics of 69 patients is the largest cohort studied for THP addiction. Patients from the Centres for Addiction Prevention and Treatment were the youngest and most recently addicted, whereas general practice patients had been addicted for longer and were more socially integrated. This clinical description of THP addiction therefore enables us to identify the patients who are the most at risk, to set up an adapted care protocol.
