Scrub typhus, caused by mite-borne Orientia tsutsugamushi (O. tsutsugamushi), is a major febrile disease in the Asia-Pacific region. The DNA load of O. tsutsugamushi in the blood was previously found ...to be significantly higher in patients with fatal disease than those with non-fatal disease and correlated with the duration of illness, presence of eschar, and hepatic enzyme levels. In this prospective observation study, we analyzed the association of bacterial DNA load with clinical features, disease severity, and genotype using real-time PCR targeting the 56 kDa TSA gene of O. tsutsugamushi in the blood samples of 117 surviving patients with scrub typhus who had not received appropriate antibiotic treatment. The median O. tsutsugamushi DNA load was 3.11×103 copies/mL (range, 44 to 3.3×106 copies/mL). The severity of patients was categorized as mild, moderate, and severe based on the number of dysfunctional organs, and no significant difference in O. tsutsugamushi DNA load was found among these groups. Patients infected with the Karp group showed a significantly higher O. tsutsugamushi DNA load than those in the Gilliam (P < 0.05) and TA763 (P < 0.01) groups. Patients belonging to the Li ethnic group showed a significantly higher DNA load than those in the Han ethnic groups. The blood bacterial DNA load of patients showed no significant difference between groups divided by gender, age, with or without eschar, or the season of disease onset. The highest body temperature recorded during fever onset was positively correlated with O. tsutsugamushi DNA load (ρ = 0.272, P = 0.022). Correlation analyses indicated that the serum total bilirubin level was positively correlated with O. tsutsugamushi DNA load. In conclusion, the findings in this study demonstrated the association of DNA load of O. tsutsugamushi with the severity and genotype in patients with scrub typhus in Hainan, China.
Based on an e+e− collision data sample corresponding to an integrated luminosity of 567 pb−1 taken at the center-of-mass energy of s=4.6 GeV with the BESIII detector, we measure the absolute ...branching fraction of the inclusive decay Λc+→Λ+X to be B(Λc+→Λ+X)=(38.2−2.2+2.8±0.9)% using the double-tag method, where X refers to any possible final state particles. In addition, we search for direct CP violation in the charge asymmetry of this inclusive decay for the first time, and obtain ACP≡B(Λc+→Λ+X)−B(Λ¯c−→Λ¯+X)/B(Λc+→Λ+X)+B(Λ¯c−→Λ¯+X)=(2.1−6.6+7.0±1.6)%, a statistically limited result with no evidence of CP violation.
Fritillaria cirrhosa D. Don bulbs contain alkaloids and are one of the most intensively exploited alpine Himalayan medicinal species. In terms of proprietary medicines, our study shows that 210 F. ...cirrhosa products are offered by 46 suppliers, most of which (44) are situated in China and two in Nepal. A widespread commercial use is as one of the main ingredients in cough syrups. A well known example is "Nin Jiom Pei Pa Koa Herbal Cough & Throat Syrup", which typically contains more F. cirrhosa than any other herbal ingredient in the formulation. The biggest market for F. cirrhosa bulbs is China, where demand exceeds supply of this wild harvested species for use in traditional Chinese medicine (TCM). Cross-border trade from Nepal to China occurs in significant quantities. Bhutan also imports F. cirrhosa bulbs from Nepal. In addition, F. cirrhosa is registered as an active ingredient in traditional herbal medicinal preparations in Australia, Canada, Hong Kong SAR, Malaysia, Republic of Korea, Singapore and Taiwan. There is also an export trade in F. cirrhosa to Europe. Assessing how much F. cirrhosa is traded is complex, however, due to a “look-alike” challenge, as nine Chinese Fritillaria species are traded in Europe (Fritillaria cirrhosa, F. delavayi, F. hupehensis, F. pallidiflora, F. przewalskii, F. thunbergii, F. unibracteata, F. ussuriensis and F. walujewii).
The aims of this review were to assess the scale of the global trade in F. cirrhosa, and to synthesise studies of the impacts of wild harvest on F. cirrhosa populations and on the extent of emerging cultivation initiatives as an alternative to wild harvest.
Firstly, we reviewed published information on studies on impacts of wild F. cirrhosa harvest from across the geographic range of this species. Secondly, global trade data for F. cirrhosa were analysed.
The principal demand for F. cirrhosa bulbs is in China, where hundreds of different companies produce Fritillaria preparations. Trade data also show that in 2013, China exported over 44 tonnes of F. cirrhosa bulbs to Taiwan and 26.7 tonnes to the Republic of Korea. Extensive commercial use and limited wild stocks result in a high price (2000 – 3800 CNY per kg (around US$ 303 –560 per kg in 2017)) for F. cirrhosa bulbs. Prices of cultivated Fritillaria bulbs are much lower (600–680 CNY per kg in 2017) than wild harvested bulbs. But due to very specific growth requirements of F. cirrhosa, cultivation is not yet able to meet total demand. The consequence is continued exploitation of wild stocks. At the same time, however, an increasing proportion of the demand is met by cultivation of alternative Fritillaria species that are easier to grow than F. cirrhosa. The air-dry mass of F. cirrhosa bulbs varies between 0.0917 and 0.1116 g per bulb. This represents 8960 – 10,900 bulbs/kg or 8.9 – 10.9 million bulbs per tonne. Current demand therefore represents billions of bulbs per year.
Demand for F. cirrhosa bulbs, particularly from China, makes this species one of the most intensively harvested alpine Himalayan medicinal bulbs. Although F. cirrhosa is listed as a Class III protected species in China, billions of these tiny, wild harvested bulbs are sold per year. Due to demand exceeding supply, the price of F. cirrhosa bulbs has increased dramatically. Between 2002 and 2017, for example, the price of wild harvested F. cirrhosa bulbs increased over nine-fold, from the equivalent of US$60 in 2002 to US$560 per kg in 2017. To date, cultivation has been unable to meet the entire market demand for F. cirrhosa bulbs, although other Fritillaria species are successfully cultivated on a larger scale.

Background. Algorithms for the diagnosis of syphilis continue to be a source of great controversy, and numerous test interpretations have perplexed many clinicians. Methods. We conducted a ...cross-sectional study of 24 124 subjects to analyze 3 syphilis testing algorithms: traditional algorithm, reverse algorithm, and the European Centre for Disease Prevention and Control (ECDC) algorithm. Every serum sample was simultaneously evaluated using the rapid plasma reagin, Treponema pallidum particle agglutination, and chemiluminescence immunoassay tests. With the results of clinical diagnoses of syphilis as a gold standard, we evaluated the diagnostic accuracy of the 3 syphilis testing algorithms. The κ coefficient was used to compare the concordance between the reverse algorithm and the ECDC algorithm. Results. Overall, 2749 patients in our cohort were diagnosed with syphilis. The traditional algorithm had the highest negative likelihood ratio (0.24), a missed diagnosis rate of 24.2%, and only 75.81% sensitivity. However, both the reverse and ECDC algorithms had higher diagnostic efficacy than the traditional algorithm. Their sensitivity, specificity, and accuracy were 99.38%–99.85%, 99.98%–100.00%, and 99.93%–99.96%, respectively. Moreover, the overall percentage of agreement and κ value between the reverse and the ECDC algorithms were 99.9% and 0.996, respectively. Conclusions. Our research supported use of the ECDC algorithm, in which syphilis screening begins with a treponemal immunoassay that is followed by a second, different treponemal assay as a confirmatory test in high-prevalence populations. In addition, our results indicated that nontreponemal assay is unnecessary for syphilis diagnosis but can be recommended for determining serological activity and the effect of syphilis treatment.

Background. The prozone phenomenon is known to be associated with high antibody titers; other associations, such as host factors, have not been elucidated. Methods. A retrospective analysis was ...conducted to evaluate the incidence of the prozone phenomenon of the syphilis rapid plasma reagin (RPR) test among 46 856 clinical samples, between June 2010 and June 2013. Logistic regression was used to analyze the risk factors of the prozone phenomenon. Results. Our results showed that the incidence of the prozone phenomenon was low (0.83%) and could occur during any clinical phase, particularly during primary and secondary syphilis. Pregnancy and neurosyphilis were associated with the prozone phenomenon; sex, age, and whether the patient had been treated were not. The results also revealed that the prozone phenomenon not only occurred in patients with a high titer but also could occur in patients with a moderate/low titer. In fact, almost 31% of the patients with the prozone phenomenon had titers ≤1:16. Conclusions. The prozone phenomenon in the RPR test was associated with the phase of syphilis, pregnancy, and neurosyphilis as well as a range of RPR titers between 1:8 and 1:512. This latter finding is in contrast to previous reports that the prozone phenomenon is associated with very high RPR titers.
ABSTRACT We report calculations of energy levels and radiative rates for transitions among the lowest 116 fine-structure levels arising from the configurations in Be-like ions with Z = 10-30. The ...wavelengths, oscillator strengths, line strengths, and radiative rates for all possible electric dipole, magnetic dipole, electric quadrupole, and magnetic quadrupole transitions among the 116 levels have been calculated using the combined configuration interaction and many-body perturbation method. The accuracy of the results is determined through extensive comparisons with existing laboratory measurements and theoretical results. The present complete set of results should be of great help in line identification and the interpretation of spectra, as well as in the modeling and diagnostics of astrophysical and fusion plasmas.
In this letter, we present a low-power photodetector with high responsivity and unique capability to optimize detectivity under different illumination condition. Built on 22 nm fully depleted ...silicon-on-insulator (FDSOI) platform, the photodetector has a hybrid SOI/bulk substrate. An NMOSFET is fabricated in the SOI region, while a surface p+/n photodiode is fabricated within the n-well of the bulk-Si region. These two regions electrically share the common p-Si substrate, working both as the back gate of the NMOSFET and the p-type component of the inner n-well/p-Si substrate photodiode. The two photodiodes design not only increases the light absorption efficiency but also enhances the photovoltage-induced channel control of the NMOSFET. The NMOSFET is also designed to just have a threshold voltage close to the photovoltage, enabling a low power consumption. An adjustable bias is independently applied to the n-well, facilitating detectivity optimization under various illumination conditions. A high responsivity of ~105 A/W with a low dark power consumption of ~1 pW for a drain voltage of 0.1 V is demonstrated.
Patients with DMD have demonstrated functional abnormalities in the motor-related brain areas in previous PET, MRS, and TMS studies. We applied structural MR imaging and RS-fMRI in patients with DMD ...for the first time, and aimed to investigate the GMC and ReHo or local synchronization of spontaneous activity in the motor cortex.
Ten boys with DMD (6.4-14.0 years of age) and 15 healthy controls (7.9-15.1 years of age) underwent brain structural MR imaging and RS-fMRI scanning. GMC and local synchronization of spontaneous activity in the motor cortex were analyzed by using VBM and ReHo approaches, respectively.
Compared with healthy controls, boys with DMD showed decreased GMC in the left PSMC and decreased ReHo in the bilateral PMSC as well as in the supplementary motor area (P < .05, corrected).
The current results indicate that boys with DMD have both GMC loss and decreased local synchronization of spontaneous activity in the motor cortex, which might be due to the deficiency of dystrophin in the brain.
