Many human cancers develop as a result of exposure to risk factors related to the environment and ways of life. The aim of this study was to estimate attributable fractions of 25 types of cancers ...resulting from exposure to modifiable risk factors in Brazil. The prevalence of exposure to selected risk factors among adults was obtained from population-based surveys conducted from 2000 to 2008. Risk estimates were based on data drawn from meta-analyses or large, high quality studies. Population-attributable fractions (PAF) for a combination of risk factors, as well as the number of preventable deaths and cancer cases, were calculated for 2020. The known preventable risk factors studied will account for 34% of cancer cases among men and 35% among women in 2020, and for 46% and 39% deaths, respectively. The highest attributable fractions were estimated for tobacco smoking, infections, low consumption of fruits and vegetables, excess weight, reproductive factors, and physical inactivity. This is the first study to systematically estimate the fraction of cancer attributable to potentially modifiable risk factors in Brazil. Strategies for primary prevention of tobacco smoking and control of infection and the promotion of a healthy diet and physical activity should be the main priorities in policies for cancer prevention in the country.
Exposure to pesticides has been associated with mental disorders, especially in occupationally exposed populations, such as farmers. This effect has been attributed to the neurotoxic and ...endocrine-disrupting activity of pesticides, as suggested by experimental studies.
To determine the prevalence of common mental disorders and self-reported depression, and analyze their association with the exposure to pesticides in a rural population resident in the municipality of Dom Feliciano, Rio Grande do Sul, where tobacco farming is the main economic activity.
A cross-sectional study evaluating the prevalence of common mental disorders and self-reported depression in a sample of 869 adult individuals resident in Dom Feliciano, between October 2011 and March 2012 was performed. The evaluation of common mental disorders was performed using the Self-Reporting Questionnaire (SRQ-20), setting a cutoff point of 8 for both genders. A standardized questionnaire was used to obtain information on self-reported depression upon prior diagnosis by a health professional, and self-reported exposure to pesticide. In order to evaluate the association between exposure to pesticides and mental disorders, a non-conditional multivariate logistic regression analysis was performed.
The prevalence of common mental disorders and self-reported depression in the sample population were 23% and 21%, respectively. Among individuals who reported depression, an increase of 73% was observed in the odds of pesticide exposure at an age equal to or less than 15 years. There was a positive association between self-reported pesticide poisoning and common mental disorders (OR=2.63; 95% CI, 1.62-4.25) as well as self-reported depression (OR=2.62; 95% CI, 1.634.21). Individuals who reported depression had a greater odds of exposure to pyrethroids (OR=1.80; 95% CI, 1.013.21) and aliphatic alcohol (OR=1.99; 95% CI, 1.043.83). An SRQ208 was associated with an approximately seven times higher odds of exposure to aliphatic alcohol (95% CI, 1.7327.53). Self-reported depression positively correlated with a greater period of exposure to dinitroaniline (OR=2.20; 95% CI, 1.034.70) and sulphonylurea (OR=4.95; 95% CI, 1.0623.04).
The results suggest that exposure to pesticides could be related mental disorders. However, other common risk factors in tobacco farming, the main local economic activity, cannot be excluded.
IntroductionBrazil is one of the world’s largest consumers of pesticides and this intense use impacts not only the environment but also exposes a wide range of individuals, such as rural workers who ...are occupationally exposed to pesticides and also the residents of the rural area environmentally exposed.ObjectivesTo evaluate occupational exposure to pesticides and to identify the neurotoxic and genotoxic effects in rural workers (RW) and rural residents (RR).MethodsA sectional epidemiological study was carried out with 104 RW and 23 RR of the city of Casimiro de Abreu (RJ/Brazil). A comparison group (CG) was formed with 103 residents of the urban area of the same city. Genotoxic analyses, through the comet assay and micronucleus test (MN), and the evaluation of the activity of cholinesterase enzymes (AChE and BChE), were performed.ResultsA reduction in cholinesterase enzyme activity was observed, mainly for butyrylcholinesterase of RW and RR when compared to CG (RW = 3856.40; RR = 3956.04; CG = 4359.57; p = 0.002). An increase in genotoxic effects in RW when comparing CG were observed (comet assay: RW = 21; CG = 10; p <0.001 and MN number: RW = 6.50; CG = 3.00; p <0.001), demonstrating that individuals occupationally exposed to pesticides are more likely to have genotoxic effects when compared to non-exposed individuals.ConclusionThe findings from this research will serve to support the execution of programs to monitor populations exposed to neurotoxic and genotoxic substances and allow the development of strategies for the prevention, control and surveillance of effects generated by occupational and environmental exposures to pesticides.
Cancer is the second leading cause of death in the world with great impact on public health and leukemia is a hematological cancer directly related to different exposures at work. This study aimed to ...describe the occupational profile of individuals diagnosed with leukemia. This is a cross-sectional study of cases registered between 2007 and 2011 in the Integrador RHC database. Individuals from 26 Brazilian states, aged 20 years or older, were included. Of the 7,807 cases of leukemia, Minas Gerais recorded the highest occurrence (1,351). Only 52% of the cases had information on occupation. Occupations with the greatest number of cases of leukemia were agricultural, forestry and fishing workers; services, stores and markets vendors; and workers in the production of industrial goods and services. These occupations are exposed to substances considered by literature as carcinogenic agents to humans. There was a high underreporting of occupational data, compromising the quality of information and, therefore, the effectiveness of the Brazilian health surveillance system. The RHC also does not provide information about the agent used during the working day, the exposure time during working life and data from previous occupations.
