Purpose
The aim of this study was to investigate the association between fatigue and phase angle (PA) in patients with chronic kidney disease (CKD) on hemodialysis (HD).
Methods
A cross-sectional ...study with older patients than 18 years diagnosed with CKD undergoing HD. From 160 (58.36±15.05 years) patients, (n=96; 60%) are men. Body mass and height were assessed using a portable scale and stadiometer, followed by body mass index (BMI) calculus. The bioimpedance electrical analysis was performed using the Bodystat QuadScan 4000. The sample size was dichotomized in two groups, using the median of our sample, either normal when PA ≥5.4°, or low when the PA <5.4°. The Chalder fatigue questionnaire was used the assess the fatigue. The multiple regression was applied to assess the association between fatigue questionnaire and PA.
Results
Were considered normal PA≥5.4° (n=78, 48.8%) and low PA<5.4° (n=82, 51.2%). The patients of the PA<5.4° group are older compared to the PA≥5.4° group (63.1±15.1 vs. 53.3±13.4 years, p<0.001). There was no association between PA and fatigue score in the crude model (OR: 1.02, 95%CI: 0.96–1.08, p=0.47) and after confounding variables (OR: 1.03, CI: 0.95–1.12, p=0.43).
Conclusions
In HD patients, we found that patients with lower PA values are older. In addition, we did not find association between fatigue and PA.
Objectives
In patients with chronic kidney disease, sarcopenia is associated with dialysis treatment, accelerated protein catabolism, and high energy demand. Thus, this study aimed to assess the ...association between 25-OH vitamin D concentrations and muscle function in patients undergoing hemodialysis.
Design
A cross-sectional study.
Setting
Performed in a hemodialysis clinic.
Participants
79 adult and elderly patients undergoing hemodialysis.
Measurements
The sample was divided according to the SARC-F definition, being 55 patients allocated in the SARC-F <4 group and 24 in the SARC-F ≥4 group. Normal 25-OH vitamin D concentrations was considered when ≥30 ng/dL.
Results
Vitamin D concentrations and number of patients with low or normal muscle function did not differ between the groups. There was no correlation between SARC-F and 25-OH vitamin D levels (r: −0.09, p=0.42).
Conclusions
25-OH vitamin D has no association with muscle function loss in adults and elderly patients undergoing hemodialysis.

: 22 healthy adult women were included in a randomized, double-blind, placebo-controlled crossover study. On two separate occasions, 1 week apart from each other, we offered 800 mg of corn starch (placebo) or 752 mg of EGCG. Appetite was assessed through gastric emptying; perceptions of hunger, desire to eat and satiation; and plasma insulin, adiponectin, leptin and glucose concentrations. The evaluations were carried out in fasting, 30, 90 and 150 min after supplementation.
: EGCG supplementation induced higher relative gastric volume at 30 and 90 min. Satiation at 90 min was higher in the EGCG group. Adiponectin concentrations at 150 min were higher with EGCG, but no difference was found for glucose, insulin and leptin concentrations.
: Acute EGCG supplementation is able to delay gastric emptying in healthy women to a small, but statistically significant extent. This study was registered at the Brazilian Registry of Clinical Trials (ReBEC) as RBR-9svwrv.
The aim was to evaluate whether the ingestion of a high protein diet containing dairy protein is associated with anthropometric indicators of adiposity and blood glucose.
A cross-sectional study was ...conducted with volunteers of 20-89 years of age who performed leisure physical activity. We assessed dietary intake, body mass index (BMI), waist circumference (WC), triceps skinfold thickness (TSFT), random blood glucose as well as sociodemographic and behavioral variables.
A total of 418 individuals were evaluated. The consumption of a high-protein diet (1.80 ± 0.49 g/kg/day) was found in 37.8% of individuals, which showed lower BMI, WC, TSFT and blood glucose concentrations compared to those with a low-protein diet (0.56 ± 0.18 g/kg/day). Dairy products consumption was inversely associated with BMI when adjusted for sex and age (odds ratio (OR): 0.79, 95% confidence interval (CI): 0.68-0.93,
= 0.004) and by sex, age, fiber and energy (OR: 0.79; 95% CI: 0.67-0.92;
= 0.004), and with blood glucose when adjusted for sex and age (OR: 0.86; 95% CI: 0.74-0.99;
= 0.042). Cheese consumption was inversely associated with blood glucose when adjusted for sex and age (OR: 0.73, 95% CI: 0.55-0.96,
= 0.023) and by sex, age, calories and fibers (OR: 0.74, 95% CI: 0.56-0.98,
= 0.036). Two portions of cheeses/day reduced the risk of having high blood glucose levels by approximately 80%.
A high-protein diet containing dairy food, in particular two servings of cheese, was associated with low BMI and random glucose concentration.

Purpose
Allogeneic hematopoietic cell transplant recipients undergo myelosuppressive chemotherapy to allow engraftment of stem cells and are at particularly high risk for bacterial infections and ...adverse outcomes. Patients undergoing hematopoietic cell transplant are at increased risk for healthcare-associated infections, including infections with multidrug-resistant pathogens. Cefepime is a commonly prescribed antibiotic for empiric therapy in hematopoietic cell transplant patients, but there is minimal data describing cefepime resistance rates, risk factors for resistance, and clinical outcomes associated with cefepime-resistant infections.
Methods
Adult (≥18 years old) allogeneic hematopoietic cell transplant recipients with a culture positive for a gram-negative rod between January 2010 and January 2016 were spilt into two groups: cefepime susceptible and cefepime nonsusceptible . The primary objective of this study was to identify risk factors for cefepime nonsusceptible through multivariable logistic regression.
Results
A total of 107 patients were included (27 cefepime nonsusceptible, 80 cefepime-susceptible), yielding a 25.2% nonsusceptibility rate. Multivariable analysis yielded age >60 years old, Klebsiella spp. infection, Acinetobacter spp. infection, healthcare exposures within 90 days, acute gastrointestinal graft-vs-host-disease, and chronic graft-vs-host-disease at multiple locations as significant risk factors for cefepime nonsusceptible. The receiver operating characteristic area under the curve of the model was 0.851. Thirty-day all-cause mortality (29.6% versus 16.3%, p = 0.13) and length of hospitalization (19 versus 12.5 days, p = 0.0650) were numerically higher in the cefepime nonsusceptible group.
Conclusions
Hematopoietic cell transplant patients with acute gastrointestinal graft versus host disease, extensive chronic graft-vs-host-disease, advanced age, previous healthcare exposures, or infections with Klebsiella and Acinetobacter are at increased risk for cefepime nonsusceptible. Patients infected with cefepime nonsusceptible pathogens may have higher rates of mortality and length of hospitalization.
In the frame of the multi-scale approach of the fracture toughness prediction defined in the PERFECT project, we proposed a new crystal plasticity model and applied it to the computation of stress ...heterogeneities within a reference polycrystalline aggregate defined in the project RPV material.
The proposed crystal plasticity model is able to take into account the effects of temperature and irradiation hardening. The analysis of the results of aggregate computations shows that the distributions of the maximum values of the maximal principal stresses are found to be well described by a Gumbell function. Applying these distributions on a Griffith criterion allows settling the basis of an original fracture criterion. However the increasing resistance to fracture of the steel with temperature can be reproduced only by introducing a temperature dependence of the fracture energy.
Long-standing evidence indicates that human immunodeficiency virus type 1 (HIV-1) preferentially integrates into a subset of transcriptionally active genes of the host cell genome. However, the ...reason why the virus selects only certain genes among all transcriptionally active regions in a target cell remains largely unknown. Here we show that HIV-1 integration occurs in the outer shell of the nucleus in close correspondence with the nuclear pore. This region contains a series of cellular genes, which are preferentially targeted by the virus, and characterized by the presence of active transcription chromatin marks before viral infection. In contrast, the virus strongly disfavours the heterochromatic regions in the nuclear lamin-associated domains and other transcriptionally active regions located centrally in the nucleus. Functional viral integrase and the presence of the cellular Nup153 and LEDGF/p75 integration cofactors are indispensable for the peripheral integration of the virus. Once integrated at the nuclear pore, the HIV-1 DNA makes contact with various nucleoporins; this association takes part in the transcriptional regulation of the viral genome. These results indicate that nuclear topography is an essential determinant of the HIV-1 life cycle.
