Izhodišča: Čeprav so srčne kateterizacije v primerjavi s kardiokirurškimi posegi manj invazivne, imajo manj zapletov, bolniki pa po njih hitreje okrevajo, te niso brez zapletov. Z raziskavo smo ...želeli ugotoviti značilnosti populacije, najpogostejše patologije, vrste kateterizacij, zaplete pri kateterizacijah v našem centru ter opredeliti morebitne dejavnike tveganja.
Metode: Zbrali smo podatke o vseh pediatričnih srčnih kateterizacijah, opravljenih med junijem 2018 in avgustom 2021. Opredelili smo jih glede na vrsto kateterizacije (diagnostične in terapevtske), težo, starost, spol bolnikov in kompleksnost patologije. Zaplete smo ločili na lažje in težje ter z uporabo Pearsonovega testa hi-kvadrat in testa Mann-Whitney U ugotavljali njihovo povezanost s morebitnimi dejavniki tveganja.
Rezultati: V tem obdobju je bilo opravljenih 191 srčnih kateterizacij (54 diagnostičnih in 137 terapevtskih) pri 175 otrocih, od tega pri 95 dečkih in 80 deklicah. Otroci so bili stari od nekaj ur do 18 let, tehtali so od 2,6 kg do 85,5 kg. Zabeležili smo 9 zapletov, od tega 5 težjih in 4 lažje. Z našo raziskavo nismo dokazali statistično pomembne povezave med dejavniki tveganja (telesna teža, spol, vrsta kateterizacije, starost, kompleksnost patologije) in pojavom zapletov.
Zaključek: Največ kateterizacij je bilo opravljenih pri otrocih, starih med 1 letom in 10 let in tistih, ki so tehtali med 10 in 40 kg. Nekoliko več kateterizacij je bilo opravljenih pri dečkih. Najpogostejše so bile terapevtske kateterizacije. Pojavnost zapletov v našem centru je primerljiva z drugimi centri. Telesna teža, spol, starost otroka, vrsta kateterizacije in kompleksnost patologije v našem centru niso bili statistično značilno povezani s pojavom zapletov ob katerizaciji.
To estimate the incidence and describe the spectrum of inflammatory and autoimmune diseases linked to SARS-CoV-2 infection and COVID-19 vaccination in children from two neighbouring south central ...European countries.
We performed a multi-centre prospective cohort study of children under 18 years diagnosed with inflammatory/autoimmune diseases linked to SARS-CoV-2 infection or COVID-19 vaccination, who were admitted to the paediatric tertiary care hospitals in Slovenia and Friuli Venezia Giulia, Italy, from January 1, 2020, to December 31, 2021. Disease incidence was calculated based on laboratory-confirmed cases only.
Inflammatory and autoimmune diseases linked to SARS-CoV-2 were diagnosed in 192 children (127 laboratory-confirmed), of whom 112 had multisystem inflammatory syndrome (MIS-C), followed by vasculitis, neurological and cardiac diseases. Calculated risk of MIS-C was 1 in 860 children after SARS-CoV-2 infection and cumulative incidence of MIS-C was 18.3/100,000 of all children. Fifteen children had severe COVID-19. Two patients with MIS-C and a patient with myositis presented after COVID-19 vaccination. All 3 had at presentation also a serologically proven recent SARS-CoV-2 infection. After MIS-C, nine patients were vaccinated against COVID-19 and 25 patients had a SARS-CoV-2 reinfection, without recurrence of MIS-C.
Autoimmune diseases following SARS-CoV-2 infection in children were 8.5 times as common as severe COVID-19. MIS-C was the most common manifestation and its incidence in this predominantly white population was higher than previously reported. MIS-C does not seem to recur after SARS-CoV-2 reinfection or COVID-19 vaccination. Autoimmune diseases were much more common after SARS-CoV-2 infection than after COVID-19 vaccination.
Objective
To evaluate the effect of interhospital air and ground transportation of artificially ventilated neonates on heart rate and peripheral blood leukocyte counts.
Design
Prospective, ...observational study.
Setting
Level III multidisciplinary Neonatal and Pediatric Intensive Care Unit.
Patients
Fifty-eight near-term artificially ventilated transported neonates between May 2006 and April 2007.
Interventions
Day-helicopter, day- and night-ground transportation.
Measurements and results
Heart rate at retrieval, on admission to the ICU and 1 h later, and peripheral blood leukocyte counts on admission and 1 d later were compared. Fifteen neonates were transported by helicopter during the daytime (D-HEL), 20 by daytime ground and 23 by nighttime ground transportation (D-GROUND, N-GROUND). No differences in delivery mode, birth weight, gestational age, gender, primary diagnoses for transportation, response time and duration of transportation were found between the groups. , blood pressure and skin temperature at retrieval and on admission to the ICU were found between the three groups. The mean heart rate at retrieval did not differ significantly, while on arrival in the ICU and 1 h later the D-GROUND group of patients showed a significantly higher mean heart rate compared to the D-HEL and N-GROUND groups. Moreover, leukocyte counts on arrival in the ICU showed significantly higher leukocyte counts in the D-GROUND group of patients compared to the D-HEL group of patients.
Conclusions
These results demonstrate that there is an association between daytime ground transportation and higher heart rate and peripheral blood leukocytes.
Introduction. Cyanosis and dyspnoea with interatrial shunting of blood occurring without change in body position are termed platypnea-orthodeoxia-like syndrome.
Case presentation. We describe a ...female newborn with sepsis, who developed cyanosis. Two weeks after the initial appearance of cyanosis she presented with a right-sided diaphragmatic hernia, which was not seen on the initial X-rays. The hernia was surgically repaired. Her postoperative course was uneventful.
Conclusions. In our case, interatrial shunting of blood was presumably caused by disturbed intrathoracic pressures. Positive pressure ventilation and sepsis may also contribute to the development of such a syndrome.
A total of 249 serum samples from 13 wild animal species namely fallow deer (Dama dama, n = 1), roe deer (Capreolus capreolus, n = 80), red deer (Cervus elaphus, n = 22), chamois (Rupicapra ...rupicapra, n = 21), mouflon (Ovis musimon, n = 4), brown hare (Lepus europaeus, n = 2), nutria (Myocastor coypus, n = 1), red fox (Vulpes vulpes, n = 97), stone marten (Martes foina, n = 12), European badger (Meles meles, n = 2), golden jackal (Canis aureus, n = 2) Eurasian lynx (Lynx lynx, n = 2) and grey wolf (Canis lupus, n = 3) were analysed for the presence of antibodies against Leptospira interrogans sensu stricto. Serum samples were examined via the microscopic agglutination test for the presence of specific antibodies against Leptospira serovars Icterohaemorrhagiae, Bratislava, Pomona, Grippotyphosa, Hardjo, Sejroe, Australis, Autumnalis, Canicola, Saxkoebing and Tarassovi. Antibodies to at least one of the pathogenic serovars were detected in 77 (30.9%; CI = 25–37%) sera. The proportion of positive samples varied intraspecifically and was the biggest in large carnivores (lynx, wolf and jackal; 86%), followed by mezzo predators: stone marten (67%) and red fox (34%), and large herbivores: red deer (32%), roe deer (25%), alpine chamois (10%) and mouflon (0%). Out of the 77 positive samples, 42 samples (53.8%) had positive titres against a single serovar, while 35 (45.4%) samples had positive titres against two or more serovars. The most frequently detected antibodies were those against the serovar Icterohaemorrhagiae. The present study confirmed the presence of multiple pathogenic serovars in wildlife throughout Slovenia. It can be concluded that wild animals are reservoirs for at least some of the leptospiral serovars and are a potential source of leptospirosis for other wild and domestic animals, as well as for humans.
