Acute zonal occult outer retinopathy (AZOOR) is a rare syndrome characterized by sudden onset of photopsia, scotomas, and abnormal electrophysiological tests, predominantly affecting young women. ...Although its pathogenesis remains unknown, auto-reactivity to retinal components is thought to mediate tissue damage. A 42-year-old woman presented with symptoms and examination consistent with the diagnosis of AZOOR. She was treated with azathioprine for 5 years. In spite of the immunosuppressive treatment, clear progression in the visual field, autofluorescence, electrophysiological tests and optical coherence tomography was observed. Treatment with intravenous immunoglobulins (IVIg) and subcutaneous Abatacept was subsequently started with little efficacy. Hereby, we present a case of progressive AZOOR despite aggressive immunosuppression with 10-year follow up. Currently, there is no consensus regarding management of AZOOR, and the convenience of administering aggressive immunosuppression remains uncertain.
PurposeThe aim of this study was to describe the cellular infiltrate in aqueous and vitreous samples of patients with uveitis analysed by multiparametric flow cytometry.MethodsThis is a retrospective ...analysis of aqueous and vitreous samples analysed by flow cytometry for diagnostic purposes, in cases of masquerade syndromes and infectious and non-infectious uveitis. Data collected included demographics, anatomical classification of uveitis, phenotypic diagnosis, anterior chamber cells grading, vitreous haze and time of follow-up since presentation to sample obtained.ResultsThirty-one samples (17 aqueous and 14 vitreous fluids) from 31 patients, 18 men, were analysed. The mean age at the time of sample collection was 60.23±17.03 years. The most frequent anatomical classification was panuveitis (14 of 31). T cells accounted for the main cellular component in the majority of the samples (10 of 13 aqueous samples; 7 of 14 in vitreous samples). CD4:CD8 ratios ranged from 0.21 to 16.3 in the case of aqueous samples and from 0.5 to 9.7 in the case of vitreous samples.DiscussionFlow cytometry analysis of aqueous and vitreous samples from patients with uveitis could provide insight into the pathogenesis of human uveitis and help develop accurate animal models which better mimic human disease.
To describe a retrospective review of HIV patients with noninfectious uveitis. Data collected included: demographics, anatomic classification and phenotypic diagnosis of the uveitis, systemic ...immune-mediated disorders (IMD), time from HIV diagnosis to uveitis, CD4 count, viral load, treatment and complications of treatment and time of follow-up.
Twenty patients (18 males) were included. The time lag between HIV diagnosis and the onset of uveitis was 9 ± 8.5 years. Mean CD4 count was 670 ± 294 cells/ml. Viral load was undetectable in 14 out of 18 cases. In 6 patients IMD was diagnosed prior to or concurring with the uveitis diagnosis. The use of immunosuppressive therapies was necessary in 6 patients (including biologics in 4 cases). The mean follow-up was 42.2 months.
noninfectious uveitis could be the first manifestation of IMD in patients with well-controlled HIV infection. Immunosuppression appeared to be a safe therapeutic option in our cohort of patients.
To identify the genetic causes underlying early-onset autosomal recessive retinitis pigmentosa (arRP) in the Spanish population and describe the associated phenotype.

A total of 244 ...unrelated families affected by early-onset arRP.
Homozygosity mapping or exome sequencing analysis was performed in 3 families segregating arRP. A mutational screening was performed in 241 additional unrelated families for the p.Ser452Stop mutation. Haplotype analysis also was conducted. Individuals who were homozygotes, double heterozygotes, or carriers of mutations in RP1 underwent an ophthalmic evaluation to establish a genotype-phenotype correlation.

Four novel mutations in RP1 were identified. The new mutation p.Ser542Stop was present in 11 of 244 (4.5%) of the studied families. All chromosomes harboring this mutation shared the same haplotype. All patients presented a common phenotype with an early age of onset and a prompt macular degeneration, whereas the heterozygote carriers did not show any signs of retinitis pigmentosa (RP).
p.Ser542Stop is a single founder mutation and the most prevalent described mutation in the Spanish population. It causes early-onset RP with a rapid macular degeneration and is responsible for 4.5% of all cases. Our data suggest that the implication of RP1 in arRP may be underestimated.

RESUMEN Los fenotipos de obesidad se presentan en individuos con igual índice de masa corporal que tienen diferentes perfiles metabólicos y pronósticos de salud. Su presencia desde etapas tempranas ...de la vida hace que incremente la probabilidad de que una mujer arribe al embarazo con estas características, por lo que es necesario promover un posicionamiento conceptual para su identificación. En gestantes normopeso, se sugiere identificar el fenotipo normopeso obeso cuando presenta valor igual o superior al 30 % de la grasa corporal o al 90 percentil de la suma de pliegues cutáneos tricipital y subescapular. De ellas, las que tengan valores iguales o superiores al 75 percentil del índice de adiposidad visceral y del producto de acumulación de los lípidos, se consideran normopeso metabólicamente obesas. En las obesas se propone el uso de los criterios que definen al síndrome metabólico en mujeres, con valores ajustados para gestantes, para identificar la salud metabólica. Los argumentos expuestos demuestran lo idóneo de estratificar el riesgo metabólico al inicio de la gestación al clasificarlas en fenotipos de obesidad, mediante indicadores antropométricos, bioquímicos y clínicos que identifican al síndrome metabólico.
To describe the different phenotypes of syphilitic chorioretinitis (SCR) and its clinical features in multimodal imaging (MI).
Complete ophthalmological examination and MI including spectral domain ...optic coherence tomography (SD-OCT), fundus autofluorescence (FAF) and fluorescein angiography (FA) were performed at diagnosis and during follow-up in patients diagnosed with SCR.
17 eyes of 10 patients were included. Clinical phenotypes included acute syphilitic posterior placoid chorioretinopathy (ASPPC), syphilitic outer retinitis (SOR) and punctate inner retinitis (PIR). Disruption of outer retinal layers were visible in all patients in SD-OCT. Inner retinal layers were only affected in PIR. FAF revealed macular hyperautofluorescence in all patients. FA showed maintained hyperfluorescence in ASPPC and SOR.
ASPPC, SOR and PIR are different phenotypes of syphilitic chorioretinitis and present common features in SD-OCT and FAF. MI provides useful information to help the clinician during diagnosis and follow-up of these entities.
RESUMEN Introducción: El crecimiento y el desarrollo son fenómenos biológicos que implican aumento de masa celular y diferenciación de forma o función, respectivamente. En beneficio del diagnóstico, ...se utilizan las bondades del ultrasonido en la evolución biométrica del crecimiento fetal. Objetivo: Determinar mensuraciones biométricas fetales más asociadas a la restricción del crecimiento fetal en infantes que sufrieron restricción del crecimiento intrauterino. Métodos: Se realizó un estudio longitudinal descriptivo y retrospectivo de gestantes captadas en dos áreas de salud del municipio Santa Clara, que terminaron su embarazo entre septiembre del 2013 y octubre del 2018 y cuyos recién nacidos experimentaron restricción del crecimiento. La muestra se clasificó al nacimiento en pequeños y adecuados, según condición trófica y en cada grupo se estudió relación con valores percentilares de variables biométricas en los dos últimos trimestres. Resultados: Predominaron en todas las biometrías y en los dos trimestres los valores por debajo del décimo percentil. En la totalidad de los infantes y durante todo el período fetal, la circunferencia abdominal estuvo por debajo del percentil 10. En las variables largo del fémur y circunferencia cefálica los percentiles más altos se encontraron en el tercer trimestre y desde el segundo en la variable diámetro biparietal, posiblemente relacionado con restricciones asimétricas del crecimiento. Conclusiones: La variable biométrica circunferencia abdominal desde el segundo trimestre es fiable en la detección de restricciones del crecimiento intrauterino y su existencia por debajo del décimo percentil debe servir como certeza de su existencia, aun cuando la condición trófica del recién nacido parezca revelar otra realidad.
RESUMEN Introducción: La adiposidad central como factor desencadenante de resistencia a la insulina precoz constituye una amenaza potencial de riesgo metabólico y cardiovascular en el embarazo. ...Objetivo: Determinar la capacidad discriminante de las grasas abdominales sobre la resistencia a la insulina, diagnosticada por el índice triglicéridos/glucosa-IMC al finalizar el primer trimestre del embarazo. Métodos: Se realizó un estudio observacional analítico de 526 gestantes con embarazo simple y edad gestacional entre 12 y 13 semanas, entre los años 2016 y 2020. Se estudió el test de triglicéridos/glucosa-IMC y las grasas abdominales por ultrasonido. Se utilizaron las curvas ROC (Receiver operating characteristic Curve) para discriminar la resistencia a la insulina al finalizar el primer trimestre de la gestación, cuando aumentan las grasas abdominales. Resultados: La grasa subcutánea fue la que presentó mayor área bajo la curva en la discriminación de la resistencia a la insulina, con un nivel de sensibilidad y especificidad aceptable. Conclusiones: La grasa subcutánea, aunque con bajo valor discriminativo, puede considerarse como augurio de resistencia a la insulina y de diabetes gestacional. Se requiere profundizar en el estudio de las grasas abdominales dado el conocimiento de su impacto en los desórdenes metabólicos en el curso avanzado de la gestación.
