In the current study we used transthoracic echocardiography to measure stroke volume (SV), heart rate (
) and cardiac output (CO) in adult bottlenose dolphins (
), a male beluga whale calf
, body ...mass (
) range: 151-175 kg and an adult female false killer whale (
, estimated
: 500-550 kg) housed in managed care. We also recorded continuous electrocardiogram (ECG) in the beluga whale, bottlenose dolphin, false killer whale, killer whale (
) and pilot whale (
) to evaluate cardiorespiratory coupling while breathing spontaneously under voluntary control. The results show that cetaceans have a strong respiratory sinus arrythmia (RSA), during which both
and SV vary within the interbreath interval, making average values dependent on the breathing frequency (
). The RSA-corrected
was lower for all cetaceans compared with that of similarly sized terrestrial mammals breathing continuously. As compared with terrestrial mammals, the RSA-corrected SV and CO were either lower or the same for the dolphin and false killer whale, while both were elevated in the beluga whale. When plotting
against
for an inactive mammal, cetaceans had a greater cardiac response to changes in
as compared with terrestrial mammals. We propose that these data indicate an important coupling between respiration and cardiac function that enhances gas exchange, and that this RSA is important to maximize gas exchange during surface intervals, similar to that reported in the elephant seal.
Introducción: El síndrome de Asperger (SA) se enmarca en los Trastornos generalizados del desarrollo. El presente seminario de investigación se centró en la comparación de elementos del discurso ...narrativo referido a habilidades mentalistas y el reconocimiento de estados mentales en fotografías de miradas y si este desempeño se explica por una dificultad en habilidades de Teoría de la mente (TM). Esta relación permitiría un acercamiento a futuros lineamientos terapéuticos, basados en los déficits específicos de estos individuos. Metodología: Se evaluó a 18 sujetos con SA que cursaron NB1 y se comparó con 18 sujetos con desarrollo típico de lenguaje y comunicación (DT), pareados en sexo y edad. Los instrumentos utilizados fueron el libro de imágenes “Frog where are you?”, comparando cantidad y tipo de expresiones mentales, deixis y cantidad de pronombres anafóricos y la prueba “Reading the mind in the eyes”, versión niños. De manera complementaria, se aplicaron dos pruebas para controlar los sesgos: TEVI-R y una encuesta dirigida a los padres acerca del vocabulario mentalista de sus hijos. Resultados: Se encontraron diferencias significativas entre los sujetos con SA y DT en la producción de expresiones mentalistas de voluntad, pronombres anafóricos, deixis temporales y en la cantidad de respuestas correctas en el reconocimiento de estados mentales. Conclusiones: Las dificultades presentadas por los sujetos con SA en la pruebas, responden principalmente a una alteración en las habilidades de la TM. A pesar de lo anterior, se da cuenta de reparos metodológicos que deberían ser tomados en cuenta en investigaciones futuras.
Introduction: Asperger’s Syndrome (SA) is framed within the Pervasive Developmental Disorders. This research seminar focused on the comparison of narrative elements referred to mentalistic skills and mental states recognition in pictures of looks and if this performance is due to a difficulty in skills of Theory of Mind (TM). This relationship would allow an approach to future therapeutic guidelines based on specific deficits of these individuals. Method: 18 subjects with SA who completed NB1 were evaluated. These were compared with 18 subjects with typical development of language and communication (DT), matched for sex and age. The instruments used were the picture book "Frog where are you?", Comparing quantity and type of mental expressions, deixis and anaphoric pronouns and the test "Reading the Mind in the Eyes" children’s version. To complement this, two tests applied to control biases: TEVI-R and a survey addressed to parents about their children's mentalistic vocabulary. Results: There were significant differences between subjects with SA and DT in producing mentalistic expressions of will, anaphoric pronouns, temporal deixis and the number of correct answers in the recognition of mental states. Conclusions: The difficulties presented by subjects with SA in the tests, mainly respond to an impairment in the skills of TM. Despite this, methodological objections are shown which should be taken into account in future research.
La simulación de alta fidelidad ha surgido como un importante método tecnológico de aprendizaje y enseñanza para escenarios clínicos complejos en áreas especializadas, lo que permite enseñar tanto ...habilidades técnicas como no técnicas. La experiencia descrita se relaciona a la inserción curricular de la simulación de alta fidelidad en alumnos de pregrado de cuarto y quinto año (2018-2019) de la Facultad de Medicina de la Universidad Peruana Cayetano Heredia (UPCH) con el objetivo de establecer diagnósticos, solicitar e interpretar exámenes auxiliares, instaurar y evaluar la respuesta a un tratamiento inicial, así como, establecer comunicación efectiva con otros profesionales de la salud, mediante el empleo de escenarios clínicos con simuladores de alta complejidad, haciendo énfasis en el debriefing con buen juicio inmediato, donde se reflexionó sobre los objetivos de aprendizaje establecidos para cada caso, y concluyendo la sesión con los aprendizajes de cada alumno.

Observational study of patients treated with NRT outside of an intensive care or intermediate respiratory care unit setting, diagnosed with COVID-19 and acute respiratory distress syndrome by radiological criteria and SpO

ratio. A multivariate logistic regression model was developed to determine independently associated variables, and the outcomes of high flow nasal cannula and continuous positive airway pressure were compared.
In total, 107 patients were treated and 85 (79.4%) survived at 90 days. Before starting NRT, the mean SpO
/FiO
ratio was 119.8 ± 59.4. A higher SOFA score was significantly associated with mortality (OR 2,09; 95% CI 1.34-3.27), while self-pronation was a protective factor (OR 0.23; 95% CI 0.06-0.91). High flow nasal cannula was used in 63 subjects (58.9%), and continuous positive airway pressure in 41 (38.3%), with no differences between them.
Approximately 4 out of 5 patients treated with NRT survived to 90 days, and no significant differences were found between high flow nasal cannula and continuous positive airway pressure.
Resumen: Introducción: Las terapias respiratorias no invasivas (TRNI) fueron ampliamente utilizadas en la primera ola de la pandemia de COVID-19, en escenarios distintos según los medios disponibles. ...El objetivo fue presentar la supervivencia a 90 días y los factores asociados a esta de los pacientes tratados con TRNI en un centro de tercer nivel sin Unidad de Cuidados Respiratorios Intermedios. Como objetivo secundario comparar los resultados obtenidos de las distintas terapias. Métodos: Estudio observacional de pacientes tratados con TRNI fuera de un ambiente de Cuidados Intensivos o Unidad de Cuidados Respiratorios Intermedios, diagnosticados de COVID-19 y con síndrome de distrés respiratorio agudo por criterios radiológicos y de ratio SpO2/FiO2. Se desarrolló un modelo multivariante de regresión logística para determinar las variables independientemente asociadas, y se compararon los resultados de la terapia de alto flujo con cánula nasal y la presión positiva continua en la vía aérea. Resultados: Se trataron 107 pacientes y sobrevivieron 85 (79,4%) a los 90 días. Antes de iniciar la TRNI el ratio medio de SpO2/FiO2 fue de 119,8 ± 59,4. Un mayor score de SOFA se asoció significativamente a la mortalidad (OR 2,09; IC95% 1,34 – 3,27), mientras que la autopronación fue un factor protector (OR 0,23; IC95% 0,06 – 0,91). Burnout syndrome is a three-dimensional clinical syndrome caused by stress at work. It is frequent in professions which require direct contact with people. In Mexico, the presence of Burnout Syndrome ...in doctors and medical students, is characterized as a threat to their health, quality of life and professional performance.
To evaluate the prevalence of burnout syndrome in students of years 1 through 6 of medical school at a private university in northern Mexico.
Cross-sectional study in the Escuela de Medicina Campus Laguna de la Universidad Autónoma de Durango. The one-dimensional scale of Burnout Student (EUBE) and the Maslach Burnout Inventory (MBI) were applied to the participants. SPSS 19 was used to analyze the data.
Of the 344 students, 255 participated; 153 from years 1 to 3 (group 1); and 72 from years 4 to 6 (group 2). We found that 94.1% of the students of group 1 had mild burnout syndrome, and 2.8% had moderate burnout syndrome. In Group 2, 27.8% had moderate burnout syndrome, and 8.3% had severe burnout syndrome. The prevalence of severe burnout syndrome was higher in group 2 than in group 1 (p=0.02).
Burnout syndrome affects medical students across all stages of their studies, and develops in a progressive way. In our study, external factors have no influence on the development of burnout syndrome.
El propósito fundamental de este estudio consistió en fortalecer la competencia ortográfica de estudiantes de quinto grado pertenecientes al colegio Salvador de Entregas, a través de la ...implementación de estrategias lúdicas. Esta investigación, caracterizada por su naturaleza de acción, se desarrolló empleando un diseño de acción participativo. La muestra estuvo conformada por un docente y treinta alumnos de quinto grado de educación básica primaria durante el año lectivo 2018. Para llevar a cabo la recopilación de información pertinente, se optó por utilizar la encuesta y el test como técnicas, utilizando como instrumentos específicos el cuestionario y la prueba diagnóstica respectivamente. Los resultados obtenidos revelaron notables deficiencias en el dominio de aspectos fundamentales de la ortografía por parte de los estudiantes, particularmente en áreas vinculadas con la acentuación, los diptongos y los hiatos. Asimismo, se identificó el uso recurrente por parte del docente de métodos de enseñanza tradicionales, lo cual incidió en estos hallazgos. En virtud de estos resultados, se justificó plenamente la necesidad de diseñar una estrategia pedagógica que se apoyara en actividades lúdicas. Como resultado de la implementación de la propuesta educativa denominada "Aprender ortografía jugando", se constató un impacto positivo significativo en las competencias relacionadas con la escritura, la redacción, la pronunciación de vocablos y la interpretación de textos entre los estudiantes evaluados.
