Long-term adefovir administration has been reported overseas to induce renal impairment in a certain percentage of patients. In Japan, its influences on renal function are unclear. In patients ...continuously treated with nucleotide analogues, we compared changes in the serum creatinine level and estimated glomerular filtration rate among patients in the control and treated and not treated with adefovir. The group treated with adefovir showed a significant increase in the serum creatinine level and a significant decrease in the estimated glomerular filtration rate compared with the group not receiving adefovir (p<0.001). There were no significant differences in these changes between the control and the group not receiving adefovir. Logistic regression analysis revealed adefovir administration to be a significant factor (odds ratio, 6.6), inducing a ≥20% decrease in the estimated glomerular filtration rate. In patients in whom daily was changed to alternate-day adefovir administration due to renal impairment, the serum creatinine level gradually decreased, suggesting that renal impairment is reversible.
