The paper substantiates the development of a simulation model in the design of the Silk Road Museum. A brief description of the current world museums and exhibitions on the territory of the Russian ...Federation dedicated to its legacy has been made. A description of the methods used to establish the system of key objects and their corresponding qualitative and quantitative characteristics has been presented. Such information is the input parameters and rules of the simulation model, enabling to implement scenarios for the behavior of all participants in various processes in order to create regulations for the organization and management of the museum.
Fibrodysplasia ossificans progressive (FOP) is an ultra-rare genetic disorder that is caused by a mutation in the ACVR1 gene and provokes severe heterotopic ossification. Since flares of the disease ...are associated with inflammation, it is assumed that JAK inhibitors can control active FOP due to blocking multiple signaling pathways. Keywords: Fibrodysplasia ossificans progressiva, FOP, Heterotopic ossification, Tofacitinib, Autoinflammation, Spondyloarthritis-like disease, ACVR1 gene, Bone morphogenetic protein, BMP
The article defines the problem of environmental condition of a road segment and the reasons for such condition. Key factors have been defined having a negative effect on the environment. One of the ...factors is organization of pedestrian traffic in the transport system of the Koptevo district in the Northern Administrative District of Moscow. The research target boundaries have been defined, which are limited by Mikhalkovskaya Street and adjacent area. The research target has been structurally analyzed to identify all key objects and their characteristics affecting the pedestrian flows. Graphical and parametric models of the research target have been developed, which are a basis for modeling scenarios of changes in various characteristics of a real object. Such changes are necessary to develop the concept of sustainable development of the area.
This article presents a plan for the development of the area adjacent to Pryanishnikov Street in the Timiryazevsky District of the Northern Administrative District of Moscow. This plan involves ...upgrading the roadway to distribute traffic flows and isolate the public transport flow. Simulation models are created, reflecting all the objects and rules of interaction that are typical for the studied area to assess the effectiveness of the developed concept. The main feature of the site development plan is the safety of all road users, reducing the downtime of personal road and public transport to increase the capacity of the road network and improve the environmental condition of the area. The created scheme and traffic models are the basis for the urban city development plan and can be used to simulate different scenarios of territory changes and operational management of traffic and pedestrian flows.
Due to the complexity of the biomedical domain, the ability to capture semantically meaningful representations of terms in context is a long-standing challenge. Despite important progress in the past ...years, no evaluation benchmark has been developed to evaluate how well language models represent biomedical concepts according to their corresponding context. Inspired by the Word-in-Context (WiC) benchmark, in which word sense disambiguation is reformulated as a binary classification task, we propose a novel dataset, BioWiC, to evaluate the ability of language models to encode biomedical terms in context. BioWiC comprises 20'156 instances, covering over 7'400 unique biomedical terms, making it the largest WiC dataset in the biomedical domain. We evaluate BioWiC both intrinsically and extrinsically and show that it could be used as a reliable benchmark for evaluating context-dependent embeddings in biomedical corpora. In addition, we conduct several experiments using a variety of discriminative and generative large language models to establish robust baselines that can serve as a foundation for future research.
The results of formal model design of a major intersection area essential for providing residents with transport accessibility and mobility are presented in the article. Simulation methods were ...applied to develop a model. The quantitative and qualitative characteristics crucial for the functioning of the model as its input parameters were determined. The importance of using a model when modelling different behavior scenarios of model objects based on typical situations of the real world is established.
Remission is the primary objective of treating juvenile idiopathic arthritis (JIA). It is still debatable whether early intensive treatment is superior in terms of earlier achievement of remission. ...The aim of this study was to evaluate the effectiveness of early etanercept+methotrexate (ETA+MTX) combination therapy versus step-up MTX monotherapy with ETA added in refractory disease.
A multi-centre, double-blind, randomized study in active polyarticular JIA patients treated with either ETA+MTX (n = 35) or placebo+MTX (n = 33) for up to 24 weeks, followed by a 24-week open-label phase. The efficacy endpoints included pedACR30 criteria improvement at week 12, inactive disease at week 24, and remission at week 48. Patients who failed to achieve the endpoints at week 12 or at week 24 escaped to open-label ETA+MTX. Safety was assessed at each visit.
By intention-to-treat analysis, more patients in the ETA+MTX group reached the pedACR30 response at week 12 (33 (94.3%)) than in the placebo+MTX group (20 (60.6%); p = 0.001). At week 24, comparable percentages of patients reached inactive disease (11 (31.4%) vs 11 (33.3%)). At week 48, 11 (31.4%) and eight (24.2%) patients achieved remission. The median (+/-IQR) times to achieve an inactive disease state in the ETA+MTX and placebo+MTX groups were 24 (14-32) and 32 (24-40) weeks, respectively. Forty-four (74/100 patient-years) adverse events (AEs) were reported, leading to treatment discontinuation in 6 patients.
Early combination therapy with ETA+MTX proved to be highly effective compared to the standard step-up regimen. Compared to those treated with the standard regimen, more patients treated with a combination of ETA+MTX reached the pedACR30 response and achieved inactive disease and remission more rapidly.
Background.
Systemic onset juvenile arthritis is a chronic disease of childhood. Its severity is determined by systemic manifestations, high risk of complications, such as macrophage activation ...syndrome, interstitial lung disease, and persistent polyarthritis with severe functional disorders. Genetically engineered biological drugs administration significantly improves the prognosis in these patients, thus, there are many questions about the possibilities of successful treatment correction in order to maintain long-term remission in real clinical practice.
Clinical case description
. The results of long-term (9 years) administration of tocilizumab in female patient with early onset of systemic onset juvenile arthritis are presented. It has been shown that control over disease activity can be achieved via adjusting the drug dosage and the intervals between infusions.
Conclusion.
Tocilizumab significantly improves prognosis of patients with systemic onset juvenile arthritis at good safety profile of this treatment.
Systemic onset juvenile arthritis is the complex autoinflammatory disease with multigenic type of inheritance. It is characterized by significant systemic manifestations and unfavourable adverse ...events, among which macrophage activation syndrome and pulmonary involvement are playing crucial role. This article presents data on the frequency and features of the disease course, possible development predictors and clinical-radiological picture of pulmonary involvement in systemic onset juvenile arthritis. Possible factors (including hereditary) affecting the process of pulmonary involvement development and the role of genetically engineered biologic therapy are considered.
