Forests are increasingly recognized for their beneficial roles in human health. However, there is a debate on how forest health benefits can be accessed equitably, particularly by vulnerable ...forest-dependent rural communities in low- and middle-income countries (LMICs). Access to forest health benefits is determined by a range of interconnected means, including property rights, as well as natural, physical, human, social, and financial capital. This paper presents a literature review of the roles of means of access in shaping human health effects of forests. Evidence suggests that variations in these means of access are associated with varying ability to access forest health benefits. However, existing evidence is thin, mixed, and weak. A conceptual model is then developed to provide a framework for understanding how means of access moderate the effects of forests on health in rural LMICs to guide the generation of strong evidence. The multiple interconnected factors moderating the health effects of forests at the core of the conceptual framework promote the multisectoral and transdisciplinary approaches needed to enhance equitable access to forest health benefits.
Drowning is an overlooked public health concern and drowning risk is dependent on environmental risk factors. The preponderance of drowning deaths occurs in low- and middle-income countries. ...Small-scale fishers face high occupational risk of drowning. Climate change increases the frequency and intensity of storms, thereby exacerbating fishers' risks and creating a need to examine the contribution of storms to fisher drowning deaths for the development of mitigation strategies. We examined this relationship between weather and fisher drowning deaths in Lake Victoria, which is Africa's largest lake, a site of high fishing pressure, and where climate change is predicted to increase thunderstorms. We conducted a verbal autopsy with people knowledgeable about recent fatal fisher drowning incidents to collect information about the deceased fishers and circumstances surrounding the incidents across 43 landing sites in the Kenyan shore of Lake Victoria. Semi-structured interviews with stakeholders also elucidated community perspectives on drowning risks. Fatal drownings were often attributed to bad weather (41.8%). Other risk factors, such as non-use of life jacket and navigation equipment, co-occurred with bad weather at high rates (69.5% and 67.8%, respectively) to jointly contribute to fatal drowning incidents. Such co-occurrence of risk factors indicates that actions across multiple risk factors can help mitigate the issue. Stakeholder analysis revealed a range of opportunities for improved communication of risks and action to mitigate risks across boat operators and manufacturers, as well as multiple levels of management. Across global small-scale fisheries, limited use of safety equipment and intensive fishing pressure may coincide with increases in extreme weather events, necessitating action to address current and mitigate future drowning risks to small-scale fishers.
Understanding the effectiveness of conservation interventions during times of political instability is important given how much of the world's biodiversity is concentrated in politically fragile ...nations. Here, we investigate the effect of a political crisis on the relative performance of community managed forests versus protected areas in terms of reducing deforestation in Madagascar, a biodiversity hotspot. We use remotely sensed data and statistical matching within an event study design to isolate the effect of the crisis and post-crisis period on performance. Annual rates of deforestation accelerated at the end of the crisis and were higher in community forests than in protected areas. After controlling for differences in location and other confounding variables, we find no difference in performance during the crisis, but community-managed forests performed worse in post-crisis years. These findings suggest that, as a political crisis subsides and deforestation pressures intensify, community-based conservation may be less resilient than state protection.

Community Forest Management (CFM) is a widespread conservation approach in the tropics. It is also promoted as a means by which payment for ecosystem services schemes can be implemented. However, evidence on its performance is weak. We investigated the effectiveness of CFM at reducing deforestation from 2000 to 2010 in Madagascar. To control for factors confounding impact estimates, we used statistical matching. We also contrasted the effects of CFM by whether commercial use of forest resources is allowed or not. We cannot detect an effect, on average, of CFM compared to no CFM, even when we restricted the sample to only where information suggests effective CFM implementation on the ground. Likewise, we cannot detect an effect of CFM where commercial use of natural resources is allowed. However, we can detect a reduction in deforestation in CFM that does not permit commercial uses, compared to no CFM or CFM allowing commercial uses. Our findings suggest that CFM and commercial use of forest resources are not guarantees of forest conservation and that differentiating among types of CFM is important.
Abstract
Environmental degradation has been associated with increased burden of diseases such as malaria, diarrhea, and malnutrition. As a result, some have argued that continuing ecosystem change ...could undermine successes in global health investments. Here we conduct an empirical study to investigate this concern. Child deaths due to diarrhea have more than halved since the year 2000, partly due to increased access to improved water, sanitation and hygiene (WASH). We examine how the effectiveness of a water quality treatment may vary as a function of upstream watershed condition. We use data on occurrence of diarrhea and point-of-use water treatment methods from the Demographic and Health Surveys for Haiti and Honduras. We integrate these data with a variable that reflects the influence of upstream tree cover on surface water quality. Point-of-use chlorination is significantly associated with 3.4 percentage points reduction in prevalence of diarrheal disease on average. However, we only detect a significant reduction in diarrheal prevalence when upstream watersheds are moderately forested. At low upstream tree coverage, point-of-use water chlorination does not have significant effects, suggesting that forest clearing could undermine its effectiveness at reducing childhood diarrhea. Our results suggest that forested watersheds may reduce water-borne sediments and contaminants, and thus improve raw water quality in ways that moderate the effectiveness of water quality treatment. Watershed protection should be considered in WASH investments, as deforested watersheds could undermine their effectiveness, particularly in parts of low-income countries where access to improved WASH services is challenging.
Community Forest Management (CFM) devolves forest management to local communities to achieve conservation and human well‐being goals. Yet, the evidence for CFM's impacts is mixed and difficult to ...interpret because of inadequate attention to rival explanations for the observed empirical patterns. In a national‐scale analysis in Madagascar that carefully considers these rival explanations, we estimate CFM impacts on household living standards, as measured by per capita consumption expenditures. The estimated impact is positive, but small and not statistically different from zero. However, we can statistically reject substantial negative impacts (which others have suggested may exist). The estimated impacts vary conditional on household education and proximity to forests: they are more positive and statistically significant for households closer to forest and with more education. To help improve CFM design, scholars and practitioners should anticipate heterogeneity in CFM impacts and work to better characterize them, theoretically and empirically.
Micronutrient deficiency affects about a third of the world's population. Children in developing countries are particularly vulnerable. Consequences include impaired cognitive and physical ...development and increased childhood morbidity and mortality. Recent studies suggest that forests help alleviate micronutrient deficiency by increasing dietary diversity. However, evidence is mostly based on weakly designed local case studies of limited relevance to global policies. Furthermore, impacts of forests on diet vary among communities, and understanding this variation can help target actions to enhance impact. We compile data on children's diets in over 43,000 households across 27 developing countries to examine the impacts of forests on dietary diversity. We use empirical designs that are attentive to assumptions necessary for causal interpretations and that adequately account for confounding factors that could mask or mimic the impact. We find that high exposure to forests causes children to have at least 25% greater dietary diversity compared to lack of exposure, a result comparable to the impacts of some nutrition-sensitive agricultural programs. A closer look at a subset of African countries indicates that impacts are generally higher for less developed communities, but highest with certain access to markets, roads, and education. Our results also indicate that forests could help reduce vitamin A and iron deficiencies. Our study establishes the causal relationship between forests and diet and thus strengthens the evidence for integrating forest conservation and management into nutrition interventions. Our results also suggest that providing households some access to capital can increase the impact of forest-related interventions on nutrition.

There is growing interest in the importance of ensuring that biodiversity conservation is not achieved at the expense of local people’s well-being. It has been suggested that when evaluating the impact of an intervention, the affected population should be allowed to define well-being (requiring a subjective measure), and impacts (requiring a participatory approach), but very few, if any, conservation evaluations live up to these standards. We used a participatory impact evaluation approach with the Global Person Generated Index (GPGI) to investigate the relative impacts of strict protection and community forest management on local well-being in Madagascar’s rainforests. The GPGI captures the subjective and multidimensional nature of well-being by asking respondents to identify the five most important domains for their quality of life, to evaluate their own performance in each domain, and the relative importance of the five identified domains. Participatory impact evaluation establishes local perceptions of the cause-effect relationship between an intervention and respondents’ performance in each domain. Over half the respondents perceived no positive or negative impacts from the conservation interventions. We found no significant difference between strict protection and community forest management in the measures we used to examine the magnitude of their relative impacts, but there were differences in the characteristics of domains impacted and in the priority domains that could be targeted to improve well-being in locally meaningful ways. Because of its subjectivity, the GPGI cannot provide quantitative information on the magnitude of impacts. Its strength lies in the wealth of information it provides on what life domains people value and their performance in these domains. Combined with the participatory impact evaluation approach, the GPGI provides highly relevant insights that can be used to improve interventions in ways which increase the local legitimacy and acceptability of conservation initiatives.
Childhood undernutrition yearly kills 3.1 million children worldwide. For those who survive early life undernutrition, it can cause motor and cognitive development problems that translate into poor ...educational performance and limited work productivity later in life. It has been suggested that nutrition-specific interventions (e.g., micronutrient supplementation) that directly address the immediate determinants of undernutrition (e.g., nutrient intake) need to be complemented by nutrition-sensitive interventions that more broadly address the underlying determinants of undernutrition (e.g., food insecurity). Here, we argue that forest conservation represents a potentially important but overlooked nutrition-sensitive intervention. Forests can address a number of underlying determinants of undernutrition, including the supply of forest food products, income, habitat for pollinators, women's time allocation, diarrheal disease, and dietary diversity. We examine the effects of forests on stunting—a debilitating outcome of undernutrition—using a database of household surveys and environmental variables across 25 low- and middle-income countries. Our result indicates that exposure to forest significantly reduces child stunting (at least 7.11% points average reduction). The average magnitude of the reduction is at least near the median of the impacts of other known nutrition interventions. Forest conservation interventions typically cover large areas and are often implemented where people are vulnerable, and thus could be used to reach a large number of the world's undernourished communities that may have difficult access to traditional nutrition programs. Forest conservation is therefore a potentially effective nutrition-sensitive intervention. Efforts are needed to integrate specific nutrition goals and actions into forest conservation interventions in order to unleash their potential to deliver nutritional benefits.
