NT-proBNP is emerging as a novel tool for improving management of patients with heart failure (HF). The concept of health-related outcomes as the primary endpoint for therapeutic intervention in ...chronic disease, such as HF, should be the focal point going forward.
We conducted a prospective real-world study in heart failure with reduced ejection fraction (HFrEF) patients. The main target was to evaluate the impact on patient's health-related outcomes of a personalized medical follow-up procedure, based on a laboratory model of risk stratification supported by NT-proBNP. One hundred and five consecutive patients admitted to the Hospital Heart-Failure unit were stratified into three groups (low, medium, and high risk) and prospective follow-ups during the 12 months post discharge.
It was found that patients under this new approach experienced early and robust improvements in patient health-related outcomes with consistency in most domains which persisted beyond 12 months post follow-up. Improvements in health related quality of life score (HRQLS) was observed over the time of the study. After 6 months we found a significant improvement in HRQLS of 18.2% (from 76.5 ± 22.4 to 95.0 ± 15.7) and 14.4% (from 76.5 ± 22.4 to 96.3 ± 15.9) after 12 months of follow-up (p < 0.001). The highest improvements were found in the symptom severity domain where patients reported an improvement of 22.6% after 6 months and 18.9% after 12 months (p < 0.001). The lowest scores were reported in the physical domain with increase of 11.0% and 4.3% after 6 months and 12 months (p = 0.089). Psychosocial domain and the ability to carry out the activities of normal life showed improvement as well.
Our strategy based on NT-proBNP optimizes HFrEF management and represents a major new approach for clinical laboratories to improve patient health-related outcomes in HFrEF.
Heart failure (HF) is a major and growing medical and economic problem, with high prevalence and incidence rates worldwide. Cardiac Biomarker is emerging as a novel tool for improving management of ...patients with HF with a reduced left ventricular ejection fraction (HFrEF).
This is a before and after interventional study, that assesses the impact of a personalized follow-up procedure for HF on patient's outcomes and care associated cost, based on a clinical model of risk stratification and personalized management according to that risk. A total of 192 patients were enrolled and studied before the intervention and again after the intervention. The primary objective was the rate of readmissions, due to a HF. Secondary outcome compared the rate of ED visits and quality of life improvement assessed by the number of patients who had reduced NYHA score. A cost-analysis was also performed on these data.
Admission rates significantly decreased by 19.8% after the intervention (from 30.2 to 10.4), the total hospital admissions were reduced by 32 (from 78 to 46) and the total length of stay was reduced by 7 days (from 15 to 9 days). The rate of ED visits was reduced by 44% (from 64 to 20). Thirty-one percent of patients had an improved functional class score after the intervention, whereas only 7.8% got worse. The overall cost saving associated with the intervention was € 72,769 per patient (from € 201,189 to € 128,420) and €139,717.65 for the whole group over 1 year.
A personalized follow-up of HF patients led to important outcome benefits and resulted in cost savings, mainly due to the reduction of patient hospitalization readmissions and a significant reduction of care-associated costs, suggesting that greater attention should be given to this high-risk cohort to minimize the risk of hospitalization readmissions.

Carotenoids in human milk are associated with other lipid counterparts in several metabolic processes. One interesting association that has not been demonstrated to date is the presence of ...xanthophyll esters. Colostrum and mature milk samples were analyzed to determine the occurrence of xanthophyll esters and identify the compounds. Thus, the association of the amounts of these compounds with lactation and whether they are significant contributors to the carotenoid profile of human milk was assessed.
Methods and results
Pre‐term and term delivering mothers were included in the study to donate colostrum at 3–5 days postpartum and mature milk at 15 days postpartum. Carotenoids extracts were subjected to a clean‐up procedure to remove the triacylglycerol fraction and then analyzed by HPLC‐MSn. Identification of xanthophyll esters was achieved by considering their chromatographic behaviour, UV‐visible characteristics and MSn features.
Conclusion
Xanthophyll esters are significant contributors to the carotenoid profile in the colostrum, while mature milk does not contain these compounds. Therefore, fatty acid acylation to xanthophylls is activated during the accumulation of carotenoids in the human mammary gland. The sharp decline in the amount of xanthophyll esters in mature milk indicates that the lipophilic components are those recently incorporated in the mammary epithelium.
The human colostrum contains significant amounts of xanthophyll esters accumulated in mammary gland. Mass spectrometry has allowed the identification of the main contributors to the xanthophyll esters fraction.
Respiratory tract infections remain among the leading causes of mortality worldwide. The COVID-19 pandemic has highlighted the importance of mucosal immunity in defending against infectious agents. ...Vitamin A is known to influence the production of secretory immunoglobulin A (SIgA) predominantly in the gut, where it is a critical component of the first line of defense on mucosal surfaces.
This cross-sectional study, conducted 14 days post-positive COVID-19 diagnosis, aimed to determine the relationship between the nutritional status of vitamin A and SIgA levels in COVID-19 outpatients. Serum and saliva samples were collected. Vitamin A nutritional status was determined based on the assessment of dietary intake and the analysis of retinol-binding protein 4 (RBP4). SIgA levels were analyzed from salivary samples. In addition, serum antibodies were analyzed.
Dietary vitamin A intake and RBP4 levels positively correlated with SIgA. Patients with higher vitamin A intake showed higher SIgA/IgG1 and SIgA/IgG3 ratios, while those with higher RBP4 levels showed higher SIgA/IgM, SIgA/IgG1, and SIgA/IgG2 ratios.
These findings underscore a significant correlation between vitamin A nutritional status and SIgA levels in COVID-19 outpatients, which may suggest the potential importance of maintaining optimal vitamin A levels for the prevention of viral infections.

Resultados: Se explica con detalle el proyecto, su diseño, su desarrollo durante las primeras semanas, además de evaluar sus resultados. Se ofrecen datos estadísticos y se proponen acciones de mejora para cursos sucesivos
Conclusiones: la práctica ha sido una experiencia muy enriquecedora para los alumnos, han explicado su sistema de trabajo y el potencial de las RR.SS. para la docencia, además de contar sus vivencias y su adquisición de distintas habilidades blandas que los aproxima al mundo real y laboral.
The Andean Condor (Vultur gryphus) is a culturally iconic wildlife symbol for the South American Andes, but is naturally found at very low population densities, and is increasingly threatened. Using ...the Range Wide Priority Setting methodology, we (a group of 38 Andean Condor experts) updated the Andean Condor historical range (3,230,061 km2), systematized 9998 Andean Condor distribution points across the range, and identified geographic areas for which there was expert knowledge (66%), including areas where Andean Condors no longer occur (7%), and geographic areas where condors are believed to range, but for which there was not expert knowledge about condor presence (34%). To prioritize conservation action into the future and identify existing Andean Condor population strongholds, we used expert knowledge to identify 21 of the most important areas for the conservation of the species (i.e., Andean Condor Conservation Units ACCUs) that cover 37% of the revised historical range, and range in size from 837 km2 to 298,951 km2. In general, ACCUs were relatively small in the northern portion of the range in Venezuela, Colombia, Ecuador, and northern Peru, and significantly larger in the central and southern portion of the range in Peru, Bolivia, Chile, and Argentina, reflecting the reduced and narrower historical range in the northern portion of the range, as well as increased threats. Andean Condors can fly extremely long distances and so the populations of many neighboring ACCUs are probably still functionally connected, although this situation also underlines the need for integrated and large-scale conservation efforts for this species. As a function of the Range Wide Priority Setting results, we make recommendations to ensure population connectivity into the future and engage a wide range of actors in Andean Condor conservation efforts.
