Background
Vocal cord injuries, postoperative hoarseness, and sore throat are common complications after general anesthesia. One-lung ventilation can be achieved via two techniques: double-lumen ...endotracheal tube or endobronchial blocker such as the Arndt blocker. The current study was designed to assess the impact of these techniques for one-lung ventilation on the incidence and severity of postoperative hoarseness, vocal cord lesions, and sore throat.
Methods
In this prospective trial, 60 patients were randomly assigned to two groups. One-lung ventilation was achieved with either an endobronchial blocker (blocker group) or a double-lumen-tube (double-lumen group). Postoperative hoarseness and sore throat were assessed at 24, 48, and 72 h after surgery. Bronchial injuries and vocal cord lesions were examined by bronchoscopy immediately after surgery.
Results
In 56 included patients, postoperative hoarseness occurred significantly more frequently in the double-lumen group compared with the blocker group: 44% versus 17%, respectively (P = 0.046). Similar findings were observed for vocal cord lesions: 44% versus 17%, respectively (P = 0.046). The incidence of bronchial injuries was comparable between groups (P = 0.540). Cumulative number of days with hoarseness and sore throat were significantly increased in the double-lumen group compared with the blocker group (P < 0.01). No major complications such as bronchial ruptures were observed.
Conclusions
Clinicians should be aware of an increased incidence of minor airway injuries that may impair patient satisfaction when using a double-lumen tube instead of an endobronchial blocker for one-lung ventilation.

Die stetig wachsende Bedeutung von Kulturpflanzen zur Ernährung und zur industriellen Verwendung erfordert es, deren Eigenschaften kontinuierlich zu verbessern. Zur Unterstützung der ...lebenswissenschaftlichen Forschung in diesem Bereich wurde das Informationssystem MetaCrop entwickelt, welches Daten über metabolische Netzwerke (Pathways) agronomisch bedeutsamer Organismen verwaltet. Der Fokus des Systems liegt neben der ausschließlichen Verwendung manuell kurierter, und damit qualitativ hochwertiger, Informationen auf der hohen Granularität der gespeicherten Daten, mit der versucht wird, die hohe biologische Komplexität der Pflanzen abzubilden. Zusammenfassung
Continuously improving crop performance is a crucial necessity due to the growing importance of crop plants as feedstock for both nutrition and industrial use. The information system MetaCrop has been developed to support life science research in this important field. MetaCrop stores data about metabolic pathways of agronomically important crops. The system focusses on the exclusive usage of manually curated and therefore high quality data as well as on a high granularity of the managed information, thus aiming to represent the high biological complexity of plants. In this paper we introduce AIT’s Smart Sensor Technology platform, developed within the FOLDOUT project (Through-foliage detection, including in the outermost regions of the EU). The novel platform ...is part of FOLDOUT’s solution for ground based border surveillance, and in particular detection of foliage penetration. The platform combines a high resolution (4K) RGB camera, a thermal camera, as well as a 30x zoom (2K) NIR low-light camera. It is designed to work day and night under strongly varying weather conditions, it is transportable and can be deployed in remote areas, as well as being self-sufficient if operated by battery. The typical sensor range is about 100-200m which can be adapted per specific usage. The platforming thobe used for surveillance of dedicated areas or by providing an open interface to be integrated into whole surveillance systems in which ground-based sensors are combined with high altitude sensors to provide both close and wide area coverages. The platform provides state-of-the-art AI-based detection algorithms for object detection and classification, for both RGB and thermal images. The individual detections are further fused by establishing coincidence in time and space between the individual detectors, where sensor is geo-localized for accurate target localization and visualization on a map of the area to be monitored.

In this article, we propose the Coopetititve Soft Gating Ensemble or CSGE for general machine learning tasks and interwoven systems.The goal of machine learning is to create models that generalize ...well for unknown datasets. Often, however, the problems are too complex to be solved with a single model, so several models are combined. Similar, Autonomic Computing requires the integration of different systems. Here, especially, the local, temporal online evaluation and the resulting (re-)weighting scheme of the CSGE makes the approach highly applicable for self-improving system integrations. To achieve the best potential performance the CSGE can be optimized according to arbitrary loss functions making it accessible for a broader range of problems. We introduce a novel training procedure including a hyper-parameter initialisation at its heart. We show that the CSGE approach reaches state-of-the-art performance for both classification and regression tasks. Further on, the CSGE provides a human-readable quantification on the influence of all base estimators employing the three weighting aspects. Moreover, we provide a scikit-learn compatible implementation.
Die Borreliose ist eine von Zecken übertragene entzündliche Erkrankung, verursacht durch die Spirochäte Borrelia burgdorferi. Im Rahmen dieser können drei klinische Stadien durchlaufen werden, wobei ...im Stadium 2 und 3 neurologische Symptome möglich sind.
Vorgestellt wird eine 78jährige Frau, die aufgrund eines zunehmenden dementiellen Syndroms zur stationären Aufnahme kam. In der Liquoruntersuchung zeigte sich neben einem erhöhten Gesamteiweißgehalt von 898mg/l eine Pleozytose von 132 Mpt/l mit lymphozytärem Zellbild (60%) sowie eine intrathekale IgG-Synthese und mehr als 10 oligoklonale IgG-Banden. Der Borrelien-IgG-Antikörperindex lag bei 12,9. In Zusammenschau aller Befunde ließ sich das Vorliegen einer Neuroborreliose bestätigen.
Die neuropsychologische Testung nach antibiotischer Behandlung dokumentierte Verbesserungen im Kurzzeitgedächtnis für numerisches und visuelles Material, verbal und logisches Langzeitgedächtnis und den visuell räumlichen Fähigkeiten.
Wie der vorgestellte Fall zeigt, ist die Liquordiagnostik in der Differentialdiagnostik der Demenz unerlässlich, um eine zugrundeliegende Neuroborreliose aufzudecken. Eine adäquate antibiotische Therapie kann in diesen Fällen zu einer deutlichen Besserung der kognitiv-mnestischen Einschränkungen führen. Mögliche pathogenetische Mechanismen kognitiver Störungen bei Neuroborreliose werden diskutiert.
The Photo Injector Test Facility at DESY Zeuthen (PITZ) was built to test and optimize electron sources for Free Electron Lasers and future linear colliders. The focus is on the production of intense ...electron beams with minimum transverse emittance and short bunch length as required for FEL operation. The experimental setup includes a 1.5 cell L-band gun cavity with coaxial RF coupler, a solenoid for space charge compensation, a laser capable to generate long pulse trains with variable temporal and spatial pulse shape, an UHV photo cathode exchange system, and an extensive diagnostics section. This contribution will give an overview on the facility and will mainly discuss the measurements of the electron beam transverse phase space. This will include measurements of the transverse and longitudinal laser profile, beam charge as a function of RF phase, and transverse emittance as a function of different parameters. The corresponding measurements of momentum and momentum spread as well as the RF commissioning results will be summarized. As a first application of the PITZ electron source it will be installed at the TESLA Test Facility Free Electron Laser at DESY Hamburg in autumn 2003.
With growing complexity and responsibility of automated driving functions in road traffic and growing scope of their operational design domains, there is increasing demand for covering significant ...parts of development, validation, and verification via virtual environments and simulation models. If, however, simulations are meant not only to augment real-world experiments, but to replace them, quantitative approaches are required that measure to what degree and under which preconditions simulation models adequately represent reality, and thus allow their usage for virtual testing of driving functions. Especially in research and development areas related to the safety impacts of the "open world", there is a significant shortage of real-world data to parametrize and/or validate simulations - especially with respect to the behavior of human traffic participants, whom automated vehicles will meet in mixed traffic. This paper presents the intermediate results of the German AVEAS research project (www.aveas.org) which aims at developing methods and metrics for the harmonized, systematic, and scalable acquisition of real-world data for virtual verification and validation of advanced driver assistance systems and automated driving, and establishing an online database following the FAIR principles.
Atovaquone-proguanil has recently been introduced for the treatment and prophylaxis of malaria. However, resistance of Plasmodium falciparum is increasingly reported. We assessed P. falciparum ...polymorphisms associated with resistance to atovaquone (cytochrome b, cytb) and to cycloguanil, the active compound of proguanil (dihydrofolate reductase, dhfr) in 100 isolates from northern Ghana. None of these exhibited cytb codon 268 mutations. Moreover, no dhfr V16A, S108T or I164L mutations linked with cycloguanil resistance were detected. However, dhfr triple mutants (S108N-I51L-C59R) conferring resistance to proguanil and sulphadoxine-pyrimethamine were seen in 51% of the isolates. In northern Ghana, P. falciparum cytb codon 268 mutations associated with atovaquone resistance are absent. Although proguanil appears to act synergistically with atovaquone in a way different from its antifolate property, the abundance of dhfr polymorphisms will likely compromise the prevention of dissemination of atovaquone-resistant parasites once emerged.
