Natural compounds containing fungal β-glucans have been used to improve general health for thousands of years in China and Japan. Lentinan, the backbone of β-(1, 3)-glucan with β-(1, 6) branches, is ...one of the active ingredients purified from Shiitake mushrooms and has been approved as a biological response modifier for the treatment of gastric cancer in Japan. Despite recent advances in chemotherapeutic agents, unresectable or recurrent gastric cancer remains an incurable disease, with survival rates being far from satisfactory. Recent clinical studies have shown that chemo-immunotherapy using lentinan prolongs the survival of patients with advanced gastric cancer, as compared to chemotherapy alone. In addition, trastuzumab, an antibody against HER2/neu growth factor receptor, has been used for the treatment of gastric cancer in combination with cytotoxic chemotherapeutic agents. Lentinan may exert a synergistic action with anti-cancer monoclonal antibodies to activate complement systems through the mechanism of antibody-dependent cellular cytotoxicity and complement dependent cytotoxicity. Because a better understanding of its biological activities should enable us to use lentinan more efficiently in the treatment of gastric cancer, immunological effects provided by β-glucans, a possible mode of action of lentinan, and its clinical application including future potential uses are discussed in the present review.
The ability to switch fuels for oxidation in response to changes in macronutrient composition of diet (metabolic flexibility) may be informative of individuals' susceptibility to weight gain. ...Seventy-nine healthy, weight-stable participants underwent 24-h assessments of energy expenditure and respiratory quotient (RQ) in a whole-room calorimeter during energy balance (EBL) (50% carbohydrate, 30% fat) and then during 24-h fasting and three 200% overfeeding diets in a crossover design. Metabolic flexibility was defined as the change in 24-h RQ from EBL during fasting and standard overfeeding (STOF) (50% carbohydrate, 30% fat), high-fat overfeeding (HFOF) (60% fat, 20% carbohydrate), and high-carbohydrate overfeeding (HCOF) (75% carbohydrate, 5% fat) diets. Free-living weight change was assessed after 6 and 12 months. Compared with EBL, RQ decreased on average by 9% during fasting and by 4% during HFOF but increased by 4% during STOF and by 8% during HCOF. A smaller decrease in RQ, reflecting a smaller increase in lipid oxidation rate, during HFOF but not during the other diets predicted greater weight gain at both 6 and 12 months. An impaired metabolic flexibility to acute HFOF can identify individuals prone to weight gain, indicating that an individual's capacity to oxidize dietary fat is a metabolic determinant of weight change.
Many older adults have difficulty seeing brief visual stimuli which younger adults can easily recognize. The primary visual cortex (V1) may induce this difficulty. However, in neuroimaging studies, ...the V1 response change to the increase of temporal frequency of visual stimulus in older adults was unclear. Here we investigated the association between the temporal frequency of flickering stimuli and the BOLD activity within V1 in older adults, using surface-based fMRI analysis. The fMRI data from 29 healthy older participants stimulated by contrast-reversing checkerboard at temporal flicker frequencies of 2, 4, and 8 Hz were obtained. The participants also performed a useful field of view (UFOV) test. The slope coefficient of BOLD activity regarding the temporal frequency of the visual stimulus averaged within V1 regions of interest was positive and significantly different from zero. Group analysis in the V1 showed significant clusters with positive slope and no significant clusters with a negative slope. The correlation coefficient between the slope coefficient and UFOV performance was not significant. The results indicated that V1 BOLD response to a flickering visual stimulus increases as the stimulus temporal frequency increases from 2 to 8 Hz in older adults.
Motorized transport access and out-of-home activities are two potential correlates of Life-space mobility (LSM), a common research topic in mobility studies of older adults. These correlates remain ...mostly unexplored in previous literature and relating them with LSM can reveal directions for improving the LSM of older adults.
The associations between motorized transport access, out-of-home activities, and LSM were examined using data from 1,333 older adults (mean age = 70.63) living in 15 cities and towns in Japan. LSM was assessed using composite life-space assessment (LSA) scores. Motorized transport access was measured using dummies showing whether a person had car access (divided into five levels) and used public transport (bus and railway), and out-of-home activities were measured using the number of various activities that were conducted during the most recent weekday and weekend day. Generalized linear models were used to assess the associations.
The sample was dominated by males (74.42%), with more than half of the sample had their own cars. On average, each respondent had four activities during two survey days, and shopping was the most common activity. The results showed that owning a car and using railway, as well as various activities were associated with increased composite LSA scores, whereas no cars or only shared cars in home were associated with decreased composite LSA scores. However, these associations differed between males and females.
In this study, different levels of motorized transport access and different types of out-of-home activities were found to associate differently with composite LSA scores. Based on these findings, we suggest that policymakers should provide more transport access, pay more attention to the LSM of older adults with high clinics/hospital activities, and trigger more shopping and daily leisure activities for older adults to improve the LSM of this population.
Aim: To examine whether mild early time-restricted eating (eating dinner at 18:00 vs. at 21:00) improves 24-h blood glucose levels and postprandial lipid metabolism in healthy adults. Methods: Twelve ...participants (2 males and 10 females) were included in the study. In this 3-day (until the morning of day 3) randomized crossover study, two different conditions were tested: eating a late dinner (at 21:00) or an early dinner (at 18:00). During the experimental period, blood glucose levels were evaluated by each participant wearing a continuous blood glucose measuring device. Metabolic measurements were performed using the indirect calorimetry method on the morning of day 3. The study was conducted over three days; day 1 was excluded from the analysis to adjust for the effects of the previous day’s meal, and only data from the mornings of days 2 and 3 were used for the analysis. Results: Significant differences were observed in mean 24-h blood glucose levels on day 2 between the two groups (p = 0.034). There was a significant decrease in the postprandial respiratory quotient 30 min and 60 min after breakfast on day 3 in the early dinner group compared with the late dinner group (p < 0.05). Conclusion: Despite a difference of only 3 h, eating dinner early (at 18:00) has a positive effect on blood glucose level fluctuation and substrate oxidation compared with eating dinner late (at 21:00).

The aim of this study was to verify our hypothesis that glycemic response is elevated when a meal with a higher carbohydrate content follows a meal with a lower carbohydrate content.
This was a secondary analysis of a study whose primary endpoint was energy metabolism (e.g., energy expenditure and substrate oxidation). This crossover study was designed to test BG responses to 3 types of meals with different macronutrient contents regular meals (R), meals with a high-carbohydrate breakfast (CB), and meals with a high-fat breakfast (FB) using a continuous glucose monitoring system. The R test included 3 meals/d with the same macronutrient composition; the CB test, a high-carbohydrate meal at breakfast, a high-fat meal at lunch, and a high-carbohydrate meal at dinner; and the FB test, a high-fat meal at breakfast, a high-carbohydrate meal at lunch, and a high-carbohydrate meal at dinner. Each test had similar daily macronutrient compositions, except CB and FB had larger variations in carbohydrate content than R. Fourteen healthy young men were tested in random order and underwent whole-body indirect calorimetry.
Daily peak BG concentrations were higher for the CB (mean ± SD: 143.9 ± 25.3 mg/dL) and FB (140.2 ± 24.8 mg/dL) conditions than for the R condition (127.5 ± 15.7 mg/dL). Postprandial BG peaks after a high-carbohydrate meal were ∼20 mg/dL higher when a previous meal was relatively high-fat than when not high-fat (P < 0.05 for all). A multiple regression analysis indicated that the postprandial glycemic response was negatively associated with the preprandial respiratory quotient.
Our findings indicate that switching from high-fat to high-carbohydrate meals contributes to larger postprandial BG excursions, along with alterations in prioritization of carbohydrate utilization. This study was registered at the UMIN Clinical Trials Registry as UMIN000028895.
Background
Although obscure gastrointestinal bleeding cannot be detected by colonoscopy or upper endoscopy, wireless video capsule endoscopy (VCE) is capable of imaging it. Few data are available on ...medical therapy for patients with nonsteroidal anti-inflammatory drug (NSAID)-induced small-intestinal mucosal injuries. The aim of this study was to compare prevention by rebamipide and placebo of NSAID-induced smallintestinal injury in healthy subjects.
Methods
Ten healthy subjects who provided written informed consent were enrolled. Rebamipide or placebo plus diclofenac was administered with omeprazole for 7 days, and for an additional 7-day period with treatments reversed in the same subjects, with a 4-week washout period between treatments. VCE of the small intestine was performed four times, before and after each of the two study periods.
Results
The number of subjects with small-intestinal mucosal injuries was higher in the placebo group (8/10) than in the rebamipide group (2/10) (
P
= 0.023). Two cases of ulcer and one of bleeding were observed in the placebo group, while no ulcer or bleeding was observed in the rebamipide group.
Conclusions
Rebamipide had significantly higher efficacy than placebo in preventing NSAID-induced small-intestinal mucosal injury.

Therefore, to assess the effects of fitness, we developed predictive equations to estimate the ...metabolic equivalent (MET) of daily activities, which includes low intensity activities, by % HR reserve (%HRR), resting HR, and multiple physical characteristics.
Forty volunteers between the ages of 21 and 55 performed 20 types of daily activities while recording HR and sampling expired gas to evaluate METs values. Multiple regression analysis was performed to develop prediction models of METs with seven potential predictors, such as %HRR, resting HR, and sex. The contributing parameters were selected based on the brute force method. Additionally, leave-one-out method was performed to validate the prediction models.
%HRR, resting HR, sex, and height were selected as the independent variables. %HRR showed the highest contribution in the model, while the other variables exhibited small variances. METs were estimated within a 17.3% difference for each activity, with large differences in document arrangement while sitting (+17%), ascending stairs (-8%), and descending stairs (+8%).
The results showed that %HRR is a strong predictor for estimating the METs of daily activities. Resting HR and other variables were mild contributors. (201 words).
Abstract
Background
Interindividual variability in 24-hour energy expenditure (24EE) during energy-balance conditions is mainly determined by differences in body composition and demographic factors. ...Previous studies suggested that 24EE might also be influenced by sympathetic nervous system activity via catecholamine (norepinephrine, epinephrine) secretion. Therefore, we analyzed the association between catecholamines and energy expenditure in 202 individuals from a heterogeneous population of mixed ethnicities.
Methods
Participants (n = 202, 33% female, 14% black, 32% white, 41% Native American, 11% Hispanic, age: 36.9 ± 10.3 y mean ± SD, percentage body fat: 30.3 ± 9.4) resided in a whole-room calorimeter over 24 hours during carefully controlled energy-balance conditions to measure 24EE and its components: sleeping metabolic rate (SMR), awake-fed thermogenesis (AFT), and spontaneous physical activity (SPA). Urine samples were collected, and 24-h urinary epinephrine and norepinephrine excretion rates were assessed by high-performance liquid chromatography.
Results
Both catecholamines were associated with 24EE and SMR (norepinephrine: +27 and +19 kcal/d per 10 μg/24h; epinephrine: +18 and +10 kcal/d per 1 μg/24h) in separate analyses after adjustment for age, sex, ethnicity, fat mass, fat-free mass, calorimeter room, temperature, and physical activity. In a multivariate model including both norepinephrine and epinephrine, only norepinephrine was independently associated with both 24EE and SMR (both P < .008), whereas epinephrine became insignificant. Neither epinephrine nor norepinephrine were associated with adjusted AFT (both P = .37) but epinephrine was associated with adjusted SPA (+0.5% per 1 μg/24h).
Conclusions
Our data provide compelling evidence that sympathetic nervous system activity, mediated via norepinephrine, is a determinant of human energy expenditure during nonstressed, eucaloric conditions.
A lower body mass is associated with the progression of Alzheimer's disease (AD) and the risk of mortality in patients with AD; however, evidence of genetic determinants of decreased body mass in ...cognitively impaired older adults is limited. We therefore investigated the genetic effect of APOE-ε4 on body composition in older adults with mild cognitive impairment (MCI) and early-to-moderate-stage AD. A total of 1631 outpatients (aged 65-89 years) with MCI and early-to-moderate-stage AD were evaluated for the association between body composition and APOE-ε4 status. After adjusting for covariates, including cognitive function evaluated with the Mini-Mental State Examination, the presence of the APOE-ε4 was associated with lower weight (β = -1.116 ± 0.468 kg per presence,
= 0.017), fat mass (β = -1.196 ± 0.401 kg per presence,
= 0.003), and percentage of body fat (β = -1.700 ± 0.539% per presence,
= 0.002) in women but not in men. Additionally, the impact of APOE-ε4 on measures of body composition in women was more remarkable in MCI than in AD patients. The presence of the APOE-ε4 allele was associated with lower fat mass, particularly in women with MCI, independent of cognitive decline.
