Patient safety is a key priority in quality management for healthcare services providers. Every patient is entitled to receive safe and effective healthcare.
The aim of this study was to design a ...patient safety plan for a Paediatric Surgery Department.
We carried out a literature review and we established a work group that included healthcare professionals from the Paediatric Surgery Department and the Quality and Medical Records Department. The group identified potential adverse events, failures and causes and established a rating using Failure Mode Effects Analysis. Potential risks were mapped out and a plan was designed establishing actions to reduce risks. We designated leaders to ensure the effective implementation of the plan.
A total of 58 adverse events were identified in the Paediatric Surgery Department. We detected 128 failures that were produced by 211 different causes. The group developed a proposal with 424 specific measures to carry out preventive and/or remedial actions that were then narrowed down to 322. The group designed a plan to apply the programme, which is currently being implemented.
The methodology used enabled obtaining key information for improvement of patient safety and developing preventive and/or remedial actions. These measures are applicable in practice, as they were designed using proposals and agreements with professionals that take active part in the care of children with surgical conditions.

Amorphous ferromagnetic wire with a highly positive saturation magnetostriction coefficient, made of Fe sub 73.5 Si sub 13.5 B sub 9 Nb sub 3 Cu sub 1 , was simultaneously submitted to both, an AC ...current passing through it and a torsional stress, in order to induce a helical magnetic anisotropy in the wire that modifies its domain structure and therefore the magnetic response of the sample. The aim of this work is to study the reactive, resistive and impedance behaviour in this Fe-rich wire, submitted to different applied torsional stresses, and for several values of the AC current amplitude through the wire (5-20 mA sub rms ), in the low frequency range, f < MHz. The experimental results are explained on the basis of the core-shell model.
Background Few studies have evaluated the possible beneficial effect of the administration of stem cells in the early stages of stroke. Intravenous administration of allogeneic mesenchymal stem cells ...(MSCs) from adipose tissue in patients with acute stroke could be a safe therapy for promoting neurovascular unit repair, consequently supporting better functional recovery. We aim to assess the safety and efficacy of MSC administration and evaluate its potential as a treatment for cerebral protection and repair. Materials A Phase IIa, prospective, randomized, double-blind, placebo-controlled, single-center, pilot clinical trial. Twenty patients presenting acute ischemic stroke will be randomized in a 1:1 proportion to treatment with allogeneic MSCs from adipose tissue or to placebo (or vehicle) administered as a single intravenous dose within the first 2 weeks after the onset of stroke symptoms. The patients will be followed up for 2 years. Primary outcomes for safety analysis: adverse events (AEs) and serious AEs; neurologic and systemic complications, and tumor development. Secondary outcomes for efficacy analysis: modified Rankin Scale; NIHSS; infarct size; and biochemical markers of brain repair (vascular endothelial growth factor, brain-derived neurotrophic factor, and matrix metalloproteinases 9). Results and Conclusions To our knowledge, this is the first, phase II, pilot clinical trial to investigate the safety and efficacy of intravenous administration of allogeneic MSCs from adipose tissue within the first 2 weeks of stroke. In addition, its results will help us define the best criteria for a future phase III study.

In this work we report some results concerning the torsion impedance of amorphous Co sub 68.1 Fe sub 4.4 Si sub 12.5 B sub 15 wires in the as-cast state, and after a stress annealing treatment. The ...influence of the drive current amplitude and frequency is also studied for these samples. A sensitivity to the applied torsion up to 17.6%/rad m is obtained for the annealed wire. The magnetoimpedance response of both types of samples gives evidence to the difference in their domain structure.

A simple consideration of the alloy as the sum of two magnetic systems is applied with good results. The observed discrepancies are explained as due to a magnetic interaction between the Co and Fe atoms.
