In the age of booming information technology, human-being has witnessed the need for new paradigms with both high computational capability and low latency. A potential solution is Vehicular Edge ...Computing (VEC). Previous work proposed a Fuzzy Deep Q-Network in Offloading scheme (FDQO) that combines Fuzzy rules and Deep Q-Network (DQN) to improve DQN’s early performance by using Fuzzy Controller (FC). However, we notice that frequent usage of FC can hinder the future growth performance of model. One way to overcome this issue is to remove Fuzzy Controller entirely. We introduced an algorithm called baseline DQN (b-DQN), represented by its two variants Static baseline DQN (Sb-DQN) and Dynamic baseline DQN (Db-DQN), to modify the exploration rate base on the average rewards of closest observations. Our findings confirm that these baseline DQN algorithms surpass traditional DQN models in terms of average Quality of Experience (QoE) in 100 time slots by about 6%, but still suffer from poor early performance (such as in the first 5 time slots). Here, we introduce baseline FDQO (b-FDQO). This algorithm has a strategy to modify the Fuzzy Logic usage instead of removing it entirely while still observing the rewards to modify the exploration rate. It brings a higher average QoE in the first 5 time slots compared to other non-fuzzy-logic algorithms by at least 55.12%, prevent the model from getting too bad result over all time slots, while having the late performance as good as that of b-DQN.
The present study aimed to evaluate the effectiveness of using platelet-rich fibrin (PRF) as the apical matrix for the placement of MTA in nonsurgical endodontic therapy for teeth with periapical ...lesions and open apices.
Twelve teeth from eleven patients with periapical periodontitis and open apices were enrolled in the study. Nonsurgical endodontic therapy was performed with the PRF used as an apical barrier and the MTA manipulated as an apical plug for further thermoplasticized gutta percha in the remaining part of the root canal. Clinical signs and periapical digital radiographs were recorded and analyzed to evaluate the curing progress after periodical follow-ups of 1, 3, and 6 months. The horizontal dimension of the periapical lesion was determined, and the changes in the dimensions were recorded each time. The Friedman test was used for statistical analysis, with P < .05 serving as the threshold for determining statistical significance.
All patients had no clinical symptoms after the first month of treatment, with a significant reduction in the periapical lesion after periodical appointments.
PRF is an effective barrier when combined with MTA for the treatment of teeth with periapical periodontitis and open apices.
In the age of the ever-growing number of tasks generated from the Internet of Things (IoT) devices, one of the most crucial problems with enhancing the Quality of Service in multi-access computing ...(MEC) is to have a low overdue-task rate (tasks with processing time greater than their deadlines) while minimizing the energy consumed. To properly formulate the task offloading in a vehicular network, we consider both the number of overdue tasks and the total energy consumed by the edge system. We focus not only on maximizing the users' experience by minimizing the percentage of overdue tasks rate but also on saving energy for the service provider. Thus, our Deadline-constrained and Energy-aware problem requires finding a task offloading strategy to reduce the total power consumption of the edge system while still minimizing the number of overdue tasks. Findings from a 2D-street real-data bus traces are also provided for analysis. Furthermore, we develop a schema based on rein-forcement learning techniques named Deadline Constrained and Energy-Aware Task Offloading (DCEAO) to solve the problem. It proves to reduce the base station's power consumption by 38% to 51% while maintaining competitive overdue-task rate to other benchmarks that only focus on minimizing overdue-task rate.
It is generally difficult to specify the sources of infection by which domestic animals may acquire pathogens. Through 16S rRNA gene amplicon sequencing, we compared the composition of microbiota in ...the saliva, vaginal mucus, and feces of pigs, and in swabs of feeder troughs and water dispensers collected from pig farms in Vietnam. The composition of the microbiota differed between samples in each sample group. Streptococcus, Actinobacillus, Moraxella, and Rothia were the most abundant genera and significantly discriminative in saliva samples, regardless of the plasticity and changeability of the composition of microbiota in saliva. Moreover, species assignment of the genus Streptococcus revealed that Streptococcus suis was exceptional in the salivary microbiota, due to being most abundant among the streptococcal species and sharing estimated proportions of 5.7%-9.4% of the total bacteria in saliva. Thus, pig oral microbiota showed unique characteristics in which the major species was the pig pathogen. On the other hand, β-diversity analysis showed that the microbiota in saliva was distinct from those in the others. From the above results, pig saliva was shown to be the major natural habitat of S. suis, and is suggested to be the most probable source of S. suis infection.
Antibiotic resistance (AR) rates in Vietnam are among the highest in Asia, and recent infections due to multi-drug resistance in the country have caused thousands of deaths each year. This study ...investigated a Vietnamese community's preferences for antibiotic treatment and its knowledge and attitudes regarding antibiotics. A discrete choice experiment-based survey was developed and administered to the population of interest. The respondents were given sociodemographic-, knowledge- and attitude-related items and 17 pairs of choice tasks. Two hypothetical options were included in each choice task. Latent class analysis was conducted to determine the differences among the respondents' preferences. Among 1,014 respondents, 805 (79.4%) gave valid questionnaires. A three-latent-class model with four covariates (age, healthcare-related education or career, occupation, and attitude classifications) was used in the analysis. All five attributes significantly influenced the respondents' decisions. The majority, including young employed respondents with non-healthcare-related work or education, found treatment failure more important. Older respondents who had healthcare-related education/careers and/or appropriate antibiotic use- and antibiotics resistance-related attitudes, regarded contribution to antibiotic resistance as an important attribute in selecting antibiotic treatments. Unemployed individuals with correct knowledge identified the cost of antibiotic treatment as the most essential decision-making factor. Findings suggest minimal antibiotic impact on resistance; only 7.83% view it as amajor concern. The respondents exhibited substantial preference heterogeneity, and the general Vietnamese public had poor knowledge of and attitudes toward antibiotic use and antibiotic resistance. This study emphasizes the need for individual responsibility for antibiotic resistance and appropriate antibiotic use.
The World Health Organization recently recommended Video Observed Therapy (VOT) as one option for monitoring tuberculosis (TB) treatment adherence. There is evidence that private sector TB treatment ...has substandard treatment follow-up, which could be improved using VOT. However, acceptability of VOT in the private sector has not yet been evaluated.
We conducted a cross-sectional survey employing a theoretical framework for healthcare intervention acceptability to measure private provider perceptions of VOT across seven constructs in three cities of Viet Nam: Ha Noi, Ho Chi Minh City, and Hai Phong. We investigated the differences in private providers' attitudes and perceptions of VOT using mixed ordinal models to test for significant differences in responses between groups of providers stratified by their willingness to use VOT.
A total of 79 private providers completed the survey. Sixty-two providers (75%) indicated they would use VOT if given the opportunity. Between private providers who would and would not use VOT, there were statistically significant differences (p≤0.001) in the providers' beliefs that VOT would help identify side effects faster and in their confidence to monitor treatment and provide differentiated care with VOT. There were also significant differences in providers' beliefs that VOT would save them time and money, address problems faced by their patients, benefit their practice and patients, and be relevant for all their patients.
Private providers who completed the survey have positive views towards using VOT and specific subpopulations acknowledge the value of integrating VOT into their practice. Future VOT implementation in the private sector should focus on emphasizing the benefits and relevance of VOT during recruitment and provide programmatic support for implementing differentiated care with the technology.
A synergistic multilayer membrane design is necessary to satisfy a multitude of requirements of an ideal wound dressing. In this study, trilayer dressings with asymmetric wettability, composed of ...electrospun polycaprolactone (PCL) base membranes coated with oligomer chitosan (COS) in various concentrations of polyvinylpyrrolidone (PVP), are fabricated for wound dressing application. The membranes are expected to synergize the hygroscopic, antibacterial, hemostatic, and biocompatible properties of PCL and COS. The wound dressing was coated by spraying the solution of 3% COS and 6% PVP on the PCL base membrane (PVP6–3) three times, which shows good interaction with biological subjects, including bacterial strains and blood components. PVP6–3 samples confirm the diameter of inhibition zones of 20.0 ± 2.5 and 17.9 ± 2.5 mm against Pseudomonas aeruginosa and Staphylococcus aureus, respectively. The membrane induces hemostasis with a blood clotting index of 74% after 5 min of contact. In the mice model, wounds treated with PVP6–3 closed 95% of the area after 10 days. Histological study determines the progression of skin regeneration with the construction of granulation tissue, new vascular systems, and hair follicles. Furthermore, the newly-growth skin shares structural resemblances to that of native tissue. This study suggests a simple approach to a multi-purpose wound dressing for clinical treatment.
The treatment of wastewater from the textile industry containing organic dyes faces many challenges since these compounds resist the biodegradation process in conventional treatment units. Among the ...physicochemical processes, photocatalysis is considered a facile, cheap, and environmental-friendly technology for treating persistent organic pollutants in waters at low concentrations. This study investigated several physicochemical factors determining the photocatalytic activity of titanate nanotubes (TNTs) to remove Procion MX 032 (PMX), an azo dye, in waters. Degradation of PMX by photocatalytic oxidation process at room temperature (30°C) was set up with the UV irradiation in the presence of different types of photocatalyst such as ST-01 (100% anatase), industrial TiO2, TNTs calcined at 120°C and 500°C. Effect of reaction time, catalyst amount, pH, light wavelength and intensity, and oxidants was investigated. Consequently, TNTs calcined at 500°C provided the highest removal efficiency. The photocatalytic oxidation of PMX by TNT calcined at 500°C was affected by pH variation, getting the highest removal at pH of 8, and inhibited with the presence of H2O2 and O2. Particularly, the PMX degradation using titanate nanotubes was optimized under the UV-A intensity of 100 W/m2. The dye was degraded by more than 95% at the TNTs concentration of 75 mg/L and pH 8.0 after 90 min. The results suggest that photocatalysis using TNTs can be a simple but efficient treatment method to remove PMX and potentially be applied for the treatment of wastewaters containing dyes.
Objectives:
In Vietnam, although surveillance and control of multidrug-resistant organisms is a national priority, information on the burden of these pathogens remains scarce. At the University ...Medical Center in Ho Chi Minh City, we assessed the proportion of carbapanemase-producing carbapenem-resistant organisms (CP-CRO) and evaluated an intervention package to prevent transmission of carbapenemase-producing carbapenem-resistant Enterobacterieacea (CP-CRE) in the intensive care unit (ICU).
Methods:
All gram-negative isolates collected between November 2018 to April 2019 were tested for carbapenem resistance using the disc-diffusion method. Carbapenem-resistant bacteria, defined as meropenem resistant, were tested for phenotypic carbapenemase-production using the Becton Dickinson Phoenix CPO Detect assay. An intervention package, including placement of patients in cohorts, enhanced barrier precautions, enhanced discharge environmental cleaning, and CP-CRE rectal screening, was implemented from July 2019 through December 2020. During this period, all ICU patients were screened on admission, and negative patients were rescreened every 2 days or 7 days until discharge, death, or CRE-positive result. Admission prevalence and incidence of CP-CRE transmission was calculated among CP-CRE infected or colonized patients.
Results:
Among 599 gram-negative isolates collected, 108 were carbapenem-resistant isolates, of which 107 (99%) were CP-CRO by the phenotypic method. Most CP-CRO were
Acinetobacter baumannii
(42%) and
Klebsiella pneumoniae
(36%). Of 1,206 patients, 433 (35.9%) were already colonized or infected with CP-CRE before admission to the ICU. The incidence rate (cases per 100 risk days) of CP-CRE colonization or infection during ICU treatment decreased from 11.5 before the intervention to 2.9 after the implementation of the intervention package. The average number of days to change from a negative to positive screening result in the intervention phase was 7.4, compared with 4.9 days during preintervention phase.
Conclusions:
Nearly all CROs isolated from our ICU are carbapenemase-producing CROs, with high presence on admission as well as new acquisition during an ICU stay. An intervention package containing enhanced infection control measures was effective in reducing CP-CRE transmission.
