Dix-sept patients porteurs d'un cancer colorectal métastatique ont reçu en première (12 cas) ou seconde ligne (5 cas) thérapeutique une chronochimiothérapie associant l'irinotecan (CPT11) (au jour 1, ...perfusé de 2 à 8 heures avec un pic à 5 heures) à du 5 FU et de l'acide folinique (respectivement 700 et 300 mg/m
2 par jour – J2 à J5 ; infusion de 22 heures à 10 heures avec pic à 4 heures) et du carboplatine (40 mg/m
2/jour – J2 à 5 ; infusion de 10 heures à 22 heures avec pic à 16 heures). Les cures étaient répétées toutes les deux semaines. Le CPT 11 a pu être monté de 120 à 180 mg/m
2 (3
e cohorte) dans la phase I de l'étude, les toxicités clinique et biologique étant réduites. Cinq patients ont déjà été traités dans la phase II. L'efficacité clinique s'est avérée intéressante : aucune progression tumorale ; 88 % de réponses tumorales majeures ; survie sans progression de neuf mois et survie médiane de 19,7 mois.
Seventeen colorectal cancer patients received as first (12 cases) or second line (5 cases) treatment a combined chronotherapy with CPT 11 (infused at day 1 from 2 to 8 am; peak at 5 am), given with 5 FU (700 mg/m
2 per day ; days 2–5) and folinic acid (300 mg/m
2 per day, days 2–5) both infused from 10 pm to 10 am with a peak at 4 am, and carboplatin (40 mg/m
3/day – days 2–5 ; infused from 10 am to 10 pm – peak at 4 pm). The doses of CPT 11 could be easily increased from 120 up to 180 mg/m
2 in successive cohorts of four patients through acceptable toxicity. Thus, five patients have already been treated in the phase II part of the trial. An interesting tumoral outcome has been observed: 88% major responses with no progression; median time to progression: nine months and median survival: 19.7 months.
Trente-sept patients opérés d'un cancer du colon, de stade Dukes B2 ou C ont été randomisés pour recevoir neuf cures de chimiothérapie adjuvante infusionnelle comportant du 5 FU et de l'acide ...folinique avec ou sans carboplatine, en administration standard (2 jours toutes les 2 semaines : schéma de Gramont) ou chronomodulée (4 jours toutes les 2 semaines). La tolérance globale s'est avérée excellente, avec moins de 7 % des cures nécessitant une adaptation thérapeutique. Les bras comportant le carboplatine ont présenté une toxicité hématologique accrue ; aussi plus de toxicité cutanée fut constatée dans le groupe recevant la chronothérapie avec les trois substances.
Thirty-seven patients operated from a Dukes B2-C colon cancer were randomised to receive as adjuvant infusional chemotherapy, nine 5 FU and folinic acid courses with or without carboplatin, as standard (de Gramont ; 2 days every 2 weeks) or chronomodulated administration (4 days every 2 weeks). The overall tolerance was judged excellent with less than 7% courses with dose-adaptations. The two carboplatin arms presented an enhanced haematological toxicity, while some more cutaneous toxicity was observed in the chronomodulated arm with the three drugs.
Soixante-douze patients souffrant d'un cancer colorectal métastatique ont reçu en première ligne de traitement une chronothérapie comportant du 5-fluorouracile, de l'acide folinique (respectivement ...700 et 300 mg/m
2 par jour, infusion de 22 heures à 10 heures avec pic à 4 heures) et du carboplatine (40 mg/m
2, infusé de 10 heures à 22 heures avec pic à 16 heures). Les cures de quatre jours furent répétées toutes les deux semaines. Une réponse tumorale majorée a été observée chez 60 % des patients (chez 68 % de ceux n'ayant pas reçu de chimiothérapie adjuvante préalable) au prix d'une toxicité limitée (toxicité de grade 3 ou 4 : clinique dans 5 % des cas ; hématologique : dans 10 à 29 % des cas). La médiane de la survie sans progression et de la survie globale s'est établie respectivement à 11 et 27 mois. L'index thérapeutique observé suggère l'intérêt d'associer le carboplatine à un traitement infusionnel à base de 5-FU et acide folique dans le traitement du cancer colorectal.
Seventy-two patients suffering from a metastatic colorectal cancer received, as first line treatment, a combination chronotherapy with 5-FU and folinic acid (infused from 10 pm to 10 am with a peak at 4 am, respectively at doses of 700 and 300 mg/m
2 per day) and carboplatin (infused at the dose of 40 mg/m
2 per day from 10 am to 10 pm with a peak at 4 pm). The courses of four days were repeated every two weeks. A major tumoral response was observed in 60% cases (68% in those not previously treated with adjuvant chemotherapy). The median times to progression and overall survival established at 11 and 27 months. The clinical (grades 3–4 in maximum 5% cases) and hematological (grades 3–4 in maximum 10–29% cases) toxicities were quite limited. Our observations suggest the interest to incorporate carboplatin in the combined infusional treatment of colorectal cancer.
Objective To assess the relation between self reported intensity and frequency of stress and first time incidence of primary breast cancer. Design Prospective cohort study with 18 years of follow-up. ...Setting Copenhagen City heart study, Denmark. Participants The 6689 women participating in the Copenhagen City heart study were asked about their perceived level of stress at baseline in 1981-3. These women were followed until 1999 in the Danish nationwide cancer registry, with < 0.1% loss to follow-up. Main outcome measure First time incidence of primary breast cancer. Results During follow-up 251 women were diagnosed with breast cancer. After adjustment for confounders, women with high levels of stress had a hazard ratio of 0.60 (95% confidence interval 0.37 to 0.97) for breast cancer compared with women with low levels of stress. Furthermore, for each increase in stress level on a six point stress scale an 8% lower risk of primary breast cancer was found (hazard ratio 0.92, 0.85 to 0.99). This association seemed to be stable over time and was particularly pronounced in women receiving hormone therapy. Conclusion High endogenous concentrations of oestrogen are a known risk factor for breast cancer, and impairment of oestrogen synthesis induced by chronic stress may explain a lower incidence of breast cancer in women with high stress. Impairment of normal body function should not, however, be considered a healthy response, and the cumulative health consequences of stress may be disadvantageous.
