Currently, the French High Authority for Health does not recommend mass screening for prostate cancer (PCa), due to the risk of over-treatment, notably of low risk patients. Our study is intended to ...reflect the therapeutic attitudes for the management of patients classified as low risk of progression in French clinical centers.
For all positive prostate biopsies performed during 2012 and 2013 in five French departments of urology, clinicopathological characteristics required to calculate the d'Amico risk group and the Cancer of the Prostate Risk Assessment (CAPRA) score were filled. Information on the first treatment of "low risk" patients was collected.
A total of 1035 patients were included, with a median age at diagnosis of 66 years old. According to d'Amico and CAPRA classifications, 30.4% and 35.0% of patients were at low, 34.5% and 33.2% at intermediate, 35.1% and 31.8% at high risk. The diagnosis severity increased with age (P<0.0001). The main treatment for low risk patients was radical prostatectomy (41.6% and 42.0% for d'Amico and CAPRA, respectively), but active surveillance was the most frequent treatment if diagnosed after 75 years old. The management of low risk patients varied significantly between centers (P<0.0001), according to the therapeutic platforms available within the hospital.
In absence of strong progression predictor, the management of low risk PCa remains based on center habits and local therapeutic platforms. New predictive markers, such as multiparametric MRI or molecular tests, are needed to guide rational management of low risk PCa.

La hipertermia hídrica se ha utilizado para tratamientos de diversas formas de cáncer, en hueso aún no se ha determinado cuál es su efecto; se realizó este estudio prospectivo para demostrar los ...cambios estructurales y metabólicos y los efectos en la consolidación de hueso expuesto a hipertermia hídrica. Se utilizaron 30 conejos de raza New-Zealand. Con peso entre 2.8 y 3.2 kg divididos en 9 grupos y 3 subgrupos, se realizó osteotomía en el tercio medio del fémur derecho; este segmento fue expuesto a hipertermia a 15, 20 y 25 grados centígrados, durante 50, 60 y 70 minutos, gammagrafía ósea cada cuatro semanas y radiográficos cada semana hasta la semana 13. Para el análisis estadístico se utilizó la prueba de post-hoc y test de Tukey, encontrando diferencias en las concentraciones de elementos químicos a temperaturas superiores a 60 grados durante 20 minutos con significancia estadística, retardo en la consolidación y evidencia de actividad metabólica. Se concluye que con la exposición a temperaturas de 60 grados durante 20 minutos se presentaron alteraciones morfológicas en potasio, magnesio, azufre y fósforo y retardo en la consolidación ósea. Estos resultados son usados como parámetros para el tratamiento con hipertermia hídrica controlada en tumores óseos.
We explore the possibility of having a fermionic dark matter candidate within \(U(1)'\) models for CE\(\nu\)NS experiments in light of the latest COHERENT data and the current and future dark matter ...direct detection experiments. A vector-like fermionic dark matter has been introduced which is charged under \(U(1)'\) symmetry, naturally stable after spontaneous symmetry breaking. We perform a complementary investigation using CE\(\nu\)NS experiments and dark matter direct detection searches to explore dark matter as well as \(Z^{\prime}\) boson parameter space. Depending on numerous other constraints arising from the beam dump, LHCb, BABAR, and the forthcoming reactor experiment proposed by the SBC collaboration, we explore the allowed region of \(Z^{\prime}\) portal dark matter.
We present a minimal left-right symmetric flavor model and analyze the predictions for the neutrino sector. In this scenario, the Yukawa sector is shaped by the dihedral \(D_4\) symmetry which leads ...to correlations for the neutrino mixing parameters. We end up with four possible solutions within this model. We further analyzed the impact of the upcoming long-baseline neutrino oscillation experiment DUNE. Due to its high sensitivity, DUNE will be able to rule out two of the solutions. Finally, the prediction for the neutrinoless double beta decay for the model has also been examined.
This retrospective cohort study reviews the incidence of bacteremia in 48 patients undergoing hemodialysis using subclavian vein dialysis catheters (SDC) as temporary vascular access. Twelve (25%) of ...these patients had catheter-related bacteremia, and the most frequently isolated organisms were coagulase-negative staphylococci. Three patients developed right-sided endocarditis and one of them died due to pulmonary embolism. The presence of possible risk factors for SDC-related bacteremia, including duration of catheterization and number of hemodialysis procedures, were not statistically different when patients with and without bacteremia were compared, with the exception of a significantly lower incidence of bacteremia among those patients receiving antibiotic therapy at the time of catheter insertion. The use of resterilized catheters was not a risk factor. Specific guidelines for SDC insertion and care were established and followed, after which the infection frequency was reduced to 7.5% (1 episode per 45.5 patient-weeks of catheter use) in this high-risk population.
Abstract Background With the availability of high-quality asthma guidelines worldwide, one possible approach of developing a valid guideline, without re-working the evidence, already analysed by ...major guidelines, is the ADAPTE approach, as was used for the development of National Guidelines on asthma. Methods The guidelines development group (GDG) covered a broad range of experts from medical specialities, primary care physicians and methodologists. The core group of the GDG searched the literature for asthma guidelines 2005 onward, and analysed the 11 best guidelines with AGREE-II to select three mother guidelines. Key clinical questions were formulated covering each step of the asthma management. Results The selected mother guidelines are British Thoracic Society (BTS), GINA and GEMA 2015. Responses to the questions were formulated according to the evidence in the mother guidelines. Recommendations or suggestions were made for asthma treatment in Mexico by the core group, and adjusted during several rounds of a Delphi process, taking into account: 1. Evidence; 2. Safety; 3. Cost; 4. Patient preference – all these set against the background of the local reality. Here the detailed analysis of the evidence present in BTS/GINA/GEMA sections on prevention and diagnosis in paediatric asthma are presented for three age-groups: children with asthma ≤5 years, 6–11 years and ≥12 years. Conclusions For the prevention and diagnosis sections, applying the AGREE-II method is useful to develop a scientifically-sustained document, adjusted to the local reality per country, as is the Mexican Guideline on Asthma.

Avena barbata, a tetraploid grass, is much more widely adapted and successful in forming dense stands than its diploid ancestors. The success of such polyploids has often been attributed to heterosis ...associated with ability to breed true for a highly heterozygous state in which allelic differences between the parents are fixed in the polyploid by chromosome doubling. We have examined the relationship between genetic diversity and adaptedness for 14 allozyme loci in A. barbata and its diploid ancestors in samples collected from diverse habitats in Israel and Spain. The relationship varied from locus to locus: superior adaptedness was associated with genetic uniformity for five loci, in part with genetic uniformity and in part with genetic diversity (monomorphism for a single heteroallelic quadriplex) for one locus, and with allelic diversity in the form of heteroallelic quadriplexes combined with genotypic diversity in the form of complex polymorphisms among different homoallelic and/or heteroallelic quadriplexes for the eight remaining loci. These results indicate that allelic diversity fixed in nonsegregating form through chromosome doubling was an important factor in the evolution of adaptedness in A. barbata. However, it is unlikely that heterosis associated with heterozygosity contributed significantly to superior adaptedness in either the diploids or the tetraploid because virtually all loci (≈99%) were homozygous in the Avena diploids and tetraploid.
