Standardizing case definitions for priority vaccine safety conditions facilitates uniform evaluation and consolidation of data obtained from different settings. The Brighton Collaboration case definitions (BCCD) were created to support this harmonization and enable classification from level 1 (most certain) to level 5 (not a case) of certainty. Assessing the performance of BCCD in practice is critical, particularly in resource-limited settings, where many new vaccines may be introduced without prior monitoring in high-income countries. We assessed the performance of BCCD in Addis Ababa, Ethiopia, as applicable to COVID-19 and other vaccines.
Active surveillance was conducted at Tikur Anbessa Specialized Hospital, the largest referral hospital in Ethiopia. During June 1, 2022–May 31, 2023, three trained physicians prospectively identified patients eligible for COVID-19 vaccination (regardless of vaccine receipt) who presented with one or more of eleven pre-specified adverse events of special interest (AESI) from the emergency department and inpatient wards. Standardized data collection forms were used to capture patient information and assign level of certainty (LOC), regardless of vaccination status for COVID-19. We conducted descriptive analysis to characterize cases and the LOCs reached for each AESI.
We detected 203 AESI cases. The most detected conditions were thrombosis and thromboembolism (n = 100, 49 %) and generalized convulsions (n = 38, 19 %). Ninety-six percent of the cases were confirmed at levels 1–3 (n = 187) or level 5 (n = 9) LOC. Non-classifiable (level 4) cases were observed for pericarditis (n = 2), encephalitis (n = 2), myelitis (n = 2), and generalized convulsion (n = 1).
The BCCD were successfully applied in > 95 % of cases in a large referral hospital in Ethiopia, with generalized convulsion, pericarditis, and encephalomyelitis as the exceptions. We recommend further evaluation in other low-resource settings, particularly in rural or non-referral hospitals, to gain additional insights into performance of these definitions for revision or adaptation, as needed.
Ordinary in Brighton? offers the first large scale examination of the impact of the UK equalities legislation on lesbian, gay, bi- and trans (LGBT) lives, and the effects of these changes on LGBT ...political activism. Using the participatory research project, Count Me In Too, this book investigates the material issues of social/spatial injustice that were pertinent for some - but not all- LGBT people, and explores activisms working in partnership that operated with/within the state. Ordinary in Brighton? explores the unevenly felt consequences of assimilation and inclusion in a city that was compelled to provide a place (literally and figuratively) for LGBT people. Brighton itself is understood to be exceptional, and exploring this specific location provides insights into how place operates as constitutive of lives and activisms. Despite its placing as ’the gay capital’ and its long history as a favoured location of LGBT people, there is very little academic or popular literature published about this city. This book offers insights into the first decade of the 21st century when sexual and gender dissidents supposedly became ordinary here, rather than exceptional and transgressive. It argues that geographical imaginings of this city as the ’gay capital’ formed activisms that sought positive social change for LGBT people. The possibilities of legislative change and urban inclusivities enabled some LGBT people to live ordinary lives, but this potential existed in tension with normalisations and exclusions. Alongside the necessary critiques, Ordinary in Brighton? asks for conceptualisations of the creative and co-operative possibilities of ordinariness. The book concludes by differentiating the exclusionary ideals of normalisation from the possibilities of ordinariness, which has the potential to render a range of people not only in-place, but commonplace. In 2020, prior to COVID-19 vaccine rollout, the Brighton Collaboration created a priority list, endorsed by the World Health Organization, of potential adverse events relevant to COVID-19 vaccines. ...We adapted the Brighton Collaboration list to evaluate serious adverse events of special interest observed in mRNA COVID-19 vaccine trials.
Secondary analysis of serious adverse events reported in the placebo-controlled, phase III randomized clinical trials of Pfizer and Moderna mRNA COVID-19 vaccines in adults (NCT04368728 and NCT04470427), focusing analysis on Brighton Collaboration adverse events of special interest.
Pfizer and Moderna mRNA COVID-19 vaccines were associated with an excess risk of serious adverse events of special interest of 10.1 and 15.1 per 10,000 vaccinated over placebo baselines of 17.6 and 42.2 (95 % CI −0.4 to 20.6 and −3.6 to 33.8), respectively. Combined, the mRNA vaccines were associated with an excess risk of serious adverse events of special interest of 12.5 per 10,000 vaccinated (95 % CI 2.1 to 22.9); risk ratio 1.43 (95 % CI 1.07 to 1.92). The Pfizer trial exhibited a 36 % higher risk of serious adverse events in the vaccine group; risk difference 18.0 per 10,000 vaccinated (95 % CI 1.2 to 34.9); risk ratio 1.36 (95 % CI 1.02 to 1.83). The Moderna trial exhibited a 6 % higher risk of serious adverse events in the vaccine group: risk difference 7.1 per 10,000 (95 % CI –23.2 to 37.4); risk ratio 1.06 (95 % CI 0.84 to 1.33). Combined, there was a 16 % higher risk of serious adverse events in mRNA vaccine recipients: risk difference 13.2 (95 % CI −3.2 to 29.6); risk ratio 1.16 (95 % CI 0.97 to 1.39).
The excess risk of serious adverse events found in our study points to the need for formal harm-benefit analyses, particularly those that are stratified according to risk of serious COVID-19 outcomes. These analyses will require public release of participant level datasets.
Guillain-Barré syndrome is an acute polyradiculoneuropathy with a variable clinical presentation. Accurate diagnostic criteria are essential for patient care and research, including clinical trials ...and vaccine safety studies. Several diagnostic criteria for Guillain-Barré syndrome have been proposed, including the recent set by the Brighton Collaboration. In the present study we describe in detail the key diagnostic features required to meet these Brighton criteria in a study population of 494 adult patients with Guillain-Barré syndrome, previously included in therapeutic and observational studies. The patients had a median age of 53 years (interquartile range 36-66 years) and males slightly predominated (56%). All patients developed bilateral limb weakness which generally involved both upper and lower extremities. The weakness remained restricted to the legs in 6% and to the arms in 1% of the patients. Decreased reflexes in paretic arms or legs were found initially in 91% of patients and in all patients during follow-up. Ten (2%) patients however showed persistence of normal reflexes in paretic arms. Disease nadir was reached within 2 weeks in 80%, within 4 weeks in 97% and within 6 weeks in all patients. A monophasic disease course occurred in 95% of patients, of whom 10% had a treatment-related fluctuation. A clinical deterioration after 8 weeks of onset of weakness occurred in 23 (5%) patients. Cerebrospinal fluid was examined in 474 (96%) patients. A mild pleocytosis (5 to 50 cells/μl) was found in 15%, and none had more than 50 cells/μl. An increased cerebrospinal fluid protein concentration was found only in 64% of patients, highly dependent on the timing of the lumbar puncture after onset of weakness (49% at the first day to 88% after 2 weeks). Nerve electrophysiology was compatible with the presence of a neuropathy in 99% of patients, but only 59% fulfilled the current criteria for a distinct subtype of Guillain-Barré syndrome. Patients with a complete data set (335) were classified according to the Brighton criteria, ranging from a high to a low level of diagnostic certainty, as level 1 in 61%, level 2 in 33%, level 3 in none, and level 4 in 6% of patients. Patients categorized in these levels did not differ with respect to proportion of patients with preceding events, initial clinical manifestations or outcome. The observed variability in the key diagnostic features of Guillain-Barré syndrome in the current cohort study, can be used to improve the sensitivity of the diagnostic criteria.

The issues addressed by the IWG in its first three decades of activism reflect global socio-political progress, as well as emergent new problems, for women, sport, and human rights. The IWG's commitment to collaboration with, among others, the International Olympic Committee and the United Nations has provided the foundations for globally accepted frameworks to address gender-based issues in and through sport. The advocacy work of the IWG is told via first-hand interviews with key personnel from each of the IWG Secretariats, from its establishment in 1994 to 2024, providing insight into the most significant issues, achievements, and outcomes for the international social movement for women and sport.

Previous projections of total joint replacement (TJR) volume have not quantified demand for TJR surgery in young patients (< 65 years old). We developed projections for demand of TJR for the young ...patient population in the United States. The Nationwide Inpatient Sample was used to identify primary and revision TJRs between 1993 and 2006, as a function of age, gender, race, and census region. Surgery prevalence was modeled using Poisson regression, allowing for different rates for each population subgroup over time. If the historical growth trajectory of joint replacement surgeries continues, demand for primary THA and TKA among patients less than 65 years old was projected to exceed 50% of THA and TKA patients of all ages by 2011 and 2016, respectively. Patients less than 65 years old were projected to exceed 50% of the revision TKA patient population by 2011. This study underscores the major contribution that young patients may play in the future demand for primary and revision TJR surgery.

This article is the first detailed academic analysis of the background, organisation, content, and immediate outcomes of the first World Conference on Women and Sport which took place in Brighton, ...UK, between 05 and 08 May 1994. Women and sport conferences are now commonplace in many parts of the world. Yet in the mid-1990s, Brighton was ground-breaking and the result of concerted activism for women and sport by agents who had participated in various advocacy organisations for decades. Titled 'Women, Sport, and the Challenge of Change', the Conference convened approximately 280 delegates, including representatives of major sporting and non-sporting organisations, based in over eighty countries. They contributed toward establishing a more coordinated and purposeful international strategy for women and sport. However, confusion and competition between existing organisations advocating for women and sport is apparent before and during Brighton. The Conference has also encountered criticism for Western ethnocentrism and liberalised political outcomes. This article contributes to understanding the galvanisation of a collective identity and politicisation of advocacy for women's sport, and the salience of conferences as sites enabling this. Archival document analysis and interviews with key agents involved with the advocacy were employed to understand the relations, politics and significance of the Conference.

This ...article discusses the phenomenon of Odessan cuisine both in Odessa and in Brighton Beach, Brooklyn, a Russian-speaking enclave where more than 10,000 ex-Soviet citizens, many of whom were Odessans. The aim of the study is to explore the migration of the Odessan culinary tradition, the gastropoetic aspect of Odessan food narration and the embodiment of the myth of Odessa in food discourse. The study analyses the websites, social media, the menus of two restaurants labelling themselves as Odessan, and the clients’ comments related to said restaurants. The gastropoetic aspect of Odessan culinary tradition is presented through examples from the literary works of Odessan authors.
