La protéine C réactive (CRP) sérique est un bon marqueur du retentissement général des infections urinaires de l'enfant, mais il existe des faux négatifs (taux bas au cours des pyélonéphrites), et ...cette mesure manque de pouvoir discriminant au plan individuel pour distinguer infections hautes et infections basses. L'interleukine 6 (IL6) sérique n'est pas un meilleur marqueur de gravité mais l'IL6 urinaire semble avoir une valeur pronostique puisqu'elle est corrélée à l'intensité des lésions rénales dues à l'infection, avec là aussi des faux négatifs. La procalcitonine sérique est un excellent marqueur pronostique au cours des pyélonéphrites car son taux initial est corrélé aux cicatrices rénales révélées par la scintigraphie avec un taux de faux négatifs inférieur à 10 %. Elle est bien adaptée à la pédiatrie en raison de sa facilité de dosage et de sa stabilité et doit être évaluée par des études supplémentaires.
Serum C reactive protein (CRP) remains a good marker of the severity of urinary tract infections in children, despite false negative results. Serum IL-6 is not a better marker: urinary IL-6 might have a better prognostic value as it is higher in patients with renal lesions due to infection, but low values are found in some cases. Serum procalcitonin levels are correlated with the importance of renal scars at scintigraphy, with less than 10% of false negative results. Further studies are needed to confirm the sensitivity and sensibility of these markers, especially for procalcitonin.
L'étude de la bandelette urinaire en aveugle de l'ECBU a été réalisée chez 100 patients (65 femmes, 35 hommes), âgés de 85,52 ± 6,28 ans, admis en court séjour gériatrique. L'infection urinaire était ...présente dans 19 % des cas. Les signes cliniques classiques (pollakiurie, dysurie, hématurie, brûlures mictionnelles) étaient rares avec une fréquence inférieure à 10 % ; en revanche, les signes généraux comme l'altération de l'état général (65 % des cas), ainsi que la perte d'autonomie (53 % des cas) prédominaient et leur caractère peu spécifique nécessitait un test fiable de dépistage de l'infection urinaire. Les bons résultats de la sensibilité (89,5 %), de la spécificité (98,7 %) des plages leucocytes/nitrites confirmaient la validité de la bandelette urinaire chez la personne âgée hospitalisée et la valeur prédictive négative de l'association leucocytes et nitrites (90,8 %) aurait permis de ne pas adresser 71 prélèvements au laboratoire pour ECBU, d'où un gain économique évident.
The blinded study of CBUE (cytobacteriological urine examination) test strips was carried out in 100 patients (65 women, 35 men), mean age 85.52 ± 6.28 years, admitted for a short stay in a geriatrics department. Urinary infection was present in 19% of the cases. Typical clinical signs (pollakiuria, dysuria, hematuria, burning upon voiding) were rare, less than 10%. General signs, such as deterioration of general status (65% of the cases) and loss of autonomy (53% of the cases), on the other hand, were predominant and their relatively unspecific nature required a reliable screening test for urinary infection. Good results of sensitivity (89.5%), specificity (98.7%), and the leukocytes/nitrites values, confirmed the validity of the urine test strip in hospitalized elderly patients. In addition, the negative predictive value of the combination leukocytes and nitrites (90.8%) would have prevented 71 samples from being sent to the laboratory for CBUE, resulting in an obvious financial savings.

Sur 7472 échantillons urinaires, 896 répondaient aux critères d’infection urinaire (12 % du total des prélèvements). Parmi les infections, 57,8 % provenaient des malades hospitalisés (sex-ratio F/H

1,16). Les microorganismes étaient surtout des entérobactéries (85 %, dont
Escherichia coli 44,7 %). Les Gram positifs (11,6 %) étaient dominés par
Staphylococcus saprophyticus (3,1 %). La fréquence élevée de la résistance des souches d’
E. coli à l’amoxicilline et au sulfaméthoxazole est élevée, alors que les quinolones et les aminosides conservent une bonne activité. La résistance aux céphalosporines de troisième génération par production de bétalactamase à spectre étendu était présente chez 3,2 % des entérobactéries.
The authors had for aim to evaluate the frequency of isolation and the antibiotic susceptibility of bacteria responsible for urinary infections among in and out-patients.
A retrospective study was carried out at the Microbiology Laboratory of the Mohammed-V Military Instruction Hospital in Rabat, on bacteria isolated from 896 positive cytobacteriologic examinations of urine, from January 1 to December 31, 2005.
Among the 7472 urinary samples, 896 answered the criteria of urinary infection (12%). Urinary infections were more frequent in in-patients (57.8%; sex-ratio F/M

1:16). Enterobacteria were the most frequent (85%) and especially
Escherichia coli (44.7%). Gram positive bacteria (11.6%) were more frequently
Staphylococcus saprophyticus (3.1%).
E. coli was highly resistant to amoxicillin, and sulfamethoxazole, whereas quinolones and aminosides maintained a good activity and third generation cephalosporines remain active (97% of susceptibility). Among the enterobacteria present, 3.2% were resistant to third generation cephalosporines with extended spectrum betalactamase.
Cet essai en double-aveugle visait à comparer, dans une approche probabiliste, la ceftazidime à la posologie initiale de 2 g/jour et la ciprofloxacine 2 × 200 mg/jour dans les infections urinaires ...nosocomiales post-chirurgie des voies urinaires. Les patients adultes devaient présenter des signes d'infections urinaires post-chirurgicales, une température supérieure à 38°5 associée à une pyurie ou à une bactériurie supérieure à 10
5/ml. La durée de traitement était de 4 à 10 jours avec une évaluation à J2, J5 et à la fin du traitement qui prenait en compte l'examen cytobactériologique des urines, l'antibiogramme, et l'état clinique des patients. Le traitement devait être poursuivi 72 heures après la disparition des signes cliniques. Soixante-sept patients ont été inclus dans l'étude. Tous avaient subi une intervention sur les voies urinaires. Treize patients ne présentant pas, lors de la relecture par le coordinateur national, de critère d'infection urinaire, n'ont pas été retenus pour l'analyse. Les germes le plus souvent rencontrés à J0 étaient
Escherichia coli (39 %),
Pseudomonas aeruginosa (19 %), et
Staphylococcus sp (11 %). Sur les 54 patients évaluables, 78 % et 81 % étaient guéris en fin de traitement et 85 % et 87 % des germes éradiqués à J5, respectivement dans les groupes ceftazidime et ciprofloxacine. La durée moyenne de traitement a été d'un peu moins de six jours dans chaque groupe. Aucun événement indésirable lié au traitement n'a été signalé. Aucune différence n'a été mise en évidence entre les deux traitements, tant sur l'efficacité clinique et bactériologique que sur la tolérance et la durée de traitement. L'approche probabiliste du traitement des infections sévères post-opératoires des voies urinaires s'est révélée adaptée au choix de la ceftazidime ou de la ciprofloxacine.
This double-blind study compared ceftazidime 1g twice a day and ciprofloxacine 200 mg twice a day in the empiric treatment of nosocomial urinary infections following urinary tract surgery. Adult patients should have post surgical urinary infection signs, central temperature above 38°5C associated with a pyuria or with a bacteriuria above 10
5/ml. The treatment duration was from 4 to 10 days with an evaluation at D2, D5, and at the end of the treatment. Cytobacteriological exam, antibiotic susceptibility testing and clinical examination were taken into account. Treatments should have been continued 72 hours after the disappearance of the clinical signs. Sixty-seven patients were included. They all underwent surgery on urinary tract. Thirteen patients without urinary tract infection criteria, from national coordinator point of view, were not eligible for analysis. The most common isolated bacteria at D0 were
Escherichia coli (39 %),
Pseudomonas aeruginosa (19 %), and
Staphylococcus sp (11 %). Among the 54 eligible patients, 78 % and 81 % were cured respectively in the ceftazidime and ciprofloxacine groups, and respectively 85 % and 87 % of the germs were eradicated at D5. The average treatment duration was less than six days in each group. No drug-related adverse event was notified. There was no significant difference between the two treatments on clinical and bacteriological efficacy, tolerance, and treatment duration. The empiric treatment was proved to be suitable for the choice of ceftazidime or ciprofloxacine in the treatment of severe urinary infections following urinary tract surgery.

A descriptive study was conducted of urological patients with CU-positive urine cultures (January 2009-December 2014). We calculated the annual distribution and clinical characteristics of infection by CU and OU. Minimum follow-up: 6 months. We obtained the statistical means and ranges of clinical parameters pre/post-therapy.
The total number of patients with CU was 115 (men, 87; women, 28). The mean age was 67.9 years (range, 6-95 years), and the annual distribution of cases for 2009, 2010, 2011, 2012, 2013 and 2014 was 9 (7.8%), 13 (11.3%), 9 (7.8%), 20 (17.4%), 31 (27%) and 33 (28.7%), respectively. The increase in cases for 2009-2014 was 300%. Multiple urological surgeries were performed in 89 cases (77.3%), with surgical complications in 77 cases (66.9%). Eighteen (15.6%) patients had OU (men, 13; women, 5), 12 had pyelitis (66.7%), 3 had cystopathy (16.6%), 2 had prostatic capsule disease (11.2%) and 1 had mesh calcification (5.5%). The analysis of the 18 cases with OU showed pre/postantibiotic therapy urine pHs of 8 (r, 6-9) vs. 6 (r, 5-7). All postantibiotic cultures were negative. Acidifying solution was applied in 5 cases, and surgery was performed in 13 cases (72.2%). The results from before/after the multimodal therapy showed renal impairment in 12 (66.6%) vs. 9 cases (50%) and glomerular filtration rates (GFR) of 45.8 (r, 6->90) vs. 52.7 (r, 13->90). The improvement in GFR was 6.94 points (T Wilcoxon; P=.102). The radiology results (incrustations) showed improvement in 13 patients (72.2%) and no change in 5 (27.8%). There was no specific mortality for CU.
The prevalence of infection by CU and OU is increasing. Antibiotic treatment is highly effective. Acidifying solutions are an acceptable option for reducing calcifications.

Les infections urinaires nosocomiales chez les patients de réanimation porteurs d'une sonde vésicale sont fréquentes et méconnues. Les données de la littérature sur leurs mécanismes et ...leur prévention sont abondantes et hétérogènes. L'objectif de cette mise au point est de présenter l'état de l'art sur cette question.

Synthèse des données :
La pathogénie des infections urinaires est insuffisamment élucidée. L'instillation d'antimicrobiens dans le système de drainage, la modification des composants de la sonde, l'antibioprophylaxie et le système de drainage clos sont des mesures efficaces, admises dans la pratique quotidienne pour la prévention de ces infections. Leur efficacité en unité de soins intensifs n'a pas été réellement évaluée. Le traitement de ces infections reste mal codifié et mériterait un travail prospectif.
Conclusion :
L'infection urinaire nosocomiale chez le patient ayant une sonde vésicale est le reflet d'une politique générale d'hygiène, allant des soins infirmiers lors de la pose de la sonde jusqu'à la gestion rigoureuse de l'écologie du service.
Nosocomial catheter-associated urinary tract infections in the intensive therapy unit: physiopathology, epidemiology and prophylaxis.
Objectives:
Nosocomial urinary tract infections associated with bladders catheters are common and poorly understood. Data on the prevention of urinary tract infections are numerous and heterogenous. This update article aimed at analysing mechanisms, epidemiology and prevention of these infections.

We searched in the Medline® database for articles in English or French, without limiting date of publication, using the following key words separely or in combination: urinary tract infection, nosocomial, catheter, infection urinaire, sonde urinaire.

We considered all categories of articles.
Data extraction:
Data on prevention of nosocomial urinary tract infections were analysed in depth.
Data synthesis:
The data on pathogenesis of nosocomial urinary tract infections are still controversial. Various means for preventing urinary tract infections have been recommended : addition of antibacterial agents to urinary drainage system, inclusion of antimicrobial components into the catheter itself, antibiotic prophylaxis or closed sterile drainage system. Their efficiency in intensive therapy unit has not yet been fully assessed. The therapy of these infections is still under debate and requires additional prospective studies to establish the optimal management.
Conclusion:
Catheter-associated urinary tract infections reflect the general hygiene policy, starting with nurse practice patterns at catheter insertion, and ending with antibiotherapy prescriptions by medical staff.
