Um zu evaluieren, ob das neue Einsatzmittel Gemeindenotfallsanitäter (G-NFS) den Rettungsdienst bei der Versorgung niedrigprioritärer Notfälle entlastet, wurden im Rahmen eines ...Innovationsfondsprojekts auch die hausärztliche und die Patient*innenperspektive mittels schriftlicher Befragung erhoben. Die Rekrutierung zur Studienteilnahme gestaltete sich jedoch schwierig. Mit dieser Studie soll untersucht werden, warum sich die G-NFS gegen eine Aufklärung zur Studienteilnahme entschieden und welche Maßnahmen erforderlich wären, um zukünftig mehr Notfallpatient*innen in Erhebungen einschließen zu können.
Retrospektive Analyse der im Erhebungszeitraum 01.04.2021 bis 30.06.2022 von den G-NFS dokumentierten Angaben zu Patientencharakteristika, durchgeführten Maßnahmen und Empfehlungen sowie zu den Gründen für die Ablehnung zur Studienteilnahme.
Es konnten 5.395 G-NFS-Einsatzprotokolle, die Angaben zur Nicht-Teilnahmefähigkeit beinhalteten, in die Auswertung eingeschlossen werden. Das Durchschnittsalter der Patient*innen betrug 62,4 Jahre (SD 22,7), und 50,2% waren weiblich. Als nicht dringlich zu versorgen wurden 57,4% der Fälle kategorisiert, die Nachalarmierung eines weiteren Rettungsmittels erfolgte in 35,2% der Fälle. Bei 404 (7,5%) Fällen lag eine wiederholte Inanspruchnahme und bei 1.120 (20,8%) Fällen Fremdsprachlichkeit vor, 1.012 (18,8%) Fälle lehnten die Studienteilnahme ab und 2.975 (55,1%) Fälle waren aus Sicht der G-NFS nicht teilnahmefähig. Als Gründe für die Nicht-Teilnahmefähigkeit aus Sicht der G-NFS wurden neurokognitive Einschränkungen (35%), Akut-/Notfall (26,5%), Beeinträchtigungen der psychischen Gesundheit (10,3%) und Substanzmissbrauch (6,5%) beschrieben.
Die Ergebnisse zeigen, dass aus Sicht der G-NFS über die Hälfte der Patient*innen aus unterschiedlichen Gründen nicht in der Lage war, an einer schriftlichen Befragung teilzunehmen, obwohl keine Behandlungsdringlichkeit vorlag. Ein hoher Versorgungsbedarf und das komplexe Einwilligungsverfahren könnten dafür ursächlich sein. Zusätzlich sind weitere Ressourcen für eine bedarfsgerechte Versorgung dieser Patient*innen erforderlich, um die Notfallversorgung zu entlasten. Weitere Untersuchungen sind notwendig, um festzustellen, welche Einwilligungsverfahren geeignet sind, um diesen Patient*innen die Teilnahme an Studien zu erleichtern.
In order to evaluate whether the new rescue means “community emergency paramedics” (Gemeindenotfallsanitäter G-NFS) relieves the emergency medical service (EMS) in the care of low-priority emergencies, the perspective of general practitioners and patients was also surveyed in a written questionnaire as part of an innovation fund project. Recruitment for participation in the study proved to be difficult. The aim of this study is to evaluate why the G-NFS decided against providing information on study participation and what measures would be necessary to include more emergency patients in surveys in the future.
Retrospective analysis of the assignment protocols from April 1, 2021 to June 30, 2022. In addition to patient characteristics, data on treatments, interventions and recommendations to patients as well as reasons for non-participation in the patient survey were collected.
5,395 G-NFS protocols that contained information on non-participation were included in the analysis. The average age of the patients was 62.4 years (SD 22.7), and 50.2% were female. 57.4% of the cases were categorised as non-urgent, and 35.2% of the cases required an additional ambulance to be alerted. 404 (7.5%) patients used the EMS more than once, 1,120 (20.8%) did not have sufficient language skills, 1,012 (18.8%) patients declined study participation, and 2,975 (55.1%) patients were not able to participate according to the G-NFS assessment. Dementia/neurocognitive impairment (35%), acute/emergency situation (26.5%), mental health impairment (10.3%), and substance abuse (6.5%) were given as reasons for non-participation from the G-NFS perspective.
The results show that more than half of the patients were unable to take part in a written survey for various reasons, even though there was no need for urgent care. This could be due to a high demand for care and the complex consent procedure. In addition, further resources are required to provide needs-based care for these patients in order to relieve the burden on emergency medical care. Gait impairments in persons with multiple sclerosis (pwMS) leading to decreased ambulation and reduced walking endurance remain poorly understood. Our objective was to assess gait asymmetry (GA) and ...bilateral coordination of gait (BCG), among pwMS during the six-minute walk test (6MWT), and determine their association with disease severity. We recruited 92 pwMS (age: 46.6 ± 7.9; 83% females) with a range of clinical disability, who completed the 6MWT wearing gait analysis system. GA was assessed by comparing left and right swing times, and BCG was assessed by the phase coordination index (PCI). Several functional and subjective gait assessments were performed. Results show that gait is more asymmetric and less coordinated as the disease progresses (p < 0.0001). Participants with mild MS showed significantly better BCG as reflected by lower PCI values in comparison to the other two MS severity groups (severe: p = 0.001, moderate: p = 0.02). GA and PCI also deteriorated significantly each minute during the 6MWT (p < 0.0001). GA and PCI (i.e., BCG) show weaker associations with clinical MS status than associations observed between functional and subjective gait assessments and MS status. Similar to other neurological cohorts, GA and PCI may be important parameters to assess and target in interventions among pwMS.
Ambulatory care is one of the fastest growing sections of the healthcare industry and yet little research has been done on challenges that can exist within preoperative care for ambulatory surgery. ...The research that does exist is typically only pilot studies. This paper will explore three overarching challenges that have appeared in preoperative care: Team performance, patient absences, and record errors. This paper will provide a description of these challenges and provide recommendations for further research.
Abstract
Lumbar spinal stenosis is the most common reason for spine surgery in older adults, but the effects of prehabilitation on perioperative outcomes among these patients have not been ...investigated. This study aims to evaluate the effectiveness of a preoperative exercise-based intervention program compared with usual care on the improvement of clinical status, physical capacities and postoperative recovery of patients awaiting surgery for lumbar spinal stenosis. Sixty-eight participants were randomised to receive either a 6-week supervised exercise-based prehabilitation program or hospital usual care. The outcomes included both clinical and physical measures. Data collection occurred at post-intervention, and 6 weeks, 3- and 6-months post-surgery. Significant but small improvements were found in favour of the experimental group at the post-intervention assessment for pain intensity, lumbar spinal stenosis-related disability, lumbar strength in flexion, low back extensor muscles endurance, total ambulation time, and sit to stand performance. A significant difference in favor of the intervention group was found starting at the 3-month postoperative follow-up for low back-related disability. No adverse events were reported. Exercise-based prehabilitation did not improve short-term postoperative recovery in patients with lumbar spinal stenosis.

Powered exoskeletons promise to increase the quality of life of people with lower-body paralysis or weakened legs by assisting or restoring legged mobility while providing health benefits across ...multiple physiological systems. Here, a systematic review of the literature on powered exoskeletons addressed critical questions: What is the current evidence of clinical efficacy for lower-limb powered exoskeletons? What are the benefits and risks for individuals with spinal cord injury (SCI)? What are the levels of injury considered in such studies? What are their outcome measures? What are the opportunities for the next generation exoskeletons?
A systematic search of online databases was performed to identify clinical trials and safety or efficacy studies with lower-limb powered exoskeletons for individuals with SCI. Twenty-two studies with eight powered exoskeletons thus selected, were analyzed based on the protocol design, subject demographics, study duration, and primary/secondary outcome measures for assessing exoskeleton's performance in SCI subjects.
Findings show that the level of injury varies across studies, with T10 injuries being represented in 45.4% of the studies. A categorical breakdown of outcome measures revealed 63% of these measures were gait and ambulation related, followed by energy expenditure (16%), physiological improvements (13%), and usability and comfort (8%). Moreover, outcome measures varied across studies, and none had measures spanning every category, making comparisons difficult.
This review of the literature shows that a majority of current studies focus on thoracic level injury as well as there is an emphasis on ambulatory-related primary outcome measures. Future research should: 1) develop criteria for optimal selection and training of patients most likely to benefit from this technology, 2) design multimodal gait intention detection systems that engage and empower the user, 3) develop real-time monitoring and diagnostic capabilities, and 4) adopt comprehensive metrics for assessing safety, benefits, and usability.

Antibiotikaresistenzen stellen ein zunehmendes Problem dar – Folge eines unkritischen Einsatzes insbesondere von Reserveantibiotika. Ansätze zum rationaleren ...Antibiotikagebrauch fokussierten bislang auf den stationären Sektor, obwohl der überwiegende Verbrauch im ambulanten Bereich stattfindet. Für diesen Bereich gibt es daher einen Bedarf an innovativen Ansätzen. Das 2016 in Bielefeld von ambulant tätigen Kinder- und Jugendärzten gegründete Projekt „AnTiB“ entwickelte auf lokaler Ebene entsprechende Strukturen und Maßnahmen in der ambulanten Pädiatrie.
Ziele
Die Vorgehensweise bei der Erarbeitung praxistauglicher lokaler Antibiotikaempfehlungen für häufige Infektionskrankheiten im pädiatrischen Alltag soll beschrieben werden.
Material und Methoden
Zur Erarbeitung der Empfehlungen wurde Evidenz aus Literatur und Leitlinien zusammengetragen. In einem strukturierten Prozess v. a. via Qualitätszirkel wurden die Inhalte vor Ort konsentiert und mit Hilfe von infektiologischen Experten validiert. Zusammen mit der Kassenärztlichen Vereinigung Westfalen-Lippe (KVWL) wurden exemplarisch Verordnungsdaten aus 2016 erhoben.
Ergebnisse
Die pädiatrischen Empfehlungen wurden im Januar 2017 in der 1. Version veröffentlicht. Inzwischen entstanden auch Empfehlungen für Frauenheilkunde und Allgemeinmedizin. Die für 2016 ausgewerteten Verordnungsdaten zeigen eine große Spannbreite der Antibiotika-Verordnungsprävalenz insgesamt sowie ausgewählter Wirkstoffgruppen.
Diskussion
Lokal erarbeitete und konsentierte Handlungsempfehlungen können zu einer lokalen „Antibiotika-Verordnungskultur“ beitragen. Die pädiatrischen Empfehlungen genießen vor Ort eine hohe Reichweite und Akzeptanz. Eine Ausweitung auf andere Regionen und weitere Fachgruppen erscheint möglich und sinnvoll. Die Effekte auf das Verordnungsverhalten werden zukünftig durch den Vergleich von Verordnungsdaten messbar.

Belastung auf die Höhe und die Zusammensetzung der Krankheitskosten
(ambulante Kosten, Krankenhauskosten, ...Rehabilitationskosten, Arzneimittelkosten)
im Zeitverlauf zu überprüfen.
Methodik
Im Längsschnittdesign untersuchten wir die psychische
Belastung von 3.287 Studienteilnehmern der erwachsenen
Allgemeinbevölkerung anhand der Hospital Anxiety and Depression Scale
(HADS) und ihren Effekt auf die Höhe und die Zusammensetzung der
Krankheitskosten aufgrund von psychischen Erkrankungen im Jahr der Befragung t0
und den beiden Folgejahren t1 {t2}.
Ergebnisse
Im Vergleich zur Referenzgruppe, die keine psychischen
Belastungen aufweist, waren die Krankheitskosten in der Gruppe mit geringen
psychischen Belastungen um den Faktor 2,0 2,2 {1,5}, bei jenen mit moderater
psychischer Belastung um den Faktor 3,7 4,2 {3,1} und bei jenen mit schweren
psychischen Belastungen um den Faktor 7,5 9,0 {5,2} signifikant
erhöht. Im Zeitverlauf zeigten sich signifikante Effekte auf die
Krankheitskosten nur an den beiden äußeren Rändern der
psychischen Belastungsgrade (keine und schwere psychische Belastungen) mit einer
Tendenz zur Mitte. Mit zunehmendem Grad psychischer Belastung dominierten
Krankenhauskosten die Gesamtkosten aufgrund psychischer Belastung und der Anteil
der ambulanten Krankheitskosten an den Gesamtkosten nahm ab.
Schlussfolgerung
Mit dem Grad psychischer Belastung steigen die
Krankheitskosten steil an. Die Progredienz psychischer Erkrankungen zu
verhindern, ist deshalb nicht nur aus individueller, sondern auch aus
gesundheitsökonomischer Perspektive bedeutsam.

A consequence of a complete spinal cord injury (SCI) is the loss of gait capacity. Wearable exoskeletons for the lower extremity enable household and community ambulation in people with SCI. This ...study assessed the amount, purpose, and location of exoskeleton use in the home and community environment, without any restrictions. The number of steps taken was read from the exoskeleton software. Participants kept a daily logbook, and completed two user experience questionnaires (Quebec User Evaluation of Satisfaction with assistive Technology (D-QUEST) and System Usability Scale (SUS)). Fourteen people with a complete SCI used the ReWalk exoskeleton a median of 9 (range 1-15) out of 16 (12-21) days, in which participants took a median of 3,226 (330-28,882) steps. The exoskeleton was mostly used for exercise purposes (74%) and social interaction (20%). The main location of use was outdoors (48%). Overall, participants were satisfied with the exoskeleton (D-QUEST 3.7 ± 0.4) and its usability (SUS 72.5 52.5-95.0). Participants with complete SCI report satisfaction with the exoskeleton for exercise and social interaction in the home and community, but report limitations as an assistive device during daily life.
