Ausgangssituation der Studie waren die mit Erklärung der Ausbreitung des COVID-19 Virus zur Pandemie im März 2020 durch die WHO und die in diesem Zusammenhang eingeleiteten Maßnahmen der körperlichen ...und sozialen Distanzierung. Die Studie analysiert mögliche subjektive Auswirkungen dieser Maßnahmen für ältere Menschen, die aufgrund von Krankheit und Pflegebedürftigkeit in ihrer Selbstständigkeit eingeschränkt waren. Schwerpunkte bilden in diesem Zusammenhang die Erfassung der subjektiven Gesundheit, des Kohärenzgefühls und des Wohlbefindens. Mittels standardisierter Verfahren wurden N=146 Klient_innen ambulanter Pflegedienste im Alter von 60 Jahren und älter vor und während dieser Einschränkungen in die Studie einbezogen. Die Ergebnisse lassen darauf schließen, dass ältere Menschen, die mit ihrem Partner zusammenleben, ein vergleichsweise niedriges Wohlbefinden aufweisen. Die meisten Befragten informieren sich ausschließlich über das Fernsehen. Nur wenige Befragte äußern negative Auswirkungen auf ihre subjektive Gesundheit. Schlussfolgern lässt sich, dass ambulant Pflegende sensibilisiert werden sollten, insbesondere die Aspekte, die sich negativ auf das Wohlbefinden ihrer Klientinnen und Klienten auswirken im Rahmen der Pflege systematisch zu berücksichtigen. Der Einsatz sozialer Medien z. B. zur ergänzenden Informationsvermittlung und Erweiterung sozialer Kontakte stellt bei dieser Personengruppe keine geeignete Maßnahme dar.
The background to the study presented here is the measures introduced with the declaration of the spread of the COVID-19 virus as a pandemic in March 2020, which were accompanied by physical and social distancing and changes in daily routines, as well as possible subjective effects for older people whose independence is restricted due to illness and need for care. In this context, the focus is on the recording of subjective health, the sense of coherence and well-being. Using standardised procedures, N=146 clients of outpatient care services aged 60 years and older were included in the study before and during these restrictions. The results suggest that older people living with their partner have a comparatively low sense of well-being. Most respondents inform themselves exclusively through television. Only a few respondents express negative effects on their subjective health. It can be concluded that outpatient caregivers should be sensitised to systematically consider in particular the aspects that have a negative impact on the well-being of their clients in the context of care. The use of social media, e.g. to provide additional information and expand social contacts, is not a suitable measure for this group of people.
In diesem Open-Access-Buch werden die politischen Ziele der Ambulantisierung und der sektorengleichen Vergütung den aktuellen Herausforderungen in der Chirurgie gegenübergestellt. Die unzureichend an ...die steigenden Kosten angepasste Vergütung spiegelt sich in einem signifikanten Rückgang des Reinertrags chirurgischer Einzelpraxen wider. Als Folge sinkt die Zahl ambulanter Operationen (AOPs). Die Ergebnisse der vorliegenden Studie zeigen, dass die Besondere Versorgung den Herausforderungen begegnen, die Ergebnisqualität verbessern und Sektorengrenzen aufbrechen sowie die Ambulantisierung bei geringerer Einflussnahme auf medizinische Entscheidungen und deutlich höherer Vergütung ausbauen kann. Als Handlungsempfehlung resultiert ein Ersatz der auf Soll-Werten basierenden Einzelleistungsvergütungen durch modifizierte Hybrid-DRGs mit regelmäßig aufwandsorientierten Vergütungsanpassungen für Krankenhäuser und Vertragsärzte mit/ohne Krankenhausanbindung. Findet eine Umsetzung in der Regelversorgung nicht statt und bestehen weiterhin keine finanziellen Anreize, können laufende Projekte der Besonderen Versorgung weiter ausgebaut werden.
Abstract
Economic theory predicts that outsourcing public services to private firms reduces costs, but the effect on quality is ambiguous. We explore quality differences between publicly and ...privately owned ambulances in Stockholm County, Sweden, a setting where patients are as good as randomly assigned to ambulances with different ownership status. We find that private ambulances reduce costs and perform better on contracted measures such as response time, but perform worse on noncontracted measures such as mortality. In fact, a patient has a 1.4% higher risk of death within three years if a private ambulance is dispatched (in aggregate, 420 more deaths each year). We also present evidence of the mechanism at work, suggesting that private firms cut costs at the expense of ambulance staff quality.

Primary care is a notable example of a service industry where capacity‐constrained suppliers face fluctuating demand levels. To meet this challenge, physicians trade off their time with patients with ...other inputs such as lab tests and referrals. We study this tradeoff using administrative data from a large Israeli HMO where the absence of colleagues generates exogenous variation in physician workload. We motivate and estimate a range of specifications, from a classic exclusion restriction within a linear model to non‐parametric, partially identified models. The results suggest that diagnostic inputs are unlikely to properly compensate for a decline in time spent with patients.
Objective
Depression is a global key challenge in mental health care. The implementation of effective, low‐risk and cost‐effective interventions to reduce its disease burden is a necessity. The aim ...of this study was to investigate the efficacy of the new Meditation‐Based Lifestyle Modification (MBLM) program, a “second‐generation” mindfulness‐based intervention, in depressive outpatients.
Methods
Eighty‐one patients with mild to moderate depression were randomized into three groups: intervention group (MBLM), control group (CONTROL), and treatment as usual group (TAU). The primary outcome was the change of depressive symptoms as administered by the Beck Depression Inventory‐II (BDI‐II) after 4 and 8 weeks. Secondary outcome variables included the Brief Symptom Checklist‐18 and the Perceived Stress Scale‐10. A 6‐month follow‐up was conducted.

Conclusions
Study results showed beneficial effects of MBLM in depressed outpatients. Further high‐quality controlled clinical studies including qualitative research are needed to investigate the specific and unspecific effects of the MBLM program in depression and other medical conditions.
Con el objetivo de identificar la labor educadora en materia de salud realizada por las divulgadoras sanitario-sociales de las Cátedras Ambulantes de la Sección Femenina en la provincia de Huelva ...durante el periodo de 1956 a 1977, se ha realizado este estudio histórico, consultando fuentes primarias encontradas en el Archivo Histórico Provincial de Huelva. A través del Programa de Puericultura e Higiene, el curso de Madres Ejemplares, el curso de Socorrismo y otras acciones específicas se pretendió mejorar la Higiene y la Salud Pública en los 61 municipios donde la cátedra llegó, alcanzando con sus actuaciones a gran parte de la población de estas zonas rurales. Las principales destinatarias fueron las mujeres, pero los hombres también participaron en esta formación, recibiendo cursos de Higiene. La divulgadora de la Cátedra Ambulante contó con el apoyo de otros agentes sanitarios en su labor educadora, principalmente, el médico de la localidad, el practicante, o personal sanitario de empresas de la zona. La elevada asistencia y motivación por el programa de Educación para la Salud estuvo impulsado por la aplicabilidad del aprendizaje en lo cotidiano para satisfacer la necesidad de mejorar el estado de salud en un medio con escasos recursos.

Background and objectives: Posttraumatic stress disorder (PTSD) related to childhood abuse (CA) is associated with high symptom complexity. This study examined the efficacy of Imagery Rescripting ...(ImRs) as a stand-alone treatment versus a sequenced approach with Skills training in Affective and Interpersonal Regulation (STAIR) followed by ImRs for CA-related PTSD. Methods: Outpatients of two mental health clinics with CA-related PTSD (N = 61) were randomly assigned to ImRs (16 sessions; n = 21), STAIR/ImRs (8 STAIR-sessions followed by 16 ImRs-sessions; n = 20), or Waitlist (8 weeks; n = 20). Patients of the waitlist condition were also randomized to the two active conditions for comparison of STAIR/ImRs (total n for this condition = 31) and ImRs (total n for this condition = 30) and started treatment after waitlist completion. Assessments took place at pre-treatment, after each treatment phase and at 12-week post-intervention follow-up. PTSD symptoms and diagnosis were primary outcome measures, and depression, emotion regulation and interpersonal functioning were secondary outcomes. Results: ImRs showed greater reduction of PTSD severity (effect sizes ES 1.40–1.63) than STAIR (ES, 0.23–0.33) as compared to waitlist. When comparing STAIR/ImRs and ImRs directly, (i.e. including re-randomized Waitlist-patients), PTSD symptoms reduced significantly (within condition ES, 1.64–2.10) and improved further to 12-week follow-up (within-condition ES, 2.33–2.66), with no significant difference between both conditions (between-condition ES, 0.21–0.45). Loss of PTSD diagnosis was achieved by 70% in the ImRs condition and 86% in the STAIR/ImRs condition. Limitations: The sample size was relatively small. Conclusions: Results show that ImRs is an effective treatment for CA-related PTSD, whereby the current data do not convincingly show an additive effect of STAIR.
Die Versorgung in der eigenen häuslichen Umgebung ist für viele pflegebedürftige Menschen von großer Bedeutung. Neben der Pflege durch Angehörige spielt dabei die Unterstützung durch ambulante ...Pflegedienste eine wichtige Rolle. Zunehmend mehr pflegebedürftige Menschen werden von den Angehörigen zusammen mit oder alleinig durch einen ambulanten Pflegedienst versorgt. Vor diesem Hintergrund gewinnt das Thema der Qualität und Qualitätsmessung in der ambulanten Pflege an Bedeutung. Voraussetzung für die Messung und Beurteilung von Qualität ist die Definition von spezifischen Anforderungen an die zu erbringende medizinisch-pflegerische Versorgung. Ein umfassender Qualitätsrahmen für die ambulante Pflege wurde bisher noch nicht entwickelt. Ziel der vorliegenden Übersichtsarbeit war die systematische Aufbereitung der aktuellen Versorgungssituation in der ambulanten Pflege und darauf aufbauend die Identifikation von Versorgungsaspekten, die für die Qualität in der ambulanten Pflege relevant sind. Diese Versorgungsaspekte sollten im Anschluss in einem Rahmenmodell dargestellt werden.
Zur Aufbereitung der Versorgungssituation in der ambulanten Pflege wurde in Anlehnung an das methodische Vorgehen zur Durchführung von Scoping Reviews des Joanna Briggs Instituts eine Literaturübersicht erstellt. Daraufhin erfolgte die Analyse der vorliegenden Literatur sowie in einem induktiven Vorgehen die Ableitung der relevanten Versorgungsaspekte. Darüber hinaus erfolgte eine Zuordnung dieser Versorgungsaspekte zu, für die Beurteilung der Qualität der Gesundheitsversorgung, definierten Qualitätsdimensionen (z. B. Patientensicherheit, Ausrichtung der Versorgungsgestaltung an den Patientinnen und Patienten) und die Zusammenführung zu einem Qualitätsmodell für die ambulante Pflege.
Insgesamt wurden 222 Publikationen im Rahmen der Literaturrecherche eingeschlossen. Daraus konnten insgesamt 17 Versorgungsaspekte für die ambulante Pflege abgeleitet werden. Identifiziert wurden strukturbezogene Versorgungsaspekte und Versorgungsaspekte zur Beziehungsgestaltung mit sowie zur Unterstützung von pflegebedürftigen Menschen und pflegenden Angehörigen. Des Weiteren wurden prozessbezogene und ergebnisbezogene Versorgungsaspekte identifiziert. Die anschließende Zuordnung dieser Versorgungsaspekte zu den für eine Qualitätsbeurteilung der Versorgung definierten Dimensionen zeigte, dass jede dieser Qualitätsdimension mindestens von einem Versorgungsaspekt abgedeckt wird. Im Ergebnis liegt somit ein umfassendes Qualitätsmodell für die ambulante Pflege vor, das als Rahmen für die Messung und Beurteilung der Qualität in der ambulanten Pflege dienen kann.
Die identifizierten Versorgungsaspekte können als Ausgangspunkt für die Entwicklung von Instrumenten (z. B. routinedatenbasierte Qualitätsindikatoren) dienen und somit die zielgerichtete Messung, Darstellung und Beurteilung der Qualität in der ambulanten Pflege ermöglichen.
Being cared for in one's own home environment is of great significance for many people in need of care. In addition to care provided by relatives, the care provided by professional home care services is also important. More and more people in need of care are being cared for by relatives together with or solely through such home care services. Because of this, the issue of quality and quality measurement in home care has become increasingly important. The prerequisite for measuring and assessing quality is the definition of specific requirements for medical and nursing care. A comprehensive quality framework for home care has not yet been developed. The aim of this review is to provide an overview of the current situation in home care and to identify relevant aspects of care subsequently. Finally, these aspects of care should be presented in a framework model.
In order to describe the situation in home care, a review was conducted based on the Joanna Briggs Institute methodology of a scoping review. The literature was analysed, and the relevant aspects of care were derived by using an inductive approach. Furthermore, these aspects of care were assigned to quality dimensions defined for health care (e. g., patient safety, patient-centeredness) and merged into a quality model of home care.
A total of 222 publications were included, and 17 aspects of home care were identified. These involved structural aspects of care and aspects of care for shaping relationships with people in need of care and their family carers, as well as aspects of support for people in need of care and their family carers. Furthermore, process-related and outcome-related aspects of care were identified. The comparison of these aspects with the defined quality dimensions showed that each of these quality dimensions is covered by at least one aspect of care. This results in a comprehensive quality model for home care which can serve as a framework for measuring and assessing quality in home care.
The aspects of care identified can be used for the development of instruments such as routine data-based quality indicators and thus allow for the measurement, presentation and assessment of quality in home care.
