Jegliche Veränderungen im Inanspruchnahmeverhalten bei der Wahl der Art der Pflege von einer günstigeren hin zu einer teureren Versorgungsform gehen mit Auswirkungen auf die zukünftige ...Ausgabenentwicklung der sozialen Pflegeversicherung einher. Dabei ist ein verändertes Inanspruchnahmeverhalten bereits das Resultat der demographischen Entwicklung, möglicherweise verschärft durch sich verändernde Haushalts- und Familienstrukturen sowie eine zunehmende Erwerbsbeteiligung der Frauen. In welchem Ausmaß Veränderungen in den Pflegearrangements mit Ausgabensteigerungen einhergehen, soll im Rahmen dieses Beitrags untersucht werden.
Purpose: Oral agents for cancer treatment are increasingly prescribed due to their benefits. However, oral cancer medications are difficult to handle and have a considerable potential for side ...effects. This type of therapy requires a high level of self-management competence by the patient. A standardized patient education program provided by physicians and oncology nurses may positively influence the handling of oral agents. The aim of the study was to evaluate the impact of a standardized patient education program provided by specially trained oncology nurses on therapy management regarding side effects and unplanned therapy interruptions. Methods: One hundred sixty-five patients from 28 office-based oncology practices from all over Germany participated in this cluster-randomized controlled study. Patients of both intervention (n = 111) and standard care groups (n = 54) received the usual oncologist counseling; in addition, the patients from the intervention group (k = 17 practices) received an education from specially trained oncology nurses. The time of observation was 3 months per patient. Results: The patients of the intervention group reported fewer side effects (skin rash, pain, fatigue, nausea, vomiting). Patients in the standard care group interrupted the therapy more frequently without informing their oncologist, compared to the intervention group. Conclusions: Patients benefit from a standardized patient education program provided by specially trained oncology nurses. They tend to handle side effects and critical situations better.
OBJETIVO: Conocer edad, sexo, desplazamiento, escolaridad, actividad desarrollada, jornada laboral, vivienda en calle y cobertura de seguridad social de un grupo de niños que laboran en las calles de ...las capitales latinoamericanas. MATERIAL Y MÉTODOS: Estudio de corte transversal. Se aplicó un cuestionario a 972 niños que trabajan en las calles de Bogotá, Ciudad de Guatemala, Ciudad de México, Quito y San Salvador; RESULTADOS: El 63.3% era de sexo masculino; 39% provenía de familias desplazadas; 18% habitaba en la calle; 62% trabajaba más de 40 horas a la semana; 19% tenía seguridad social, y 32% se dedicaba al comercio ambulante. Se encontró que la mayoría de las variables se comportan de forma significativamente diferente para cada ciudad. CONCLUSIÓN: El trabajo infantil ambulante es una actividad peligrosa, de largas jornadas laborales, con exposición a múltiples factores de riesgo, y se comporta de manera significativamente diferente en cada ciudad estudiada. Esto sugiere que las soluciones al problema deben diseñarse caso por caso.OBJECTIVE: To identify the age, sex, mobility, education, work activity, working hours, street dwelling, and social security coverage in a group of children working in the streets in capital cities in Latin America. MATERIAL AND METHODS: Cross sectional study. A questionnaire was applied to 972 children working in the streets of Bogotá, Guatemala City, Mexico City, Quito and San Salvador. RESULTS: A total of 63.3% subjects were boys; 39% were children from displaced families; 18% lived in the streets; 62% worked more than 40 hours per week; 19% were covered by the social security system, and 32% were street vendors. The behavior of variables differed significantly by city. CONCLUSION: Child labor in the streets is a dangerous activity characterized by long working hours and exposure to risk factors. Child work has different characteristics in each of the cities studied, which suggests that the solution to the problem must be designed on a case by case basis.
This article examines the use of outpatient mental health services in a sample of low-income women (Mothers' Well-Being Study MWS) and compares the findings with a sample of similar-aged women in the ...general population (National Comorbidity Survey NCS). Overall, the prevalence of any 12-month mental health disorder was significantly greater for MWS respondents compared with NCS respondents. Only a quarter of each sample with any disorder had received treatment at an outpatient mental health service in the past year. The authors identify implications for social workers who seek to improve access and quality of care for women in need of specialty mental health and substance use services.

Das GKV-Modernisierungsgesetz (GMG) hat erstmals für das Jahr 2004 die Datengrundlage für gezielte Vertragsanalysen der Krankenkassen im vertragsärztlichen Sektor geschaffen. Mit einer Gruppe ...repräsentativer Betriebskrankenkassen nutzte der BKK Landesverband Bayern die neue Datenbasis für die empirische Prüfung des Grundsatzes „ambulant vor stationär“. Dazu wurde die Entwicklung stationsersetzender Leistungen sowohl im strukturvertraglich geförderten ambulant-vertragsärztlichen als auch im Krankenhaussektor analysiert. Diese empirische Untersuchung steht im Mittelpunkt des Beitrags. Vorgelagert sind eine theoretische Anreizanalyse aus der Perspektive der gesetzlichen Krankenversicherung und die Beschreibung der Ausgabenentwicklung für den ambulanten und stationären Bereich auf GKV-Ebene seit dem Jahr 1991. Die empirische Studie ist ein Prä-Post-Vergleich für die Jahre 2004, 2005 und 2006. Sie bezieht sich auf sieben prospektiv ausgewählte Leistungsbereiche (Operationen Leisten- und Schenkelhernie, Katarakt, Varizen, Arthroskopie, Karpaltunnelsyndrom, Strahlentherapie und kurative Koloskopien) und umfasst für das Jahr 2006 über 40.000 Leistungen. Primäre Zielgröße ist die Entwicklung von Fallzahlen. Die unterschiedliche Kostenintensität der Leistungen wird durch eine DRG-Gewichtung berücksichtigt. Als Ergebnis zeigt sich eine Fallzahlsteigerung sowohl im vertragsärztlichambulanten als auch im stationären Bereich. Diese liegt kostengewichtet pro Jahr durchschnittlich bei 8,2%, und ist primär auf einen massiven Fallzahlanstieg im Krankenhaussektor zurückzuführen. Trotz Unschärfen und Einschränkungen hinsichtlich Methodik und Datenbasis weist das Ergebnis darauf hin, dass es in den betrachteten Jahren nicht gelungen ist, den Grundsatz „ambulant vor stationär“ mit dem Ziel der Beitragssatzstabilität konsequent umzusetzen.

Most caring is provided by family and friends out of love or duty. Some additional support to such carers can have a big effect, at relatively low cost. Workers to fill caregiving jobs can be found, as long as policy makers and employers take steps to improve the dismal image of caregiving as being low-paid, hard, and low-skilled. Providing adequate financial protection for those needing care is possible, in a way that does not unduly stretch public financing. But getting these policies right needs to start now, because the challenge to implementing sustainable, responsive and fair long-term care policies is only going to get bigger and bigger, as populations age. Learning from other countries' experiences, both good and bad, might save much money and grief.; This book is the result of a two-year project conducted between 2009 and 2010 by the OECD Health Division and Social Policy Division. The study points to key polices and strategies that can help address future demand for care and respond to the implications this will have for long-term care workforce and financing. It highlights examples of useful country experiences, but it also warns about the dearth of evidence on cost-effective policies in a number of areas, making a strong plea for advancing evidence-based research on long-term care (LTC)." Vorgestellt wird Marte Meo, ein niederschwelliges, ambulantes, videounterstütztes Beratungsangebot, um Eltern und andere wichtige Bezugspersonen von Kindern zu unterstützen, Erziehungskompetenz auf- ...und auszubauen sowie die Entwicklungsförderung und Unterstützung der Kinder in die eigenen Hände zu nehmen. Es wird ein kurzer Überblick über die Entwicklung der Methode gegeben; ihre Anwendung in der Jugendhilfe wird anhand von drei Fallvignetten illustriert. (ZPID).
