Azolium-2-thiocarboxylate zwitterion ligands have emerged as a promising class of compounds in the field of coordination chemistry due to their unique structural features and versatile applications. ...These ligands are characterized by a positively charged azolium ring and a negatively charged thiocarboxylate moiety, making them capable of forming stable coordination complexes with various metal ions. One of the key structural aspects that make these ligands attractive for coordination chemistry is their ability to adopt diverse coordination modes with metal centers. Grapevines cv Trebbiano d´Abruzzo grown in three vineyards in Teramo province, Abruzzi, Italy, severely affected by esca proper, were subjected to trunk renewal and thereafter treated with triazoles ...and fosetyl Al fungicides, applied either by trunk injection via syringe or by ground injection via injector pale. Trunk renewal, by cutting the trunk, made it possible to determine the extent and type of wood deterioration on the residual trunk portion, where it was found that rotted and discoloured wood often still remained. Fungicide treatment began in 1995, when trunk renewal was performed, and continued until 1998; after this vines were monitored for further 4 years until 2002. In 2002, vines that had received trunk injections after trunk renewal were in better vegetative condition than the ones without such injections, with full yield and lower mortality. Cyproconazole was particularly effective when trunk injected, but was not effective when injected into the ground. Moreover, the effect of the severity of observed wood necrosis at time of trunk renewal on treatment effectiveness was evaluated. The current lack of similar trials and studies in other vineyards makes standardisation of control methods difficult; positive effects may have been due to concurrent factors such as cultivar vigour or peculiar environmental or cultural conditions
Viti della cv Trebbiano d´Abruzzo coltivate in tre vigneti in provincia di Teramo, Abruzzo, affette gravemente dal mal dell´esca vero e proprio, sono state sottoposte al rinnovo del tronco e successivamente trattate con triazoli e fosetyl Al, somministrati sia per iniezione nel tronco con siringa, sia per iniezione nel terreno con palo iniettore. Il rinnovo del tronco, tramite capitozzatura, ha reso possibile la determinazione dell´ampiezza e del tipo di degradazione del legno sulla porzione residua dello stesso, dove si è riscontrato che spesso permaneva ancora legno marcescente e necrotico. Il trattamento con i fungicidi è iniziato nel 1995, quando è stato effettuato il rinnovo del tronco, ed è continuato fino al 1998; dopo tale epoca le viti sono state controllate fino al 2002. Nel 2002 le viti sottoposte a iniezioni al tronco sono risultate in condizioni vegetative migliori rispetto a quelle non trattate, con produzione piena e minore mortalità. Il ciproconazolo è risultato particolarmente efficace tramite iniezione nel tronco, mentre era inefficace con l´iniezione nel terreno. E´ stata inoltre valutato l´effetto della gravità della necrosi del legno al momento del rinnovo sull´efficacia del trattamento. L´attuale mancanza di ricerche simili in altri vigneti rende difficile la standardizzazione dei metodi di controllo; gli effetti positivi potevano essere dovuti a fattori concomitanti come il vigore della cultivar o condizioni ambientali e colturali particolari.
Literature o profesionalnom bolestima u veterinarskim stanicama za male životinje iznimno je malo, ali upućuje na niz zdravstvenih rizika koji valja uzeti u obzir, uključujući izloženost organskoj ...prašini. Cilj je ovoga pilot-istraživanja bio ocijeniti opterećenje organskom prašinom, bakterijama i gljivama u prostorijama jedne tipične portugalske veterinarske stanice, ali i utvrditi postoji li među gljivama rezistentnih na azole. Ove smo nalaze nadopunili nalazima rezistentnih bakterija u nosnom brisu. Mjerena je masena koncentracija čestica (PMC) u pet veličina (PM0.5, PM1, PM2.5, PM5, PM10), kao i njihov broj (PNC). Uzorci zraka uzeti su sa šest mjernih mjesta te prije i tijekom postupka čišćenja zuba mačaka na obradi. Uzorci su kultivirani na četirima medijima za bakterije i gljive. Za usporedbu je uzet i jedan uzorak vanjskoga zraka. Osim toga, uzorkovane su i površine na istim mjernim mjestima uzimanjem brisova i elektrostatskim krpama za prašinu. Čestice PM10 iskazale su najveće koncentracije na svim mjernim mjestima. Opterećenje gljivama kretalo se u rasponu od 88 do 504 CFU m-3. Aspergillus nigri otporan na azole identificiran je u jednom uzorku. Bakterijsko se opterećenje u zraku kretalo od 84 do 328 CFU m-3. Brisovi iz nosa 14 radnika u veterinarskoj stanici pokazali su iznimno nisku prevalenciju bakterije Staphylococcus aureus (7,1 %). Naši rezultati pokazali su da je kontaminacija organskom prašinom na većini mjernih mjesta bila viša od ograničenja Svjetske zdravstvene organizacije te upozorili na potrebu bolje ventilacije prostora. Buduća bi istraživanja trebala upotpuniti postojeće aktivno i pasivno uzorkovanje s molekularnim testovima, čime bi se dobila preciznija slika profesionalnog rizika. Što se prevencije tiče, kaveze sa životinjama trebalo bi držati u zasebnim prostorijama, a ne u onima u kojima se izvode postupci dijagnoze i liječenja, a radnici bi trebali nositi zaštitnu opremu cijelo vrijeme.

La résistance aux azolés est inhabituelle chez
Aspergillus sp. Nous décrivons le cas d´un patient ayant reçu un traitement à long terme par itraconazole et voriconazole pour une aspergillose ...intra-cavitaire bilatérale chronique, avec des aspergillomes dont les isolats d´Aspergillus fumigatus ont développé une résistance simultanée à l´itraconazole et au voriconazole. Une nouvelle mutation (G138C) du géne cible (cyp51A) codant pour la 14-déméthylase a été détectée. Le patient a présenté une réponse à l´administration intraveineuse de casponfungine, qu´il a reçu six fois par semaine, sans apparition de résistance en 9 mois.
La classe des médicaments antifongiques azolés présente un risque élevé d’interactions médicamenteuses (IAM) d’origine pharmacocinétique métabolique qui s’exercent au niveau des cytochromes P450, les ...CYP 3A4 principalement ainsi que 2C19, et/ou qui portent sur le transporteur d’efflux de la glycoprotéine P (Pgp). Les azolés sont généralement utilisateurs de ce système pour leur métabolisme et peuvent donc faire l’objet de ces IAM eux-mêmes (rifampicine, antiprotéase). Mais en tant qu’inhibiteurs du CYP 3A4 qui intervient dans le métabolisme de nombreux autres médicaments, ils sont à l’origine d’IAM modérées à sévères sur différentes cibles, notamment les immunosuppresseurs, inhibiteurs de la calcineurine (ciclosporine et tacrolimus) et du signal de prolifération (sirolimus et everolimus), les anticoagulants oraux et de nombreux autres. De mécanisme aujourd’hui connu, ces IAM sont prévisibles au plan qualitatif mais aussi au plan quantitatif et leur prise en charge, accessible à l’adaptation de dose, peut être corrigée et même anticipée. On observe une puissance d’IAM in vivo variable selon l’azolé (kéto- KTZ

Une fois évalué, leur ordre de grandeur peut guider l’adaptation individualisée des traitements, sur la base d’un suivi thérapeutique pharmacologique (STP) approprié s’appuyant sur la mesure des concentrations des médicaments impliqués. La mise en jeu de mécanismes intestinaux (CYP3A4 et Pgp) dans la production de ces IAM en modifie le développement selon la voie d’administration, la voie orale produisant des IAM d’intensité plus importante que par voie intraveineuse. De ce fait, une attention particulière et un réajustement adapté doivent être portés aussi bien aux relais entre substances qu’entre voies d’administration.
Azole antifungals are a therapeutic class known to have a high risk of drug-drug interactions (DDI), involving pharmacokinetic metabolic issues targeted on cytochromes P450 (3A4 and 2C19) and/or the efflux transport protein Pgp (glycoprotein P). Azoles are generally metabolised through these systems and may also be concerned by such DDI (rifampicin, protease inhibitors). But, as CYP3A4 inhibitors, an enzymatic metabolic pattern widely used by many drugs, azoles result in numerous moderate to severe DDI. Immunosuppressive drugs are especially concerned, i.e., tacrolimus and cyclosporine calcineurin inhibitors as well as everolimus and sirolimus proliferation signal inhibitors, oral anticoagulants and many others. The mechanism of these DDI is known, enabling us to predict both qualitative and quantitative aspects. The dosage should be adjusted very early as a corrective or preventive measure. DDI intensity decreases among azoles from ketoconazole (KTZ) to itraconazole (ITZ), voriconazole (VRZ), posaconazole (PSZ) and fluconazole (FCZ) and upon targets (rapamicin
>
tacrolimus
>
cyclosporine). The evaluation of their range magnitude helps individualize dose adjustment based on therapeutic drug monitoring (TDM) and on the concentration of the drugs concerned. The involvement of intestinal mechanisms (3A4, Pgp) in the development of such DDI influences the administration route. Expression is higher via the oral route than when the drug is administered intravenously. Indeed, particular attention should be paid to switching azoles and/or to changing the administration route.
Les flavohémoglobines (FlavoHbs) sont des protéines fixant l’oxygène qui possèdent un domaine globine N-terminal lié de manière covalente à un domaine C-terminal réductase contenant une flavine ...adénine dinucléotide (FAD) et un site de fixation du nicotinamide adénine dinucléotide (phosphate) (NAD(P)H). Ces protéines que l’on retrouve exclusivement chez les microorganismes possèdent une action NO dioxygénase et interviennent donc dans la défense des microorganismes contre les dommages générés par le NO. De par ce rôle essentiel de défense microbienne, les flavoHbs sont considérés comme des cibles attractives des antibiotiques. En particulier, les dérivés azolés ont montré la capacité d’inhiber la fonction NO dioxygénase des flavoHbs par un mécanisme inconnu. Afin de mieux comprendre le mode d’action de ces antibiotiques, nous avons entrepris une étude structurale, enzymatique et spectroscopique sur trois flavoHbs d’intérêt. Les structures tridimensionnelles de la flavoHb de R. eutropha (FHP) en complexe avec le miconazole, l’éconazole et le kétoconazole ainsi que celle de S. cerevisae (YHB) seule et en complexe avec l’éconazole ont été obtenues à des résolutions satisfaisantes permettant de décrire précisément les interactions entre la protéine et les inhibiteurs. Les structures ont révélé des réarrangements conformationnels importants selon la nature chimique de l’inhibiteur et la présence d’acides gras dans la poche de l’hème. Afin de comprendre le rôle fonctionnel des acides gras dans le cycle catalytique de l’enzyme, la structure tridimensionnelle de la FHP en complexe avec l’acide linolénique a été obtenue et des analyses enzymatiques et spectroscopiques ont montré l’importance des acides gras dans la modulation de l’activité de la protéine. Parallèlement, des études sur la FHP, la YHB et la flavoHb de S. aureus (Shb) ont permis de mieux appréhender le rôle des inhibiteurs dans le processus de transfert d’électrons au sein de la protéine.
Flavohemoglobins (FlavoHbs) are oxygen binding proteins which consist of a heme-globin domain fused with a ferredoxin reductase –like FAD and NAD-binding domain. FlavoHbs have been identified exclusively in microorganisms where they play a key role in defence against NO damages by using their NO dioxygenase activity. These proteins are therefore considered as targets for new antibiotic drugs. Recently, azole derivatives were proven to be attractive nitric oxide dioxygenase inhibitors by a mechanism that remains elusive. In order to explore their binding characteristics, we determined the X-ray structure of the flavoHb from Ralstonia eutropha in a complex with miconazole (FHPm), econazole (FHPe), and ketoconazole (FHPk) as well as the X- ray structure of S. cerevisae flavoHb in both ligand-free and econazole-bound forms. We describe the interactions between the protein matrix and the inhibitors in a comparative manner and how the bulky structures of the azole inhibitors dictate the profile of the hemebinding pocket and vice versa in flavoHbs.Although the azole compounds were able to push the lipid out of its binding site, a fatty acid fragment is still bound inside the heme pocket of FHPe and FHPk and dictates the state of the protein. To go further in the compréhension of the fatty acid function in the flavoHbs, we determined the three dimensionnal structure of FHP in complex with linolenic acid. Spectroscopic and enzymatic analyzis confirmed the important role of fatty acids in enhancing the protein activity. We also made studies to understand how azoles modulate the electron transfer process in the flavoHbs.

Histoplasma capsulatum et la coccidioidomycose sont des mycoses endémiques qui atteignent à la fois des sujets immunocompétents ou immunodéprimés, chez lesquels on observe ...fréquemment des formes disséminées. Leur incidence s'accroît avec celle de l'infection par le VIH et peut atteindre jusqu'à 30 % des sujets infectés par le VIH demeurant en zone d'endémie, le plus souvent lorsque le taux de lymphocytes CD4 est inférieur à 200/mm
3. L'histoplasmose africaine à
H. duboisii est plus rarement rapportée. Sa fréquence n'est pas augmentée au cours de l'infection par le VIH. Le traitement de ces mycoses repose dans les formes disséminées menaçantes sur l'amphotéricine B en première intention. Dans les autres formes, un antifongique azolé est utilisé d'emblée. Le kétoconazole n'a pas de place dans le traitement de ces mycoses au cours de l'infection par le VIH. Le fluconazole est particuliérement intéressant dans les formes neurologiques de ces mycoses, en particulier au cours de la coccidioidomycose, à des posologies probablement plus élevées que celles habituellement proposées. L'itraconazole est le traitement de choix de l'histoplasmose à
H. capsulatum du sujet immunocompétent et la prophylaxie secondaire de choix au cours du SIDA. Son efficacité semble également prometteuse au cours de l'histoplasmose à
H. duboisii. De nouveaux antifongiques efficaces dans ces mycoses sont également en développement.
Histoplasmosis due to
Histoplasma capsulatum and coccidioidomycosis are endemic mycoses which can be observed in both immunocompetent and immunosuppressed individuals. In these patients, disseminated diseases occur frequently. Their incidences are increasing with that of HIV infection. These opportunistic mycoses can be found in up to 30 % of HIV-infected patients in endemic areas, mostly when CD4-cell count falls below 200/mm
3. African histoplasmosis due to
H. duboisii has rarely been reported. Its incidence is not increased during HIV infection. The first line antifungal treatment still requires amphotericin B during severe disseminated diseases. In other cases, azoles can be used as initial therapy. Ketoconazole is not indicated for the treatment of these mycoses during HIV infection. Fluconazole is particularly useful in cases of neurological dissemination, probably at higher dosages than usually recommended. Itraconazole is the treatment of choice for
H. capsulatum infection occurring in immunocompetent individuals and the most efficient secondary prophylaxis during AIDS. Its efficacy is also promizing for the treatment of histoplasmosis due to
H. duboisii.
