    症例は72歳,男性.半年以上持続する下痢症状に加え食思不振,倦怠感の増強を認め,当院救急外来を受診した.血液検査で腎機能障害と電解質異常を伴う脱水症を認め,腹部造影CTでは直腸を主座とし骨盤内を占居する径12cmの腫瘍性病変を指摘された.緊急入院の上,輸液による脱水と電解質補正を行い,病理診断では一部に高分化腺癌を含む管状絨毛腺腫と診断された.入院中も腫瘍から1日1Lを超える粘液性下痢を認め,絨毛腺腫に随伴する電解質喪失症候群が示唆された.腫瘍を肛門側へ反転させることで骨盤腔に有効な視野を得て,D2郭清を伴う腹腔鏡下直腸切断術を施行した.電解質異常の再発を認めることなく,術後14日目に退院となった.直腸絨毛腫瘍は粘液性下痢による電解質異常と脱水症,および腺癌を合併することが多く,薬物治療・放射線治療も緩和治療としては選択肢となりうるが,充分な電解質異常の制御と腫瘍の根治を目指す場合は外科的な切除術を選択すべきである.
