La place du SUVmax en TEP-18F-FDG est controversée pour différencier le caractère bénin ou malin des incidentalomes parotidiens. Notre objectif était d’identifier des facteurs prédictifs de malignité ...afin d’optimiser la prise en charge du patient.
Nous avons inclus 28 patients présentant des incidentalomes parotidiens parmi les 10 756 examens TEP-18F-FDG réalisés en 3 ans. Pour chacun, nous avons relevé les données de l’IRM parotidienne, d’anatomopathologie et des imageries ultérieures. Nous avons comparé le SUVmax, SUVmean, MTV et TLG de chaque lésion par régression logistique. Les seuils ont été déterminés à l’aide des courbes ROC.
La prévalence des incidentalomes parotidiens était de 1,29 % (n = 139), parmi les 37 lésions incluses, 46 % étaient malignes. Les SUVmax, MTV et TLG étaient significativement plus élevés en cas de tumeur maligne (p ≤ 0,001). Après régression logistique, un MTV > 4 mL et un TLG > 7,5 étaient les paramètres les plus significativement associés à une lésion maligne (MTV et TLG : p < 0,001 ; SUVmax : p = 0,005 ; SUVmean : p = 0,07). D’après les courbes ROC, le MTV avec un seuil à 4 présente une AUC de 0,92 (Se 75 %, Sp 100 %) pour distinguer une lésion bénigne d’une maligne.
Le MTV et le TLG étaient des facteurs prédictifs de malignité plus pertinents que le SUVmax. Cependant, compte tenu de la forte proportion de lésions malignes et de la sensibilité suboptimale du MTV, un complément d’exploration doit être demandé devant tout incidentalomes parotidiens.
Parotid gland incidentalomas are unexpected hypermetabolic foci in the parotid region that can be found when scanning the whole-body by PET/CT. The role of SUVmax is controversial to differentiate benign or malignant tumours in this indication. The aim of this study was to determine PET/CT features of malignant parotid incidentaloma, in order to improve medical care.
We retrospectively reviewed 10,756 PET/CT scans performed during 3 years at our center in search of parotid gland incidentalomas. Twenty-eight patients were included. For each lesion, we have collected the results of parotid MRI, histology and/or subsequent imaging. Logistic regression was used to compare SUVmax, SUVmean, MTV and TLG, and ROC curves were used to assess threshold values.
Prevalence of parotid gland incidentalomas was 1.29% (n = 139). Thirty-seven were included, among them, 46% were malignant. SUVmax, MTV and TLG were significantly higher for malignant tumours (P ≤ 0.001). MTV > 4 mL and TLG > 7.5 mL were strongly associated with a malignant lesion (MTV and TLG: P < 0.001; SUVmax: P = 0.005; SUVmean: P = 0.07). According to ROC curves, MTV thresholded at 4 had an AUC of 0.92 (Se 75%, Sp 100%) to differentiate benign and malignant lesions.
MTV and TLG were predictive factors of malignancy, stronger than SUVmax. However, given the high proportion of malignant lesions and the suboptimal sensitivity of the MTV further exploration must be requested for any parotid gland incidentaloma.
Le ganglioneurome est une tumeur neurogène bénigne, rare, qui se développe à partir des cellules ganglionnaires sympathiques. Elle se localise essentiellement dans la région rétropéritonéale. La ...localisation surrénalienne est rare. Nous rapportons un cas de ganglioneurome surrénalien chez une femme âgée de 42ans, asthmatique et atteinte de dilatation des bronches. La tumeur est découverte fortuitement sur un scanner thoraco-abdominal demandé dans le cadre du bilan étiologique de sa dilatation des bronches. Le diagnostic a été confirmé par l’examen anatomopathologique.
Ganglioneuroma is a rare, benign, neurogenic tumor originating from the neural sheath. It is frequently located in the retroperitoneum. The localization in the adrenal gland is rare. We report a case of adrenal ganglioneuroma in a 42 year-old woman who had bronchiectasis and asthma. The tumor was discovered incidentally in the computed tomography indicated in the etiological assessment of her bronchiectasis. Our diagnosis was confirmed by histopathology.
Une patiente de 31 ans est suivie pour une cystinurie, justifiant des scanners réguliers. En 2006 une masse tissulaire de 3cm de la surrénale droite, homogène, mesurée à 3,5cm était notée. En 2007, ...cette masse présentait un caractère hétérogène. La pression artérielle était à 90/61mmHg, sans hypotension orthostatique. À l’interrogatoire, absence de malaise, de crise sudorale, de céphalée, de palpitation. L’aldostérone, le cortisol, les androgènes étaient normaux. La normétanéphrine urinaire et la somme des dérivés méthoxylés urinaires rapportés à la créatininurie étaient élevés. Un an plus tard, la patiente était toujours asymptomatique et normotendue. Les taux plasmatiques de normétanéphrine et de métanéphrine s’étaient majorés, devenant hautement pathologiques. La tumeur mesurait 5cm de grand axe et fixait isolément sur la scintigraphie au MIBG et le PET scan. Une surrénalectomie droite a été réalisée. Le phéochromocytome a été confirmé histologiquement. La particularité ici était l’absence de symptôme, en particulier d’hypertension artérielle. Cela concerne actuellement jusqu’à 47 % des cas, notamment dans le cadre d’explorations d’incidentalomes et de dépistage familial. En effet, 35 à 40 % des phéochromocytomes sont associés à une mutation germinale, justifiant une analyse génétique chez cette patiente. L’analyse génétique ne montrera ici aucune mutation associée au phéochromocytome. Le suivi à 6 ans montrait l’absence de récidive du phéochromocytome mais l’apparition d’une hypertension ! Les seules récidives documentées étaient... lithiasiques.
A 31-year-old patient was followed for cystinuria, justifying CT scans. In 2006, a tissue mass of 3cm of the right adrenal gland, homogeneous, measured at 3.5cm in 2007 was noted. Blood pressure was 90/61mmHg, without orthostatic hypotension. During the clinical interview, no discomfort, sweat attack, headache, or palpitation was reported by the patient. Hormonal assays did not favor a primary hyperaldosteronism, or a hypercortisolism. Adrenal androgens were normal. 24h normetanephrines urinary excretion and metanephrines to creatinine ratio were elevated. One year later, the patient remained asymptomatic and normotensive. Plasma levels of normetanephrine and metanephrine were increased and highly pathological. The tumor was 5cm long and fixed individually on MIBG scintigraphy and PET scan. Right adrenalectomy was performed. A pheochromocytoma was confirmed histologically. The peculiarity here was the absence of symptoms, especially arterial hypertension. This currently affects up to 47% of cases, particularly in the context of incidentaloma investigations and family screening. Indeed, 35 to 40% of pheochromocytomas are associated with a germline mutation, justifying a genetic analysis in this patient. Genetic analysis did not show any mutation associated with pheochromocytoma. At a 6-year interval pheochromocytoma did not reappeared while BP increased reaching hypertension threshold. The only documented recurrence was lithiasis.
La localisation surrénalienne isolée de la tuberculose représente moins de 2% des incidentalomes surrénaliens. C'est la cause infectieuse la plus fréquente des insuffisances surrénaliennes. Nous ...rapportons le cas d'un patient âgé de 53 ans, sans antécédents particuliers, qui présente un tableau d'insuffisance surrénalienne lente évoluant depuis six mois. L'examen physique n'a pas retrouvé de masse ni d'hépato-splénomégalie. La tension artérielle était à 120/60 mmHg. L'examen biologique n'a pas objectivé de syndrome inflammatoire et un taux de LDH normal. La TDM a objectivé une hypertrophie bilatérale des glandes surrénales siège de calcifications. L'intradermo réaction à la tuberculine était positive à 25mm. La recherche de BK dans les expectorations et dans les urines était négative. Le test au Quantiferon® était positif. Un traitement antibacillaire d'épreuve a été démarré avec une amélioration clinique avec prise de poids de 5kg en 12 mois. Les dosages hormonaux restent bas.
Les incidentalomes thyroïdiens sont rarement découverts lors d’examens TEP/TDM réalisés pour pathologies extrathyroïdiennes. Nous rapportons deux observations de cancers thyroïdiens papillaires ...découverts lors d’examens TEP/TDM réalisés dans le cadre de la prise en charge de lymphomes hodgkiniens. À travers ces deux observations, nous soulignons l’intérêt d’explorer tout foyer thyroïdien hypermétabolique visualisé sur TEP/TDM, par cytoponction à l’aiguille fine afin de déterminer sa nature qui peut être néoplasique.
The authors report the cases of two patients followed up for Hodgkin lymphoma, whose control PET/CT during chemotherapy revealed incidental papillary thyroid carcinoma as hypermetabolic thyroid foci. These incidental uptakes of 18F-FDG in the thyroid gland are rarely encountered in day-to-day practice of oncological PET/CT. However, they deserve an exploration to determine their etiology.
Les anomalies surrénaliennes sont fréquentes dans la population adulte et sont un challenge quotidien en médecine nucléaire. La finalité est de différencier les lésions bénignes des lésions malignes ...lorsque cela a un impact thérapeutique. Or la TEP/TDM au 18F-FDG fournit une combinaison de critères morphologiques et métaboliques permettant d’améliorer la spécificité intrinsèque de chacune des modalités. L’objectif de cette revue est de fournir quelques clés qui permettront d’identifier les lésions fréquentes ou typiques, mais aussi de proposer des éléments d’orientation et une conduite diagnostique adaptée devant des lésions plus atypiques.
Adrenal masses are common in adult population and are a daily challenge for nuclear physicians. The aim for physicians is to differentiate benign from malignant lesions when therapeutic management is impacted. 18F-FDG PET/CT combines complementary modalities, providing structural and functional information, superior to that provided by CT or PET alone. The purpose of this review is to provide some keys that will help to be familiar with the common as well as the atypical manifestations of adrenal disease.
Amaç: Adrenal insidentaloma (AI), manyetik rezonans görüntüleme (MRI) veya bilgisayarlı tomografi (BT) taramalarında farklı nedenlerle rastgele saptanan kitlelerdir. AI inflamatuar bir klinik durum ...değildir fakat proinflamatuvar özellikleri gösterilmiştir. Biz bu çalışmada AI ile nötrofil-lenfosit oranı (NLR) ve trombosit-lenfosit oranı (PLR) arasındaki ilişkiyi ve bu oranların klinik önemini ortaya koymaya çalıştık. Gereç-Yöntem: Ocak 2018 - Aralık 2019 tarihleri arasında AI nedeniyle Endokrinoloji kliniğinde takip ettiğimiz toplam 178 hasta retrospektif olarak tarandı. Sonuçlar: NLR ve PLR medyan (IQR; % 25-75) değerleri sırasıyla 2.21 (1.61; 1.58-3.2) ve 112.56 (57.31; 87.81-145.12) idi. Tümör yerleşimlerine göre; NLR medyan (IQR; % 25-75) değerleri; sağ taraflı 2.24 (1.72; 1.52-3.22), sol taraflı 2.08 (1.14; 1.63-2.77) ve bilateral 2.49 (2.61; 1.52- 4.13) olarak saptandı ve istatistiksel olarak anlamlı bir fark bulunmadı (p = 0.172). Benzer şekilde, PLR medyan (IQR; % 25-75) dğerleri; sağ taraflı 116.18 (61.1; 88.25-149.35), sol taraflı 102.87 (52.76; 85.01-137.77) ve bilateral 126.88 (66.7; 95.06-161.76) olarak saptandı ve istatistiksel olarak anlamlı bir fark bulunmadı (p = 0.158). Fonksiyonel AI’lar ve non-fonksiyonel AI’lar karşılaştırıldığında NLR ve PLR açısından istatistiksel olarak anlamlı bir fark yoktu (sırasıyla p = 0.357 ve p = 0.174). Ek olarak, NLR ve PLR sonuçları hipertansiyon (HT) varlığına göre karşılaştırıldı ve fark bulunmadı (NLR için p = 0.10, PLR için p = 0.32). Hemogram parametreleriyle ilişkili olarak incelenen faktörlerden sadece tümör büyüklüğü NLR ile düşük fakat pozitif korelasyon gösterdi (r = 0.199, p = 0,01). Tartışma: Bu çalışmada AI hastalarında NLR ve PLR'nin istatistiksel olarak anlamlı bir şekilde artmadığını saptadık. Ancak tümör boyutu ile NLR arasında pozitif korelasyon olduğu tespit edilmiştir. Genel olarak inflamatuvar hemogram parametrelerinin ucuz ve kolay ulaşılabilir olmasına rağmen AI hastalarında klinik bir öneminin olmadığı sonucuna varılmıştır.
Le FDG n’est pas adapté à la détection du cancer de la prostate. Cependant, une hyperfixation prostatique mise en évidence de façon inattendue lors de la TEP/TDM au FDG demandée pour une autre ...indication peut être l’élément révélateur d’un cancer de la prostate, même si une origine bénigne est plus fréquente. Nous rapportons 4 cas d’incidentalome prostatique décelé sur une hyperfixation du FDG, correspondant à 4 images et circonstances typiques : fixation diffuse, fixation focale avec divers niveaux de SUVmax et de concentration sérique du PSA dosée au vu de la TEP/TDM. Nous présentons une méta-analyse des 6 séries actuellement rapportées dans la littérature sur un total de 47 935 TEP au FDG : la fréquence moyenne de cet incidentalome est de 1,5 % ; il a été caractérisé dans 68 % des cas, correspondant à un cancer dans 16 % des cas caractérisés, un adénocarcinome dans 75 cas/78. Il n’y avait pas de corrélation entre le score de Gleason et le SUVmax ; des adénocarcinomes Gleason=6 peuvent être décelés avec le FDG. Parmi les facteurs de risque pour une origine maligne, on trouve un SUVmax>3, une localisation périphérique au sein de la prostate et l’absence de calcification dans la zone hyperfixante. La biopsie prostatique est utile dans la mesure où on envisage de modifier la prise en charge du patient si le cancer de la prostate est confirmé ; elle est alors indiquée en cas d’élévation franche de la concentration sérique du PSA mais aussi lorsque cette valeur est normale s’il existe un ou plusieurs facteurs de risque sur la TEP/TDM au FDG.
FDG is not suited for the detection of prostate cancer. However, high prostate uptake demonstrated unexpectedly on FDG PET/CT requested for another indication may lead to the detection of prostate cancer, although a non-malignant origin is more common. We report 4 cases of prostate incidentaloma detected on high FDG prostate uptake, corresponding to 4 typical images and circumstances: diffuse uptake, focal uptake with various levels of SUVmax and of serum PSA level assayed on the basis of PET/CT result. We performed a meta-analysis of the 6 series in the literature currently reporting characterisation of prostate incidentalomas, in a total of 47,935 FDG PET, the average frequency of this incidentaloma is 1.5%; it was characterised in 68% of cases, corresponding to cancer in 16% of characterised cases, adenocarcinoma in 75 cases/78. There was no correlation between the Gleason score and the SUVmax; adenocarcinoma Gleason=6 can be unexpectedly detected with FDG. Among the risk factors for malignancy, there is a SUVmax>3, a peripheral location within the prostate and no calcification in the hypermetabolic area. Prostate biopsy may be indicated only in case the management of the patient would be modified if prostate cancer is confirmed; it is prompted in the case of clear elevation of serum PSA level, but also when serum PSA level is normal but there is one or several risk factors on FDG PET/CT images.
