Autor souhlasí s názory profesora Kotulána na rizika chystaného programu Zdraví 2020 publikovanými v Hygieně č. 2/2015 a připomíná další aspekty, zejména skutečnost, že v hygienické službě, která by ...měla být významným aktérem realizace programu, dnes pracuje jen malý počet lékařů, kteří patří ke starší generaci. Zmiňuje hlavní zaměření práce lékařů v hygienické službě. Probírá pak zkušenosti s prevencí kuřáctví a s dalších rizikových faktorů a akcentuje preventivní aktivity vycházející ze specifických lokálních podmínek.
The author agrees with the opinions of Professor Kotulán apropos the pitfalls of the upcoming Health 2020 programme, as published in Hygiena no. 2/2015. He points out further related aspects, in particular the fact that there are very few old-school physicians active in the public health service which as such is to be a major contributor to the realisation of the programme. The commentary mentions the main specializations of doctors in public health, discusses experiences with prevention of smoking and other risk factors and emphasizes the importance of preventive activity based on specific local conditions.

V České republice se v roce 2011 diagnostikovalo 7076 nových případů karcinomu prsu (132 na 100 tisíc žen). U podstatné části z těchto pacientek se po onkologické léčbě daří dosáhnout dlouhodobé ...kompletní remise („vyléčení“), ovšem stále u nich hrozí jednak dlouhodobé komplikace způsobené onkologickou léčbou, jednak recidiva malignity. Dlouhodobá dispenzarizace pacientek s karcinomem prsu je oblastí zájmu nejen onkologů, ale i gynekologů, praktických lékařů a chirurgů. Klíčová slova: dispenzarizace – karcinom prsu – komplikace
In the Czech Republic 7076 new cases of breast cancer were diagnosed in 2011 (132 per 100,000 women). The majority of these patients will achieve long-term complete remission (“cure”) after cancer treatment. However, they still face the risk of long-term complications caused by cancer therapy as well the recurrence of malignancy. Long-term follow-up of patients with breast cancer is an area of interest not only for oncologists, but also for gynecologists, general practitioners, and surgeons. Článek pojednává o historii politiky podpory zdraví v ČR v období let 1990 až 2012. Cílem článku je 1) zachytit odlišné přístupy k politice podpory zdraví (a potažmo ke zdraví a determinantám zdraví) ...formulované vládami ve významných koncepčních materiálech a 2) na základě studia sekundárních dokumentů identifikovat významné události samotné implementace sledované politiky. Vládou vnímaná (resp. v koncepčních dokumentech deklarovaná) hodnota zdraví a konkrétní pojetí determinant zdraví, které, jak článek ukazuje, do značné míry souvisí s politickou orientací vlád, jsou nikoliv jedinými, ale významnými faktory následně realizované politiky podpory zdraví.
The paper deals with the history of health promotion policy in the Czech Republic during 1990–2012. The aims of the paper are as follows: 1) to reveal different approaches to health promotion policy (and to health and health determinants), defined by Czech governments in significant conceptual documents and 2) based on the study of secondary documents to identify significant events of the implementation of the policy under consideration. The value of health as perceived by the government (or more precisely declared in the conceptual documents) and the particular approach to health determinants which, as the article suggests, relates to the political leanings of the governments are some of the significant factors of subsequently implemented health promotion policy.
