Fractures of the medial portion of the clavicle occur infrequently in children and account for only about 5% of all pediatric clavicular fractures. In many males, a clavicular medial physis does not ...close completely until 24 to 26 years of age.1 In our case report, 17-year-old male was injured following a direct fall on shoulder. In the Emergency Department, we identified a medial clavicle physeal fracture with anterior dislocation after standard chest radiography and 'serendipity' view. In total intravenous anesthesia, we reduced the fracture and immobilized with Desault cast for 21 days and figure- of- eight dressing 3 weeks more. We didn't notice any functional deficit nor cosmetic defect.

Ugotavlja, v kolikšni ...meri slednja - novotarska gibanja avantgarde - zares prelamljajo s simbolistično tradicijo in v kolikšni meri gre v njunem odnosu za kontinuiteto in zaostritev skupnih literarno-umetniških in svetovno-nazorskih specifik, povezanih s tradicijo romantizma, na katerih sta osnovana oba modernistična pojava z začetka 20. stoletja.
Summary
A war wound to the proximal humerus is characterised by profuse bleeding, multifragmentary fracture-dislocations, defects of the muscle tissue, capsule, skin and by primary contamination with ...polymorphic bacterial flora, and it is rarely isolated.
Out of 27 injured people, subluxation/dislocation of the humeral head occured in the first four patients who suffered a proximal humeral fracture resulting from a warrelated trauma stabilized with an external fixator. Two pins were inserted into the humeral head, and the other two into the diaphysis. In the other wounded people, there was no subluxation/dislocation of the humerus because the pins were placed into the clavicle, into the acromion or spinous process of the scapula.
Out of the 27 patients treated for proximal humeral injuries, the contact was established with 11. Out of these 11 patients, only in one patient we did not use the clavicle, the acromion or spinous process of the scapula to stabilize the fracture. The result of the treatment was poor due to avascular necrosis of the head and ankylosis of the shoulder. In the ten remaining patients we used 'temporary' placement of a pin into the clavicle or into the acromion or spinous process of the scapula. The final outcome of the treatment was satisfactory. The average Constant score was 59 points.
There is little data in professional literature about injuries to the proximal humerus caused by war-related trauma. The most commonly used system of classification of peacetime traumas is the Neer or AO classification system as there are balanced algorythms in the protocol for treating the trauma.
Proximal humeral fractures should be stabilized with an external fixator, inserting one pin into the clavicle or into the acromion or scapular spine. This prevents the subluxation/dislocation of the humeroscapular joint and facilitates early mobilisation, and thus improves the clinical result and reduces complications.
72 years-old woman suffered an open fracture of the right tibia, as a pedestrian, when the bus tires crossed over the right foot. She was immediately transported to the Clinical Center Niš and ...following clinical examination, the amputation of the lower leg was advised due to extensive injury. On admission to the hospital (Clinic for Orthopedics and Traumatology), after resuscitation, arteriography of the blood vessels of the right leg was performed. Spasm of blood vessels of the right leg below the fracture was noted, but the circulation in the distal part of the lower leg and foot was preserved. After removing temporary immobilization and bandage of the lower leg, a large wound was noticed, from the toes to the popliteal crease. Primary treatment of the wound was done and tibial fracture was stabilized with external fixation (with convergent orientation of the pins). Almost the whole wound was left open, while the vital structures of the leg (the main blood vessels and nerves) were covered with local soft tissues that are adapted by situation sutures. Due to the large soft tissue destruction on the right lower leg and diabetic angiopathy, the patient was sent to special orthopedic hospital "Banjica" in Belgrade. After repeated wound debridements, external skeletal fixator was removed and the Ilizarov apparatus was placed. Soft tissue defect was covered by skin graft. In the postoperative period, patient was regularly dressed. Eight months later, the fractured tibia healed and the Ilizarov apparatus was removed. The patient was referred for rehabilitation. Following rehabilitation, patient returned to her work and everyday activities.
Pacijentkinja stara 72 godine zadobila je otvoreni konkvasantni prelom desne potkolenice gaženjem točkovima autobusa. Odmah je dovezena u KC u Nišu, gde je nakon kliničkog pregleda, zbog težine povrede predložena potkolena amputacija. Po prijemu u Kliniku za ortopediju i traumatologiju, nakon reanimacije, urađena je arteriografija krvnih sudova desne noge. Registrovan je spazam krvnih sudova desne potkolenice ispod mesta preloma, ali sa prisutnom cirkulacijom u distalnom delu potkolenice i stopala. Nakon skidanja privremene imobilizacije i zavoja na potkolenici, registrovana je velika rana koja se pružala od prstiju stopla do zatkolene jame. Urađena je primarna obrada rane, koja se pružala od prstiju stopala do kolenog zgloba. Prelom je stabilizovan spoljnim skeletnim fiksatorom sa konvergentnom orjentacijom klinova. Rana otvorenog konkvasantnog preloma velikim delom je ostavljena otvorena, dok su vitalne strukture potkolenice (magistralni krvni sudovi i nervi) pokriveni mekim tkivima, koja su situacionim šavovima adaptirana. Zbog velikog oštećenja mekih tkiva desne potkolenice prilikom gaženja i dijabetične agiopatije, radi nastavka lečenja, pacijentkinja je upućena u SOHB "Banjica" u Beogradu koja raspolaže barokomorom. Nakon ponovljenih debridmana rane, spoljni skeletni fiksator je skinut i plasiran aparat po Ilizarovu. Defekt mekih tkiva pokriven je transplantatom po Tirsch-u. U postoperativnom toku bolesnica je redovno previjana. Nakon osam meseci od povređivanja, došlo je do zarastanja preloma i aparat po Ilizarovu je skinut. Bolesnica je upućena na rehabilitaciju. Po završenoj rehablitaciji, bolesnica se vratila svojim radnim i životnim aktivnostima.

It is estimated ...that only half of those patients who were completely independent before fracture, becomes able to walk without aids, and even one-fifth must be permanently placed in institutions for the care of the elderly. The aim of this study was to determine the efficacy of the applied rehabilitaion proram on the recovery of patients older than 65 years after surgically treated hip fracture and to compare the effects of different rehabilitation programs. This prospective study was conducted on 203 patients admited on rehabilitation after operated hip fracture. Input parameters were gender, age, type of fracture, type of surgery, time since surgery and start of rehabilitation, duration of rehabilitation and comorbidity measured by Cumulative Illnese Rating Score for Geriatrics (CIRS-G). Functional recovery was evaluated by Berg Balance Scale (BBS) and the motor part of Functional Independance Measure (FIM). The quality of life was evaluated by SF-36v2 Health Survey. Values were recorded at admission, at discharge, 3 and 6 months after discharge. Patients were divided into two groups, one group that was treated in the rehabilitation program including balneotherapy and Group 2, in which the patients were treated without balneotherapy. The type of rehabilitation program determined the treating physician specialis of phzsical medicine and rehabilitation. Statistical analysis was preformed using Student's t test, Mann-Whitney U test, unifactorial ANOVA test, Pearson correlation. The results showed that all patients had a statistically significant improvement in functional recovery measured by the BBS and a motor FIM at discharge, and that this improvement took place, and after 3 and 6 months. Patients in group 1 showen at admission, discharge, and after 3 and 6 months statistically significantly better resoults than patients in group 2. Considering that the two groups except for the type of rehabilitation treatment differed according to the degree of co-morbidity (patients in Group 1 had fewer associated diseases) and by age (patients in Group 1 had a significantly lower average age) we tested significance and strength of the correlation of those three parameters with functional recovery and quality of life. The results showed that all three parameters significantly correlated with the functional recovery of patients and improving the quality of life and rehabilitation treatment showing the highest correlation, then the degree of comorbidity expressed by CIRS-G total score, and then the age. During the present study we found out that patients in Group 2 on folow up after 3 and 6 months show minimal progress, in the parameters of functional recovery and quality of life for most of the parameters of the SF-36 questionnaire, in contrast to the period during inpatient rehabilitation. Based on the present results we can conclude that investigated rehabilitation treatment showed high efficiency in all patients and therefore must be mandatory. Patients included in balneotherapy program show significantly better results in functional recovery and all the parameters of quality of life than patients who were not involved in this program of rehabilitation. The better recovery in the balneotherapy group was also due to the lower total CIRS-G and age.- Prelomi kuka predstavljaju veoma značajan medicinski problem, kako za pojedince tako i za čitav zdravstveni sistem. Procenjuje se da samo polovina pacijenata od onih koji su pre preloma bili potpuno nezavisni, biva osposobljeno do nivoa hoda bez pomagala, a čak jedna petina mora biti trajno smeštena u ustanove za brigu o starima. Cilj ovog rada je da da se utvrdi efikasnost primenjenog rehabilitacinog tretmana na opravak pacijenata starijih od 65 godina nakon operativno lečenog preloma kuka i da uporedi efekat različitih programa rehabilitacioniog tretmana. Ova prospektivna studija je sprovedena na 203 pacijenta stacionarno rehabilitovana nakon operativno lečenog preloma kuka. Ulazni parametri su bili pol, godine starosti, vrsta preloma, vrsta operacije, vreme proteklo od operacije do početka rehabilitacije, dužina trajanja rehabilitacije i komorbiditet meren kumulativnom skalom za gerijatrijsku populaciju (Cumulative Illnese Rating Score for Geriatrics CIRS-G) a praćeni su finkcionalni oporavak je vrednovan Bergovom skalom balansa (Berg Balance Scale BBS) i motornim delom testa funkcionalne nezavisnosti (motor Functional Independance measure FIM) dok je kvalitet života praćen preko upitnika SF-36. Vrednosti su beležene na prijemu, pri otpustu, 3 i 6 meseci nakon otpusta. Pacijenti su podeljeni u dve grupe, grupa 1 koja je bila tretirana u okviru rehabilitacionog programa balneoterapijiom i grupa 2 u kojoj su pacijenti bili tretirani bez balneoterapije. Vrstu rehabilitacionog programa je određivao ordinirajući fizijatar. U statističkoj obradi su korišćeni studentov T test, Mann-Whitney U test , unifaktorijelan ANOVA test, Pearsonova korelacija. Rezultati su pokazali da su svi pacijenti imali visoko statistički značajno poboljšanje u funkcionalnom oporavku merenom BBS-om i motornim FIM-om na kraju terapije i da se ovo poboljšanje održalo i nakon 3 i 6 meseci. Pacijenti iz grupe 1 su posmatrano na prijemu, otpustu, nakon 3 i 6 meseci bili visoko statistički značajno bolji od pacijenata iz grupe 2. Obzirom da su se ove dve grupe osim po vrsti rehabilitacionog tretmana razlikovali i po stepenu komorbiditeta (pacijenti iz grupe 1 su imali manje pridruženih bolesti) i po godinama starosti (pacijenti iz grupe 1 su imali statistički značajno niži prosek godina) ispitana je i značajnost i jačina korelacije ova tri parametra sa funkcionalnim oporavkom i poboljšanjem kvaliteta života. Dobijeni rezultati su pokazali da sva tri parametra visko statistički značajno koreliraju sa funkcionalnim oporavkom pacijenta i unapređenjem kvaliteta života i da rehabilitacioni tretman pokazuje najjaču korelaciju, zatim stepen komorbiditeta izražen CIRS-G total skorom, a zatim godine starosti. Tokom istraživanja utvrđena je i činjenica da pacijenti iz grupe 2 na kontrolama nakom 3 i 6 meseci u parametrima funkcionalnog oporavka i većini parametara kvaliteta života iz upitnika SF-36, pokazuju minimalan napredak, koji nije statistički značajan, za razliku od perioda tokom stacionarne rehabilitacije. Na osnovu izloženih rezultata možemo zaključiti da je ispitivani rehabilitacioni tretman pokazao visoku efikasnost kod svih pacijenta i da zato mora biti sastavi deo procesa lečenja. Pacijenti uključeni u program balneoterapije pokazuju znatno bolje rezultate u funkcionalnom oporavku i svim parametrima kvaliteta života, od pacijenata koji nisu bili uključeni u ovakav rehabilitacioni program. We describe the first results of the GPS-observation processing in three successive GPS-campaigns from 1996, 1999, and 2002 of the Large Geodynamic Network of the Velenje Coal Mine. We present site ...velocities relative to the stable Eurasian plate, from which we derive the tectonic intepretation of active movements along the faults of the Periadriatic Fault System (PFS). As the PFS is a major post-collisional feature of the Alpine orogen, the study is interesting from regional point of view, as well as for intepreting the causes of earthquake activity in the wide area of the Velenje Coal Mine (in the Šaleška Valley region). Site velocities indicate predominately N- to NNEdirected movements in a range from 0.5 to 2mm/yr, which is consistent with the idea about the ongoing eastward extrusion of the Eastern Alpine domain at the rate of ~1.5 mm/yr, and the ~2mm/yr northward moving Adriatic microplate. PFS acts as the southern boundary of the extruding East Alpine domain, however, the deformation is not tied to a single dextral corridor, but is distributed over a wide area. First results have opened new questions, due to which the geodynamic network was expanded and measured in summer 2003 for the first time and will be reoccupied this September. Results from the expanded network will hopefully provide better understanding of active kinematics of the Šoštanj, Smrekovec and Labot faults.
The study focuses on Bronislav Wellek, Adolph Donath, Camill Hoffmann, Oskar Wiener and Otto Hauser and their translations and articles on modern Czech literature published in the German journal Aus ...fremden Zungen (1891–1910). This fortnightly established by Joseph Kürschner dealt exclusively with foreign literature. Its openness to all kinds of literary currents and little-known minor literatures was unique within the system of German literary journals. A comparison indicates the key role played by mediators. The primary requirement for their activity was multilingualism, and it turns out that in the case of all the figures under consideration, language did not necessarily have the nationally symbolic significance ascribed by politics, and their identity basically did not depend on their use of one language or another. The progress and success of this mediation was not just decided by the literary standard of the translation, but all five translators were concerned to improve the standard of translation, as well as to change the approach to translation in general. The choice of text and its succeeded publication was determined by the involvement of the mediators in the Czech and German literary context. Hence Bronislav Wellek focused primarily on translations and interpretations of poetry by Jaroslav Vrchlický and J. S. Machar, whom he knew personally and who also introduced him to their own literary and intellectual circles. It was thanks to Vrchlický that he printed his translations in Eduard Albert’s anthologies Poesie aus Böhmen (1893–1895). Machar was responsible for other translations, e.g. Hilbert’s drama Vina, which Hermann Bahr was to help promote in the German-language sphere. Between 1896 and 1898, Adolph Donath attempted to build up his position as mediator between the Moderní revue circle and some representatives of Viennese modernism (e.g. Karl Kraus, Peter Altenberg and Felix Rappaport). For Aus fremden Zungen he translated an impressionistic short story by Luisa Ziková Bílý květ (White Flower). In a fivepart series and an accompanying study modern Czech prose (J. Zeyer, V. Mrštík, F. X. Svoboda, J. K. Šlejhar and R. Svobodová) was presented by Camill Hoffmann and Oskar Wiener. Their mediation activities were based on direct relations with Prague Czech authors of the youngest generation, on whom Hoffmann also focused in his articles for other German journals, though in Aus fremden Zungen he was concerned with surveying the modernist work of the 1890s. Samples of Czech prose were followed on by translations from modern Czech lyric poetry (O. Březina, A. Sova, K. Hlaváček and V. Dyk) written by Otto Hauser. This polyglot and translator from numerous languages was a different kind of Viennese mediator. In contrast to Wellek and Hoffmann, Hauser was only in sporadic correspondence with Sova and Březina. His literary assessment of their poetry was based on his broad knowledge of contemporary European poetry, including minor literatures (such as Danish, Dutch and Croatian) and as such is of importance. These examples show that literary mediation is a complex rapprochement of variously defined and construed contexts, which have their reflected and unrealized limits.
