Background. In addition to clinical picture, US, CT and peritoneal lavage, laparoscopy has been increasingly used in the diagnosis and operative treatment of abdominal trauma. Diagnostic laparoscopy ... enables the surgeon to adopt optimal treatment strategies in patients with intraabdominal injuries. Case report. A 56-year-old patient fell from a ladder 3 meters high, sustaining injuries to the head chest and abdomen. There were symptoms of peritoneal irritration, and US disclosed free fluid in the abdominal cavity. Laparoscopy confirmed the presence of bile in the abdominal cavity. Concomitant retroperitoneal injuries were excluded by laparotomy, and cholecystectomy of the ruptured gallbladder was done. Conclusion. Diagnostic laparoscopy plays a key role in the management of abdominal trauma. It allows for proper and timely treatment by laparoscopy or, if decided, by laparotomy.
