  • Uporaba bioloških zdravil v revmatologiji = Use of biologic disease modifiying drugs in rheumatology

    In the past ten years the availability of biologic diseasemodifying drugs radically changed the way wemanage many rheumatic diseases. Tumor necrosisfactor alpha inhibitors, interleukin 1 and 6 ... inhibitors,and B cell depleting agents are all used in rheumatology.Biologics are routinely used in treatment ofrheumatoid arthritis, spondyloarthritides, and recentlyalso in ANCA vasculitides. Based on case reportsand open label trials the biologics areoccasionally used off-label in the treatment of othersystemic vasculitides, systemic connective tissuediseases and autoinflammatory conditions. Currently,the biologics are not usually recommendedas first line drugs. Two most obvious reasons mightbe their high price and their unknown safety recordafter prolonged exposure as they affect fundamentalimmunologic processes. The biologics are usuallyprescribed when insufficient reduction of disease activity is achieved with synthetic immunomodulatorydrugs or these drugs are contraindicatedor patient experienced adverse events.
