Highlights • MSI is detectable in about 15% of CRCs. • MSI can be found both in sporadic and hereditary CRCs. • Lynch syndrome is the most frequent cause of heritable CRC. • In early-stage CRCs, MSI probably can predict a favourable prognosis and resistance to 5-FU. • In early clinical trials with anti-PD-1, MSI-H colorectal cancer showed impressive results.
