Ghrelin is an acylated peptide, whose lipid modification is essential for its biological activities. Previous studies demonstrated that it strongly stimulates GH release and has a potent orexigenic ...action. Meanwhile, there is enough evidence showing that feeding states influence plasma ghrelin levels. Fasting stimulates ghrelin secretion, and feeding reduces plasma ghrelin levels. In this study we examined the regulation of plasma ghrelin by fasting in genetically obese animals considering its molecular forms. Plasma levels of active form of ghrelin as well as those of total ghrelin were reduced in ob/ob and db/db mice compared with those in their control mice. Zucker fatty (fa/fa) rats also showed lower plasma ghrelin levels by fasting than the control rats. Insulin-induced hypoglycemia, however, stimulated ghrelin secretion in the fasted fatty rats. Moreover, glucose injection was revealed to reduce plasma ghrelin levels in rats. The effect of the severity of obesity on secretory regulation of ghrelin was also studied. Older fatty rats showed low plasma ghrelin levels even after 48-h fasting. These data suggest that the short-term secretory regulation of total ghrelin and the active form of ghrelin is delayed in obese animals and that blood glucose levels may be involved in the delayed regulation.
Our previous studies using microsatellite markers near or in the TSH receptor (TSHR) gene revealed significant association between autoimmune thyroid disease (AITD) in Japanese patients and TSHR ...microsatellite alleles. In the present study, we performed a case-control analysis of AITD using single-nucleotide polymorphisms (SNPs) spaced 3–50 kb apart spanning the TSHR gene. We observed significant associations between AITD/Graves’ disease (GD)/Hashimoto’s thyroiditis and multiple SNPs. Specifically, the SNP JST022302 and several adjacent SNPs in intron 7 of the TSHR gene were significantly associated with GD (P = 0.039–0.0004) but not Hashimoto’s thyroiditis. Furthermore, we identified three haplotype blocks around intron 7 by linkage disequilibrium analysis. A single SNP haplotype AATG(CT)6(TT)AG in the haplotype block including JST022302 showed significant association with GD in haplotype case-control analysis (P = 0.0058). These findings suggest that alleles of intron 7 of the TSHR gene contribute to GD susceptibility.
The synergistic relationship between GH-releasing secretagogue (GHS) and GH-releasing hormone (GHRH) with respect to GH secretion is well known. In the present study, we report a similar relationship ...between GHRH and ghrelin, a recently identified endogenous ligand for the GHS receptor. In normal male adults, various doses of ghrelin were intravenously administered alone or together with 1.0 microg/kg GHRH. At small doses of 0.08 and 0.2 microg/kg ghrelin, combined administration of the two peptides significantly stimulated GH release in a synergistic manner; the mean GH response values of the two peptide combinations were more than the summed mean GH response values of each peptide alone (P < 0.05). In addition, at 1.0 microg/kg ghrelin, the tendency of the synergistic effect was observed, although the comparison was not statistically significant probably due to a submaximal dose ceiling effect. No synergistic effects with respect to ACTH or prolactin secretion were observed. In conclusion, the synergistic interaction between ghrelin and GHRH was clearly shown and might be useful for a provocation test to diagnose GH deficiency.

Previous studies demonstrated that fasting plasma ghrelin levels were high in anorectic ...patients, suggesting ghrelin may play an important role in the pathophysiology of anorexia nervosa. However, antibodies used in previous work to measure ghrelin concentrations in human blood do not distinguish between the active form of ghrelin (active ghrelin) and desacyl ghrelin with no biological activities. Therefore, we studied plasma levels of active ghrelin during oral glucose tolerance test (OGTT) in anorectic patients, using a radioimmunoassay (RIA) specific for active ghrelin.
Active ghrelin response to OGTT was evaluated in five female anorectic patients and seven age-matched control females. All subjects were given a 75 g/225 ml glucose solution orally after overnight fasting. For RIA of active ghrelin, 1 N hydrogen chloride was added to the samples at final concentration of 0.1 N immediately after separation of plasma.
Plasma basal levels of active ghrelin were significantly higher in anorectic patients than in controls (52.1+/-10.5 vs 21.2+/-3.1 fmol/ml, P<0.01). They were significantly decreased during OGTT in anorectic patients and in controls, reaching a nadir of 49.0+/-7.7% and 57.3+/-4.5% of the basal levels, respectively.
These results suggest that hyperghrelinemia in anorectic patients is caused at least partly by increased secretion of active ghrelin and that glucose ingestion suppresses active ghrelin release in these patients.
The diagnosis of thyroid diseases is mainly divided into three categories: morphological, functional and etiological diagnoses. Great progress has recently been seen in each field. For example, in ...the functional diagnosis, improvement of sensitivities and development of non-radioimmunoassays have been achieved in the measurement of hormone level. Especially, improvement of thyrotropin (TSH) measurement has greatly contributed to precise evaluation of thyroid function. In the etiological diagnosis, marked improvements have been made by genetic analyses. Mutations of genes of hormones, their receptors and other proteins related to thyroid function have been revealed as causes of various thyroid disorders, such as cretinism, hyperthyroidism and tumors. Immunological analyses also showed significant progress, i.e., improvement of anti-thyroglobulin and anti-thyroid peroxidase antibody measurements. It is also necessary for all three diagnostical approaches to be improved concomitantly in the future.
Autoimmune thyroid diseases(AITD), which include Graves' disease(GD), Hashimoto's thyroiditis(HT), primary hypothyroidism with blocking-type anti-thyrotropin receptor antibody and idiopathic ...myxedema, are believed to be a multifactorial disease with a significant genetic and environmental component. Several genetic factors associated with AITD susceptibility have been tentatively identified, including the HLA genes and the cytotoxic T lymphocyte associated-4(CTLA-4) gene. More recently, as part of a genome scan, five additional chromosomal locations, i.e., chromosome 5, 8, 9, 15 and X, have recently been reported in a Japanese population. In the present study, in order to confirm evidence obtained by the genome scan, we performed an association study regarding these five candidate loci identified as regions where susceptible genes of AITD exist. Five microsatellite markers, which are located in these loci, were genotyped in a set of 487 unrelated Japanese AITD patients and 218 Japanese controls. Markers located in chromosome 5, 15 and X showed association in AITD patients with significant increase in particular alleles (p < 0.01), while markers in chromosome 8 and 9 did not consistently show significant association. These findings partly support evidence by the previous genome scan that attempted to identify loci of susceptible genes of AITD in a Japanese population; presence of an AITD susceptibility locus at 5q31-33 was suggested. Genotyping using other markers located in these loci would be helpful not only to confirm regions of AITD susceptible genes but also to narrow the regions.
Autoimmune thyroid disease (AITD) is caused by an immune response to self-thyroid antigens and has a significant genetic component. Antisense RNA transcripts have been implicated in gene regulation. ...Here we have identified a novel zinc-finger gene, designated ZFAT (zinc-finger gene in AITD susceptibility region), as one of the susceptibility genes in 8q23–q24 through an initial association analysis using the probands in the previous linkage analysis and a subsequent association analysis of the samples from a total of 515 affected individuals and 526 controls. The T allele of the single-nucleotide polymorphism (SNP), Ex9b-SNP10 located in the intron 9 of ZFAT, is associated with increased risk for AITD (dominant model: odds ratio=1.7, P=0.000091). The Ex9b-SNP10 falls into the 3′-UTR of truncated-ZFAT (TR-ZFAT) and the promoter region of the small antisense transcript of ZFAT (SAS-ZFAT). In peripheral blood lymphocytes, SAS-ZFAT is exclusively expressed in CD19+ B cells and expression levels of SAS-ZFAT and TR-ZFAT seemed to correlate with the Ex9b-SNP10-T-associated ZFAT-allele, inversely and positively, respectively. The Ex9b-SNP10 is critically involved in the regulation of SAS-ZFAT expression in vitro and this expression results in a decreased expression of TR-ZFAT. These results suggested that the SNP-associated ZFAT-allele plays a critical role in B cell function by affecting the expression level of TR-ZFAT through regulating SAS-ZFAT expression and that this novel regulatory mechanism of SNPs might be involved in controlling susceptibility or resistance to human disease.
